Genetic identification of a subgroup of depressed patients.

HLA antigens, blood groups and red cell enzyme polymorphisms were studied in patients with different types of affective disorders and in patients suffering from cycloid psychoses. Several statistically significant results were found. In particular, a subgroup of depressed patients characterized by a very extreme distribution of haptoglobin groups was identified. The implications of this last finding for the definition of a particular subgroup of depressed patients is discussed.

Belly Dancer's Dyskinesia or Functional Movement Disorder: Involuntary Abdominal Movements in a Pediatric Patient.

Belly dancer's dyskinesia (BDD) is a rare movement disorder marked by involuntary, rhythmic, undulating movements of the abdominal wall, often mistaken for gastrointestinal or psychiatric conditions. We describe the case of a 13-year-old girl with a seven-year history of abdominal spasms that stopped during sleep and resumed on waking. The episodes were accompanied by loud "whooshing" sounds from the abdomen. Her medical history included primary ciliary dyskinesia and a ventricular septal defect. She had never received medication for her symptoms. On examination, the right side of her upper bo

Comparative efficacy and safety of daridorexant, lemborexant, and suvorexant for insomnia: a systematic review and network meta-analysis.

BACKGROUND: In order to appraise the risk-benefit balance of the three available dual orexin receptor antagonists (DORAs; daridorexant, lemborexant, and suvorexant) for the management of adults with insomnia, we conducted a systematic review and random-effects model network meta-analysis. METHODS: Included were all published double-blind, randomized, placebo-controlled trials of these agents. Outcomes included subjective time to sleep onset at month 1 (sTSO, primary), subjective total sleep time at month 1 (sTST, co-primary), subjective wake after sleep onset at month 1, Insomnia Severity Ind

Innovation landscape in nanobiosensors for COVID-19 detection and future implications for a resilient Global Health Security.

Benzodiazepines have historically been the mainstay of treatment for sleeping disorders, yet they have many shortcomings. A new group of sedative hypnotic agents has been developed for this purpose. Similar to the benzodiazepines, zaleplon, zolpidem and zopiclone have activity at the GABA receptor complex, yet they appear to have more selectivity for certain subunits of the GABA receptor. This produces a clinical profile that is more efficacious with fewer side effects. Zaleplon, zolpidem and zopiclone are structurally distinct. Due to variation in binding to the GABA receptor subunits, these

The Stress Acceleration Hypothesis of Nightmares.

Adverse childhood experiences can deleteriously affect future physical and mental health, increasing risk for many illnesses, including psychiatric problems, sleep disorders, and, according to the present hypothesis, idiopathic nightmares. Much like post-traumatic nightmares, which are triggered by trauma and lead to recurrent emotional dreaming about the trauma, idiopathic nightmares are hypothesized to originate in early adverse experiences that lead in later life to the expression of early memories and emotions in dream content. Accordingly, the objectives of this paper are to (1) review ex

Assessment of feasibility of actigraphy as a measure of clinical change in response to an experimental interventional treatment in adolescents and adults with autism spectrum disorder.

OBJECTIVE: The use of actigraphy as a continuous experimental measure of clinical change was explored through a comparison of two clinical studies in autism spectrum disorder (ASD). The data quality, implementation ease, wear compliance, and clinical outcome correlation of actigraphy as a measure were assessed. METHODS: Two clinical studies were conducted and used as a basis of comparison: (1) AUT2001, a Phase 2A interventional study in ASD (N=63), and (2) AUT0002, a Phase 0 non-interventional study in typically-developing (TD) participants (N=53). Participants in both studies wore a wrist-ba

Orexin pathway modulation in narcolepsy: from Autoimmune mechanisms to nanotechnology enabled therapeutic delivery.

