The role of ventricular remodeling in the early decompensation of cardiorenal syndrome: Insight from studies with Ren-2 transgenic hypertensive rats subjected to volume overload induced using aorto-caval fistula.

The aim of the present study was to evaluate the role of the left ventricle (LV) remodeling in the process of the transition from the compensation to the decompensation phase of cardiorenal syndrome. Ren-2 transgenic rats (TGR) with aorto-caval fistula (ACF) were used as the model of cardiorenal syndrome. Two weeks after ACF creation or sham operation, heart morphological parameters, cardiac structure and function were assessed by echocardiography and invasive pressure-volume analysis. This time point was chosen because two weeks after ACF the TGR still exhibit 100% survival rate and are in th

The pathophysiology of wild-type transthyretin amyloidosis: implications for exercise intolerance.

Wild-type transthyretin amyloidosis (ATTRwt) is an age-associated protein misfolding disorder marked by extracellular deposition of transthyretin fibrils within the myocardium, peripheral nerves, lungs, blood vessels, and musculoskeletal tissues. ATTRwt typically presents as a restrictive cardiomyopathy with preserved left ventricular ejection fraction that contributes to the development of hallmark symptoms such as exercise intolerance. Amyloid infiltration severely impairs ventricular compliance and diastolic reserve, limiting stroke volume and cardiac output augmentation during exercise, an

TGF-β1 Is Associated with Left Ventricular Dysfunction.

There are many contradictory opinions, and the role of TGF-β1 in the vascular effects of atherosclerosis remains unclear. This study aims to verify whether plasma TGF-β1 concentrations are correlated with changes in echocardiographic and vascular parameters in individuals with early coronary artery disease (CAD), including those with type 2 diabetes mellitus (T2DM). The study group consisted of 100 patients with early-onset CAD. Patients underwent echocardiography and electrocardiography. The thickness of the internal and middle membrane complex of the carotid and brachial arteries, the ankle-

Review of Atrioventricular Node Ablation Combined with Permanent His-Purkinje Conduction System Pacing in Patients with Atrial Fibrillation with Heart Failure.

With the advancement of pacing technologies, His-Purkinje conduction system pacing (HPCSP) has been increasingly recognized as superior to conventional right ventricular pacing (RVP) and biventricular pacing (BVP). This method is characterized by a series of strategies that either strengthen the native cardiac conduction system or fully preserve physical atrioventricular activation, ensuring optimal clinical outcomes. Treatment with HPCSP is divided into two pacing categories, His bundle pacing (HBP) and left bundle branch pacing (LBBP), and when combined with atrioventricular node ablation (A

Heart transplantation in neonatal Marfan syndrome: Saving life in a rare and fatal condition.

BACKGROUND: Neonatal Marfan syndrome is a rare disease with mortality in the first year of life reported as high as 95% predominantly due to progressive heart failure from valvar regurgitation and cardiomyopathy. Multisystem involvement and uncertain prognosis have historically precluded transplant candidacy, and current management options are of limited success. CASE REPORT: We present a baby girl with a postnatal diagnosis of neonatal Marfan syndrome who at 1 year of age underwent mitral valve and tricuspid valve repair with postoperative profound left ventricular and moderate right ventric

Coronary-pulmonary artery fistula with lung hypoplasia and a bicuspid aortic valve: A case report.

BACKGROUND: With an incidence of less than 1%, a Coronary Artery to Pulmonary Artery fistula (CAPF) is a rare coronary anomaly that causes heart failure. It causes a left to right cardiac shunt. While guidelines favor surgical correction in symptomatic patients, we present a challenging case with multiple cardio-thoracic pathologies. CASE PRESENTATION: We present a 38-year-old obese male with persistent atrial fibrillation (AF). He presented to our hospital in decompensated heart failure and AF with rapid ventricular response. He was found to have a CAPF, a bicuspid aortic valve and left lung

Cardiovascular Magnetic Resonance in Peripartum Cardiomyopathy: Comparison with Idiopathic Dilated Cardiomyopathy.

BACKGROUND: Peripartum (PPCM) and dilated (DCM) cardiomyopathies are distinct forms of cardiac disease that share certain aspects in clinical presentation. AIM: We hypothesized that different cardiac structural changes underlie PPCM and DCM, and we aimed to investigate them with cardiovascular magnetic resonance (CMR). METHODS: We included 21 PPCM patients (30.5 ± 5.9 years) and 30 female DCM patients (41.5 ± 16.8 years) matched for left ventricular ejection fraction. Biventricular and biatrial volumetric and functional parameters were assessed along with ventricular and atrial strain indice

Anthracyclines in a patient with acute leukemia and severe cardiomyopathy requiring mechanical support: A case report.

INTRODUCTION: For young adult patients with acute leukemia, both the efficacy and cardiotoxicity of anthracycline-based regimens have been documented. We report the case of a patient with severe cardiomyopathy, mechanically supported by a left ventricular assist device (LVAD), who subsequently developed Philadelphia-chromosome positive acute lymphoblastic leukemia (Ph + ALL). To our knowledge, this is the first report of anthracycline administration in a patient with heart failure requiring mechanical support. CASE REPORT: Our 27-year-old female patient was diagnosed with Ph + B-ALL as part o

Risks and challenges of implantable cardioverter-defibrillators in young adults.

