Smartphone video-based early diagnosis of blepharospasm using dual cross-attention modeling enhanced by facial pose estimation.

Blepharospasm is a focal dystonia characterized by involuntary eyelid contractions that impair vision and social function. The subtle clinical signs of blepharospasm make early and accurate diagnosis difficult, delaying timely intervention. In this study, we propose a dual cross-attention deep learning framework that integrates temporal video features and facial landmark dynamics to assess blepharospasm severity, frequency, and diagnosis from smartphone-recorded facial videos. A retrospective dataset of 847 patient videos collected from two hospitals (2016-2023) was used for model development.

Assessing the Implementation Fidelity of a Patient-Centered, Combination Adherence Strategy to Support HIV Antiretroviral Therapy and Pre-Exposure Prophylaxis Adherence During Pregnancy and Breastfeeding in Malawi.

The use of antiretroviral drugs has been proven effective in preventing and treating HIV; however, adherence remains a challenge. In two parallel pilot studies, we implemented a combination adherence intervention involving patient-centered counseling and adherence supporter training, tailored for HIV treatment (ART) or prevention (PrEP) during pregnancy and breastfeeding. The intervention included Integrated Next Step Counseling (iNSC), focused on well-being and medication adherence, with an option to involve a social support member. Independent auditors assessed implementation fidelity for 15

An atypical case of Creutzfeldt-Jakob disease mimicking frontotemporal dementia: genotypic influence and clinical implications.

We report an atypical case of Creutzfeldt-Jakob Disease (CJD) mimicking Frontotemporal Dementia (FTD) in a 68-year-old male. The patient initially presented with an anxious-depressive syndrome, progressing over 29 months to include dysexecutive syndrome, stereotyped speech, inertia, social withdrawal, verbal fluency impairments, and marked dyspraxia. Diagnostic imaging revealed signal alterations on MRI, while CSF analysis showed elevated T-TAU, neurofilament light chain (NfL), and glial fibrillary acidic protein (GFAP) levels. A second-generation RT-QuIC (SG-RT-QuIC) confirmed prion disease,

Trajectories of adolescents living with vertical transmission-HIV virus.

This study analyzes the life stories of eight young people living with vertically transmitted HIV in order to understand their perceptions marked by secrecy and silence regarding their experiences with the virus. Conducted at a specialized outpatient clinic in Rio de Janeiro, the study adopted circle dance, a Complementary and Integrative Health Practice, as a care tool. Using a qualitative approach and Life History as a data collection method, the focus was to understand the youth's experiences related to the disease and the crucial aspects of their lives. Three girls and five boys participat

Strategies to Improve Access to Care for Patients With Opioid Use Disorder.

Treatment of opioid use disorder (OUD) faces several challenges, including restricted access to medications, geographical and logistical barriers, and variability in treatment availability across different communities. This article outlines several strategies aimed at improving access to medications. Pharmacy-based care could potentially extend access to medications but would require regulatory changes to empower pharmacists. In addition, telemedicine has shown promise in improving access by mitigating geographic and transportation barriers. Mobile health clinics also offer a direct approach t

Increased reactivity of the paraventricular nucleus of the hypothalamus and decreased threat responding in male rats following psilocin administration.

Psychedelics have experienced renewed interest following positive clinical effects, however the neurobiological mechanisms underlying effects remain unclear. The paraventricular nucleus of the hypothalamus (PVN) plays an integral role in stress response, autonomic function, social behavior, and other affective processes. We investigated the effect of psilocin, the psychoactive metabolite of psilocybin, on PVN reactivity in Sprague Dawley rats. Psilocin increased stimulus-independent PVN activity as measured by c-Fos expression in male and female rats. Psilocin increased PVN reactivity to an av

Reducing tobacco and nicotine use among women in treatment for substance use disorder: evaluation of the knit to quit program.

INTRODUCTION: Tobacco and nicotine use are prevalent in residential substance use disorder (SUD) treatment programs that serve pregnant and parenting women. This study evaluated a group intervention that integrates knitting instruction, psychoeducation, and social support to improve readiness to quit and reduce tobacco and nicotine use among this population. METHODS: Clients and staff in four residential SUD treatment programs were assigned to a six-week group intervention or a wait-list control group. Intervention implementation and preliminary effectiveness were assessed using surveys, inte

Epidemiological surveillance of neglected tropical diseases in silent areas: the case of zoonotic sporotrichosis.

A common practice in the analysis of the spatial distribution of neglected tropical diseases is to assume that in silent areas (no reports) there are no cases. However, when the problem is underreporting, it risks reinforcing the neglect of areas that should be a priority. Instead of this assumption, one can predict the number of cases in silent areas using epidemiologic and spatial dependence information. The present study exemplifies this approach, using the integrated nested Laplace approximation in Bayesian spatial models that relate social vulnerability and the number of reported cases of

Dyslexia Data Consortium Repository: A Data Sharing and Delivery Platform for Research.

Specific learning disability of reading, or dyslexia, affects 5-17% of the population in the United States. Research on the neurobiology of dyslexia has included studies with relatively small sample sizes across research sites, thus limiting inference and the application of novel methods, such as deep learning. To address these issues and facilitate open science, we developed an online platform for data-sharing and advanced research programs to enhance opportunities for replication by providing researchers with secondary data that can be used in their research (https://www.dyslexiadata.org). A Plea for Diversity in Eating Disorders Research.

