Shoulder surgery is very often followed by severe postoperative pain. Loco-regional anaesthesia has greatly contributed as a solution of this problem. Acute pain can be managed favourably by the use of paracetamol, non steroidal anti-inflammatory drugs, opioids and local anaesthetics, solely or in combination. Sophisticated methods of administration such as nerve blocks or patient-controlled analgesia will improve results. Chronic pain, on the other hand, presents a more complex situation and the pharmacological approach is only one aspect of bio-psycho-social management programmes. The role of opioids in the treatment of chronic non-cancer pain has still not been clarified. Adjuvant drugs are often required.

Cyanosis occurring during a transesophageal echocardiogram (TEE) is usually related to the respiratory depressant effect of the administered sedative. However, acute toxic methemoglobinemia related to topical benzocaine is a rare and not well recognized but potentially life threatening cause of cyanosis during or after TEE. A case of this condition is presented and the literature on its frequency, diagnosis and management is briefly reviewed.

Topical therapy can be considered the standard treatment for distal ulcerative colitis. The group of drugs of first choice are the aminosalicylates which are effective in inducing remission in acute disease as well as in preventing relapse. The effects of various local anaesthetics (LAs) on ryanodine binding of the sheep brain ryanodine receptor were tested. Tetracaine and dibucaine inhibit the binding with half-maximal inhibition (CI50) of 0.12 mM and 0.7 mM, respectively.
