Evaluating datopotamab deruxtecan (Dato-DXd) as a novel treatment option for EGFR-mutated non-small cell lung cancer.

Datopotamab deruxtecan (Dato-DXd) is a TROP2-directed antibody-drug conjugate (ADC) that has received accelerated approval in the US for adults with locally advanced or metastatic EGFR-mutated non-small cell lung cancer (NSCLC) after progression on EGFR-directed therapy and platinum chemotherapy. In pooled analyses from TROPION-Lung01 and TROPION-Lung05, Dato-DXd achieved an objective response rate of ~45% and a median duration of response of 6.5 months, which compares favorably with historical outcomes with docetaxel. Dato-DXd is being evaluated in combination with osimertinib in the first-li

Gestational diabetes mellitus and maternal-infant microbiome axis: mechanistic insights and therapeutic interventions.

BACKGROUND: Gestational diabetes mellitus (GDM) alters maternal metabolism and the gut microbiota, thereby significantly affecting neonatal health. This narrative review synthesizes current clinical and translational evidence on temporal changes in the maternal gut microbiota across the three pregnancy trimesters in women with GDM and examines its subsequent impact on neonatal microbiome composition, immune development, and long-term disease risk. MAIN BODY: GDM-associated dysbiosis has been reported early in pregnancy, but human evidence remains heterogeneous and does not establish direct ve

Designing catalysts and interfacial environments for CO2 reduction toward C3+ products.

The electrochemical CO2 reduction reaction (CO2RR) yielding C3+ products offers an attractive route for producing valuable long-chain fuels and chemicals. However, direct CO2-to-C3+ conversion remains substantially challenging because it requires concerted control over complex processes involving CO2 activation, types and accumulation of C1/C2 intermediates, multiple C-C couplings, and product desorption. Increasing reports demonstrate that catalysts and interfacial environments collectively govern CO2 reduction pathways, whereas general design principles and strategies are lacking. This revie

Ambulatory venoarterial ECMO via right subclavian artery and internal jugular vein cannulation: a novel technical approach to bridge-to-transplant.

Standard femoral veno-arterial extracorporeal membrane oxygenation (V-A ECMO) severely limits patient mobility. We introduce a novel "femoral-free" Ambulatory Subclavian-Jugular V-A ECMO technique to maximize rehabilitation capabilities. We employed a subclavian-right internal jugular configuration. Initially, a vascular graft anastomosis was used for arterial return, but this resulted in upper limb swelling. Subsequently, we modified our approach as a technical refinement: inserting a 17-Fr reinfusion cannula directly into the right subclavian artery with a 6-Fr distal perfusion catheter to p

TroFuse-011 study design: sacituzumab tirumotecan with or without pembrolizumab for advanced triple-negative breast cancer with PD-L1 CPS <10.

Patients with advanced TNBC with PD-L1 CPS < 10 are typically treated with chemotherapy and experience poor outcomes. Sacituzumab tirumotecan (sac-TMT; MK-2870/SKB264) is a trophoblast cell-surface antigen 2 (TROP2)-directed antibody-drug conjugate with a unique, bifunctional linker that maximizes payload delivery to tumor cells. Combining sac-TMT with immunotherapy may improve outcomes given their complementary respective direct cytotoxic and immune-mediated antitumor effects regardless of tumor PD-L1 expression. The TroFuse-011 study evaluates sac-TMT with/without pembrolizumab versus treatm

Malic enzyme 1 senses L-lactate to determine tumor heterogeneity.

L-lactate is generally elevated in tumors and acts as a signaling molecule that promotes tumor progression. Here, we reveal that malic enzyme 1 (ME1) functions as a previously unrecognized sensor of L-lactate through direct binding at arginine 155 (R155), thereby potentiating malignancy. Mechanistically, L-lactate binding promotes the nuclear translocation of ME1, a process involving reduced acetylation at lysine 362 (K362) and facilitated by nuclear import of karyopherin-α 4 (KPNA4). Nuclear accumulation of ME1 enhances metastatic potential, which is correlated with increased interaction with

Emerging Diagnostic Tools and Therapeutic Approaches for Liver Cirrhosis Management.

Liver cirrhosis, a chronic liver disease identified by the formation of regenerative nodules along with fibrous bands, poses significant challenges in diagnosis and treatment. In the world, the most common factors of cirrhosis are Hepatitis C Virus (HCV), alcoholic liver disease, and Non-Alcoholic Steatohepatitis (NASH). The disease develops from a presymptomatic phase to a symptomatic phase, which often results in hospitalization, poor quality of life, and high mortality. The treatment of liver cirrhosis is centred on the treatment of the causes and complications, and liver transplantation is

When biology meets polarity: Toward a unified framework for sex-dependent responses to magnetic polarity in living systems.