The clinical use of the implantable cardioverter-defibrillator (ICD) is well established to prevent sudden death in patients with left ventricular dysfunction due to coronary artery disease and dilated cardiomyopathy, and its use has saved thousands of lives. More recently, its use has been extended to other patients at risk for sudden cardiac arrest due to ventricular fibrillation: patients with structural heart diseases such as hypertrophic cardiomyopathy and arrhythmogenic right ventricular dysplasia and patients with normal cardiac anatomy and function except for electrophysiologic abnorma

The effects of QRS duration and pacing sites on the acute hemodynamic changes during right ventricular pacing.

BACKGROUND: Has been reported that patients exhibiting prolonged paced QRS duration tend to have more serious heart disease, and the paced QRS duration can be an effective indicator of impaired left ventricular function. However, the acute and chronic hemodynamic effects of paced QRS duration and pacing sites during right ventricular (RV) pacing remain unknown. METHODS: A total of 14 patients who underwent electrophysiologic study for paroxysmal supraventricular tachycardia were examined. RV pacing was performed at 10 different sites with cycle lengths of 600 ms and 500 ms utilizing a 6-7F de

Comparative value of transthoracic and transesophageal echocardiography in the assessment of congenital abnormalities of the atrioventricular junction.

Information obtained from transthoracic and transesophageal echocardiography (two-dimensional echocardiography with spectral Doppler and color flow imaging) was compared in 17 patients with major congenital abnormalities of the atrioventricular (AV) junction (10 discordant AV connections, 1 criss-cross connection, 5 absent right connections and 1 absent left connection). The findings by either technique were correlated with findings at cardiac catheterization (12 patients) and at surgery (5 patients). In two of six patients with an absent AV connection as defined by transthoracic echocardiogra

Fetal surgery for open neural tube defect with severe ventriculomegaly.

OBJECTIVE: Prenatal open neural tube defect (ONTD) repair is performed to decrease the risk of needing treatment for hydrocephalus after birth and to preserve motor function. Some centers may not consider patients to be candidates for surgery if severe ventriculomegaly is present and there is no expected benefit in risk for hydrocephalus treatment. This study sought to compare the postnatal outcome of fetuses with ONTD and severe ventriculomegaly (ventricular width ≥ 15 mm) that underwent prenatal repair with the outcome of fetuses with severe ventriculomegaly that underwent postnatal repair a

Noninvasive Assessment of Right Ventricular-Pulmonary Arterial Coupling in Repaired Tetralogy of Fallot by Magnetic Resonance Imaging.

BACKGROUND: Right ventricular (RV) to pulmonary arterial coupling (RVPAc) quantifies RV contractility in relation to its afterload but traditionally requires high-fidelity catheter derived pressure-volume loops. We sought to evaluate a noninvasive volume-based RVPAc parameter in children and adults with repaired tetralogy of Fallot in relation to pulmonary regurgitation (PR), RV outflow tract obstruction (RVOTO), and exercise capacity. METHODS: We retrospectively studied 92 pediatric and 105 adult patients with repaired tetralogy of Fallot who had cardiovascular magnetic resonance imaging and

Late clinical features of patients with pulmonary atresia or critical pulmonary stenosis with intact ventricular septum after biventricular repair.

BACKGROUND: We aimed to reveal late clinical features of patients with pulmonary atresia with intact ventricular septum (PA/IVS) or critical pulmonary stenosis (cPS) after biventricular repair (BVR) based on preoperative right ventricular (RV) end-diastolic volume (RVEDV) findings. METHODS: Since 1985, 23 of 73 patients with PA/IVS (n=22) or cPS (n=1) with a tripartite RV and without major sinusoidal communication underwent BVR with a hybrid approach. The mean age and weight at BVR were 1.4±2.1 years and 6.9±5.9 kg, respectively. Mean follow-up was 10.1±6.4 years (range, 1.1 to 24.6 years).

Right ventricular pacing and the risk of heart failure in implantable cardioverter-defibrillator patients.

BACKGROUND: Right ventricular (RV) pacing in implantable cardioverter-defibrillator (ICD) patients may have detrimental effects on morbidity and mortality, in particular by inducing heart failure (HF). OBJECTIVE: We investigated whether RV pacing increases the risk of HF in an asymptomatic ICD population. METHODS: We evaluated all patients without symptomatic HF who received an ICD. The primary endpoint was the occurrence of HF, which was defined as new HF, hospitalization for HF, or death due to HF. The secondary endpoint was appropriate shocks. RESULTS: The study population consisted of 4

Arrhythmogenic Cardiomyopathy.

Arrhythmogenic cardiomyopathy is an inherited heart muscle disorder, predisposing to sudden cardiac death, particularly in young patients and athletes. Pathological features include loss of myocytes and fibrofatty replacement of right ventricular myocardium; biventricular involvement is often observed. It is a cell-to-cell junction cardiomyopathy, typically caused by genetically determined abnormalities of cardiac desmosomes, which leads to detachment of myocytes and alteration of intracellular signal transduction. The diagnosis of arrhythmogenic cardiomyopathy does not rely on a single gold s
