Wrist arthrodesis with volar plate fixation in cerebral palsy.

Wrist arthrodesis can obtain better appearance, hygiene, and ease of daily care, with some degree of functional improvement in patients with cerebral palsy with a severe wrist flexion deformity. Rigid fixation using a dorsal plate and screws has been accepted as a reliable technique, although hardware-related problems are relatively common. We describe a volar plate fixation technique for wrist arthodesis in cerebral palsy, which can allow concomitant flexor tendon release and avoid a prominent hardware and a cosmetically undesirable scar on the dorsum of the wrist.

Minimally invasive arthroscopic-assisted reduction with percutaneous fixation in the management of intra-articular calcaneal fractures: a review of 24 cases.

UNLABELLED: A retrospective analysis of 24 cases of minimally invasive, open reduction, and internal fixation of intra-articular calcaneal fractures is presented. Collected data included articular step-off, medial wall displacement, and Boehler's angle, in addition to other descriptive characteristics of the fracture and case series. The operative technique is described in detail including the optimal screw constructs. Arthroscopic assistance was used in 10 of the cases. The articular step-off of the posterior facet, medial wall displacement, and Boehler's angle all displayed statistically sig

Multislice helical CT in the diagnosis of pharyngovertebral fistula due to loosening of anterior screw-plate construct.

Late dysphagia is a rare condition that results from hardware failure in anterior spine surgery patients. Dysphagia should be evaluated to find out the possible underlying causes. A barium swallow study and multislice helical computed tomography (CT) scan were used to evaluate a patient suffering from dysphagia 8 months after an anterior cervical fusion of C4-C7 vertebrae. A videofluoroscopic swallow study with a low-viscosity, high-density barium solution and different table tilt levels for obtaining slow passage of the barium meal revealed the existence of an esophagovertebral fistula with 2

[Orthodontic anchorage in the era of osseointegration].

The introduction of the concept of osseointegration had a profound impact on dentistry of the 20th century. The idea of using this concept in orthodontics already started in the late 70th. There demand for achieving optimal esthetic results in a minimal treatment time, and the significant increase in the number of adult orthodontic patients, made osseointegration an essential adjunct to orthodontic therapy. The use of osseintegrated anchorage devices helps the clinician to overcome one of the most limiting aspects of orthodontic therapy--inadequate anchorage. In the last years a large variety

The anatomical study of transoral atlantoaxial reduction plate internal fixation.

OBJECTIVE: To study relevant anatomical features of the structures involved in transoral atlanto-axial reduction plate (TARP) internal fixation through transoral approach for treating irreducible atlanto-axial dislocation and providing anatomical basis for the clinical application of TARP. METHODS: Ten fresh craniocervical specimens were microsurgically dissected layer by layer through transoral approach. The stratification of the posterior pharyngeal wall, the course of the vertebral artery, anatomical relationships of the adjacent structures of the atlas and axis, and the closely relevant a

Posterior stabilization of subaxial cervical spine trauma: indications and techniques.

The use of instrumentation for stabilization following cervical trauma has evolved rapidly in the past few decades. Nonoperative maneuvers, including traction, extended bed rest, and cast immobilization, have given way to wiring or screw rod constructs that allow immediate fixation and early mobilization of the patient. These procedures can be performed soon after trauma and with a minimum of surgical risk. They offer immediate stability of the injured spine, and can prevent the sequelae of acute cervical spinal cord injury that may accompany prolonged bed rest, thus allowing early rehabilitat

Chronic headache as a sequela of rigid fixation for craniosynostosis.

Rigid fixation has been used over the past 20 years for the long-lasting correction of traumatic and congenital craniofacial defects. It has been noted that the use of plates and screws can result in the migration of the hardware through the skull to the inner cerebral cortex where it embeds in the dura. In addition, there is controversy concerning the safety of using titanium plates in the pediatric population. We report here on an 8-year-old boy who presented to our clinic with a chronic headache after rigid craniofacial fixation using titanium plates 7 years before.

MRI assessment of the proximal pole of the scaphoid after internal fixation with a titanium alloy Herbert screw.

We report a series of scaphoid fractures fixed with titanium alloy Herbert screws in which postoperative Magnetic Resonance Imaging (MRI) was used to assess the marrow signal in the proximal pole of the scaphoid and thus detect the presence of avascular necrosis. The artefact produced by the titanium alloy Herbert screw did not preclude this assessment.

[Partial arthrodesis of the carpal bones in advanced carpal collapse in chronic scapho-lunar instability and following scaphoid pseudoarthrosis].

Degenerative arthritis of the wrist as a consequence of chronic scapho-lunate instability or chronic scaphoid non-union follows very specific patterns. Joint destruction occurs primarily in the radio-scaphoid and in the luno-capitate joints while the radio-lunar joint is preserved. Treatment by partial carpal fusion between capitate, scaphoid and lunate aims at reconstruction of correct length of the central column and restoration of normal carpal height by reducing the dorsiflexed lunate bone, thereby decompressing the painful radio-scaphoid joint. We have carried out the procedure in 21 case

[Traumatology in the elderly. Problems exemplified by femoral neck fractures].

The most frequent complications in response to a fracture of the femoral neck, a typical injury of the elderly, are necrosis of the head of femur and pseudoarthrosis of the femoral neck. Only the around dressed down fracture can be conveyed to conservative treatment. The classification of AO takes these problems into consideration. One possibility to preserve the femur head is by operative insertion of a dynamic hip screw (DHS). Specific problems of the diverse types of fracture and their surgical details are taken into special account.

Treatment of slipped capital femoral epiphysis resulting from juvenile renal osteodystrophy.

Severe bilateral slipped capital femoral epiphysis (SCFE) secondary to renal osteodystrophy is infrequent at any age. Management was a significant dilemma in the 3-year-old child we report because of the degree of slippage and our concern over the sequelae of premature capital physeal closure. Fixation without physeal closure was obtained by inserting a specially fabricated screw without threads across the physis after the slippage had been reduced with traction.

Greater trochanteric overgrowth: development and surgical correction.

Greater trochanteric advancement (GTA) is an orthopedic procedure designed to correct the biomechanical consequences of overgrowth of the greater trochanter by moving the greater trochanter and its attached muscles laterally and distally. Thirty-one children with trochanteric overgrowth who had secondary pelvic instability (Trendelenburg sign) were studied and underwent a total of 33 GTA procedures. The clinical and radiologic prerequisites for successful surgery are presented. In addition, the radiographic methods of measuring the lateral position of the trochanter, the articulotrochanteric d