Narcolepsy is a long-term brain disorder caused by disrupted orexin signaling. Orexin neurons sit in the hypothalamus and control the sleep-wake cycle; damage to these neurons leads to narcolepsy. Early evidence pointed to an autoimmune attack on orexin neurons as the main cause. Current treatments - stimulants, antidepressants, sodium oxybate, and scheduled naps - ease symptoms but do not stop the disease, and they often cause side effects such as anxiety, insomnia, and heart problems. Most of these drugs also cannot cross the blood-brain barrier (BBB) to act directly on the hypothalamus. Thi

The role of circadian rhythms in the pathogenesis of myopia.

Myopia is a complex ocular disorder arising from the interaction of genetic predisposition and environmental cues that regulate eye growth. Increasing evidence indicates that the circadian timing system plays a critical role in ocular development and refractive homeostasis. The retina, choroid, and sclera possess intrinsic molecular clocks that generate rhythmic oscillations in gene expression, neurotransmitter release, and tissue physiology. Disruption of these ocular circadian rhythms has been implicated in abnormal axial elongation through pathways involving dopamine and melatonin signaling

Obstructive Sleep Apnea and Role of the Diaphragm.

Obstructive sleep apnea (OSA) causes multiple local and systemic pathophysiological consequences, which lead to an increase in morbidity and mortality in patients suffering from this disorder. OSA presents with various nocturnal events of apnoeas or hypopneas and with sub-clinical airflow limitations during wakefulness. OSA involves a large percentage of the population, particularly men, but the estimate of OSA patients could be much broader than data from the literature. Most of the research carried out in the muscle field is to understand the causes of the presence of chronic nocturnal desat

Do Disruptions in the Circadian Timing System Contribute to Autonomic Dysfunction in Huntington's Disease?

Huntington's disease (HD) patients suffer from a progressive neurodegenerative disorder that inflicts both motor and non-motor symptoms. HD is caused by a CAG repeat expansion within the first exon of the huntingtin (HTT) gene that produces a polyglutamine repeat that leads to protein misfolding, soluble aggregates, and inclusion bodies detected throughout the body. Both clinical and preclinical research indicate that cardiovascular dysfunction should be considered a core symptom in at least a subset of HD patients. There is strong evidence for dysautonomia (dysfunctional autonomic nervous sys

Charles Bonnet syndrome in cranio-maxillofacial surgery: case report.

BACKGROUND: Complex visual hallucinations in the presence of a clear mental state and in the absence of underlying neurological disorders have received increased recognition since Charles Bonnet reported visual hallucinations secondary to mature cataracts in 1760. The prevalence of Charles Bonnet syndrome (CBS) varies widely in the literature and might be underestimated in most settings. The current paper presents a case of acute-onset CBS due to a unilateral Frost suture after revision of an orbit floor fracture. CASE REPORT: A 68-year-old male patient underwent an operation to repair a unil

A patient with anti-IgLON5 disease associated with cerebral hemorrhage, multiple ischemic strokes, and kidney failure: case report.

INTRODUCTION: Anti-IgLON5 disease is a recently discovered, rare autoimmune disorder of the nervous system associated with autoantibodies against IgLON5. It manifests with sleep disorders, bulbar syndrome, gait instability, cognitive impairment, and/ or movement disorders. Survival rate and lower long-term disability progression depend on the early initiation of immunotherapy. CASE PRESENTATION: We describe a male patient with therapy-resistant anti-IgLON5 disease associated with sleep apnea, limb tremor, cerebral hemorrhage, epileptic seizures, psychiatric symptoms, multiple ischemic strokes

Cancer and depression in middle-aged and elderly Chinese: The mediating role of sleep quality and the moderating role of social participation diversity - Evidence from CHARLS 2018.

Depression is highly prevalent among individuals with cancer, yet the pathways remain incompletely understood. This study utilized cross-sectional data from the 2018 wave of the China Health and Retirement Longitudinal Study. Cancer was identified from harmonized and health files. Depression was assessed using the 10-item Center for Epidemiologic Studies Depression Scale. Sleep quality was proxied by a single item, which captures one dimension of sleep disturbance. Social participation diversity was constructed from 6 types of activities and summarized as a variety index. Covariates included a