The human body emits a bioelectromagnetic field primarily generated by the electrical activity of the heart, with additional contributions from the brain, muscles, and peripheral nerves. These endogenous fields are not isolated and can be modulated by external electromagnetic and magnetic influences. Current evidence suggests that the main mechanisms underlying such interactions include modulation of ion channels, radical pair dynamics, and ion cyclotron resonance. Several studies report sex-specific differences in responses to magnetic exposure. The main factors implicated in these difference

The Incremental Role of Stress Echocardiography in Valvular Heart Disease: A Narrative Review.

Background/Objectives: The diagnosis and risk stratification of valvular heart disease have traditionally relied on resting echocardiography. However, in a significant portion of patients, resting findings do not fully reflect the hemodynamic severity of the condition, particularly in asymptomatic individuals with severe valvular disease or those with nonspecific symptoms. In this context, stress echocardiography emerges as a vital imaging modality, providing a dynamic assessment of valvular, ventricular, and pulmonary function under hemodynamic load (from physical exercise or pharmacological

Antithrombotic therapy in patients with atrial fibrillation: unresolved issues and future directions.

Antithrombotic therapy is highly effective for stroke prevention in patients with atrial fibrillation (AF) but increases bleeding. Direct oral anticoagulants are preferred over vitamin K antagonists for many patients with AF because they provide similar stroke protection with less intracranial bleeding, and greater convenience, yet important knowledge gaps remain. In this review, we examine the limitations of current AF risk scores for predicting stroke, bleeding, and net benefit; identify AF populations in whom vitamin K antagonists remain preferred over direct oral anticoagulants, including

Integrative transcriptomics and reverse network pharmacology prioritize DPP4 and Achyranthes bidentata in Barrett's esophagus.

Barrett's esophagus (BE) is a precursor of esophageal adenocarcinoma, but candidate expression markers and preventive hypotheses require evaluation across patients, cohorts, and evidence levels. We analyzed two GPL96 discovery cohorts comprising 15 matched normal-BE pairs, used pair-aware differential modeling and resampling, and rebuilt four external BE-tissue assessments from official GEO files. The primary analysis yielded 1,085 differentially expressed genes; LASSO selected 13 features, linear SVM-RFE selected 6, and pair-bootstrap Random Forest retained 20. DPP4 and CCRL2 were shared by t

Bacterial neurotoxins: The snake venoms of the microbial world-a clinical and bioengineering approach.

The central and peripheral nervous systems constitute highly organized and dynamic networks that rely on precise regulation of synaptic transmission, membrane integrity, cytoskeletal stability, and intracellular signaling. Disruption of these tightly controlled processes can lead to a wide range of neurological disorders. Among the various modulators of neural function, bacterial neurotoxins represent a unique class of exotoxins that selectively target key neuronal components with remarkable molecular specificity. Traditionally studied in the context of infectious pathogenesis, bacterial neuro

Overcoming barriers in cancer therapy with oncolytic adenoviruses: Engineering strategies and clinical perspectives.

Oncolytic adenoviruses (OADVs) have emerged as promising therapeutics for cancer treatment, offering tumour-selective replication and potent antitumor effects. These genetically engineered viruses infect and lyse cancer cells while simultaneously activating antitumor immunity. OADV can be engineered with therapeutic genes and tumour-specific promoters, further enhancing their specificity and efficacy. Various researchers have explored the use of OADV in cancer treatment, integrating direct oncolysis with immune activation, hence revealing promising therapeutic effects in preclinical studies. T

Current Management and the New Paradigm for ALK+ NSCLC After CROWN.

The 7-year update of the phase 3 CROWN study is the most impressive milestone in anaplastic lymphoma kinase (ALK)+ non-small-cell lung cancer (NSCLC) since the discovery of the oncogene 20 years ago and inaugurates an entire new paradigm. The progression-free survival (PFS) plateau at a high level > 50%, with an estimated very low annual progression rate < 2% after the third year, suggests that lorlatinib can inactivate the disease in most patients. The complete abrogation of on-target resistance abolishes ALK tyrosine kinase inhibitor (TKI) sequencing and will seriously impact future drug dev

Neoadjuvant Therapies for Prostate Cancer-Current Paradigms and Future Directions.

High-risk and locally advanced prostate cancer represents 20-25% of new diagnoses of prostate cancer and is associated with high rates of recurrence, morbidity, and mortality. The neoadjuvant window provides a unique opportunity for systemic control prior to definitive therapy with radical prostatectomy or radiotherapy (RT). Early trials with first-generation androgen deprivation therapy (ADT) achieved pathological downstaging but no survival benefit. In the 2000s, the advent of chemohormonal regimes using docetaxel provided excitement but mixed results tempered expectations and is now not rec