Eating disorders (EDs) are often stereotyped as affecting the SWAG, that is, as affecting mostly skinny, White, affluent girls. Over the last decade, however, significant progress has been made toward increasing diversity in ED research. There is consensus that EDs affect individuals of all genders, ages, sexual orientations, ethnic, and socio-economic backgrounds, with recent studies exploring social determinants of ED etiology, ED presentation, and developing diversity-affirming ED assessments. This article provides a brief summary of current developments related to diversity as a research t

Medicated corn feeders to disinfest cattle fever ticks, Boophilus (Boophilus) microplus (Acari: Ixodidae), from a suburban population of white-tailed deer, Odocoileus virginianus (Cervidae).

Following its eradication from the USA, the cattle fever tick, Rhipicephalus (Boophilus) microplus (Canestrini), a vector of bovine babesiosis, has made episodic incursions into, and sometimes beyond, an established barrier zone separating tick-free from endemic areas. In large part the incursions involve hosting and transport by wild ungulates, particularly deer and antelope. One approach to disinfest ticks from wild hosts is with food baits medicated to stop parasites. The approach has had mixed success due to factors that have been previously identified with supplemental feeding of wildlife

The First 40 Years of AIDS: Promising Programs, Limited Success.

HIV-prevention program planning, implementation, and evaluation began in the United States shortly after reports of a mysterious, apparently acquired, immune deficiency syndrome appeared in summer 1981. In San Francisco, New York City, and elsewhere, members of LGBT communities responded by providing accurate information, giving support, and raising money. During the first decade of the AIDS pandemic (1981-1990), social and behavioral scientists contributed by designing theory-based and practical interventions, combining interventions into programs, and measuring impact on behavior change and

Considerations for the Design of Pre-exposure Prophylaxis (PrEP) Interventions for Women: Lessons Learned from the Implementation of a Novel PrEP Intervention.

Pre-exposure prophylaxis (PrEP) uptake among women in the United States has been low. To increase uptake, we developed a peer outreach and navigation PrEP intervention. Semi-structured qualitative interviews with 32 cisgender women and 3 transgender women were conducted to assess the intervention. We used a thematic approach to identify barriers to, and facilitators of the intervention. Facilitators included interest in PrEP, offer of health and social services, the intervention's women-focused approach, and peer outreach and navigation. Barriers were perceived HIV risk, concerns about medicat

Challenges of caregiving to neurological patients.

A substantial number of neurological diseases lead to chronic impairment of activities of daily living (ADL) and physical or mental dependence. In Austria, homecare is provided mostly by female family members. Moreover, mainly female personnel, in the majority from southern and eastern European countries, contributes to care. Dependence and need for care vary between neurological diagnoses and accompanying diseases. Caregiver burden (CB) depends on patient- and caregiver-related and external factors, such as integrity of a family network, spatial resources, and socioeconomic factors. Depending

Managing Acute Seizures: New Rescue Delivery Option and Resources to Assist School Nurses.

Approximately 470,000 children and adolescents in the United States have epilepsy, 30% of whom experience seizures despite antiseizure drug regimens. School nurses, teachers, caregivers, and parents play integral roles in implementing a care plan that avoids triggers, recognizes signs, and provides supportive care-ideally, guided by a patient-specific seizure action plan, which may include the use of rescue medication. Benzodiazepines are the mainstay of seizure rescue medication; for decades, rectally administered diazepam was the only approved rescue medication for seizure clusters outside t

On the benefits and costs of extrinsic emotion regulation to the provider: Toward a neurobehavioral model.

Emotion regulation often takes place within interpersonal relationships. Prior research has focused mainly on the impact of extrinsic emotion regulation (EER) on the recipient. Yet EER may also have emotional and physical consequences for the provider. Understanding who benefits from helping others regulate their emotions and under what conditions is crucial in understanding the mechanisms that reinforce well-being and social ties. This conceptual review integrates existing literature into an interim working model of the benefits and costs of EER for the provider and of the underlying neural m

Telehealth transition in a comprehensive care unit for eating disorders: Challenges and long-term benefits.

The 2019 novel coronavirus disease (COVID-19) pandemic has forced many eating disorder medical stabilization units to consider adjustments that uphold both the quality of care delivered to patients while also observing social distancing public health directives for patients and staff. To date, inpatient facilities for eating disorders (both medical stabilization units and higher level of care facilities) have not needed to consider how to translate services to electronic platforms, given that most of these programs have in-person staff. We outline our transition to telehealth broadly, emphasiz

Reimagining Research with Pregnant Women and Parents Who Consume Cannabis in the Era of Legalization: The Value of Integrating Intersectional Feminist and Participatory Action Approaches.

Research on women who consume cannabis has predominantly focused on the perinatal period whereby cannabis consumption is problematized, linked to negative perinatal outcomes, and related to substance use and mental health challenges. When this historical literature and research about cannabis consumption is considered through a sociolegal and intersectional lens, questions emerge about how cannabis legalization may benefit and harm women, particularly women who experience marginalization along various axes of identity such as gender, race, and class. Questions also emerge about how women who c
