Clinical and radiological outcomes of a cartilage-preserving pinhole transtibial technique versus standard pull-out repair for traumatic posterior meniscal root tears.

BACKGROUND: Traumatic posterior meniscal root tears disrupt normal load transmission, resulting in meniscal extrusion and accelerated osteoarthritis. Although transtibial pull-out repair is widely used, creation of a 4.5-mm tibial tunnel at the joint surface may cause iatrogenic chondral and subchondral bone damage at the root footprint. This study aimed to compare the clinical and radiological outcomes of a modified aperture-preserving pinhole technique, designed to avoid reaming at the tibial joint surface, with those of the standard transtibial pull-out repair. METHODS: A total of 60 patie

Hyaluronic acid-based core-shell microneedles for staged local delivery of psoralen toward anti-inflammatory and cartilage-protective therapy of rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent synovial inflammation and progressive joint destruction, which requires local therapeutic strategies capable of regulating inflammation and cartilage homeostasis. Herein, we report a hyaluronic acid polymer-based core-shell microneedle patch (Mi@PSO/HPA MN) for localized RA treatment with spatially separated payloads and different release profiles. The shell is constructed from crosslinked hyaluronic acid methacrylate (HAMA) and a brush-like hyaluronic acid-based polymer (HPA), while the core is composed of h…

Intra-articular Injections of CXCR4-Overexpressing Human Cartilage-Derived Progenitor Cells Improve Meniscus Healing and Protect Against Posttraumatic Osteoarthritis in Immunocompetent Rabbits.

BACKGROUND: Meniscal injuries that fail to heal instigate catabolic changes in the knee's microenvironment, posing a high risk for developing posttraumatic osteoarthritis (PTOA). Previous research has suggested that human cartilage-derived progenitor cells (hCPCs) can stimulate meniscal repair in a manner that depends on stromal cell-derived factor 1 (SDF-1) pathway activity. HYPOTHESIS: Overexpressing the SDF-1 receptor CXCR4 in hCPCs will increase cell trafficking and further improve the repair efficacy of meniscal injuries. STUDY DESIGN: Controlled laboratory study. METHODS: hCPCs were g…

Tensile stress promotes the chondrogenic ability of condylar cartilage stem/progenitor cells in the temporomandibular joint via the Piezo1-Ca2+-Prkca pathway.

BACKGROUND: Tensile force is a key regulator for condylar cartilage remodeling in the temporomandibular joint (TMJ), and this biomechanical characteristic underlies the mechanisms of mandibular growth modification in orthodontic practice. Cartilage stem/progenitor cells (CSPCs) in the superficial layer of condylar cartilage play an essential part in the development and remodeling of condylar cartilage. However, the regulatory role of tensile force on condylar CSPCs remains unclear. This study aimed to investigate the impact of tensile loading on condylar CSPCs and explore the molecular mechani…

In situ implantation of type II collagen-based double-layer scaffolds for Articular Osteochondral Regeneration comprising hyaline cartilage and vascularized subchondral bones.

The articular osteochondral injury involves the repair of hyaline cartilage, subchondral bone plate, and cancellous bone. Due to the weak regeneration ability of chondrocytes and the complex structure of the bone-cartilage junction, there is currently no excellent repair method. The challenge of hyaline cartilage repair is to avoid fibrosis and hypertrophy, which has been solved to some extent after the advent of type II collagen scaffolds; the difficulty of the subchondral bone plate and cancellous bone repair lies in the repair of the complex transition structure of cartilage tidemark, calci…

Additional cartilage treatment for small defects in chronic ankle instability shows no mid-term benefit and delays recovery: a randomized controlled trial.

BACKGROUND: The presence of chondral lesions in patients with chronic ankle instability is common and has been suggested as a possible cause of persistent pain in some cases, even after successful ligament reconstruction. For this reason, some authors have proposed combining ankle stabilization with cartilage microfracture; however, the results reported in literature are contradictory. MATERIALS AND METHODS: The study was designed as a prospective randomized clinical study with two parallel arms. Patients with an anterior talofibular ligament tear causing pain and instability, associated with…

Targeted S100A8 PP-Cas9@PLGA-apt microparticles reduced cartilage degradation and subchondral bone isomerism in osteoarthritis treatment through JAK/STAT3 pathway modulation.

Synovial inflammation caused by osteoarthritis (OA) results in the release of numerous pro-inflammatory factors that promote cartilage degradation and pathological changes of subchondral bone. Nowadays, S100A8 has been recognized as a critical factor in the progression of inflammatory diseases, but its role in OA still needs to be confirmed. At the same time, the gene editing technology has emerged as a novel therapeutic approach for OA, such as clustered regularly interspaced short palindromic repeats (CRISPR/Cas9) technology, but application in inflammatory gene therapy still requires advanc…

Transcriptomic integration and ligand-receptor crosstalk reveal the underlying molecular mechanisms between hip cartilage and subchondral bone in osteonecrosis of femoral head.

Osteonecrosis of femoral head (ONFH) is characterized not only by ischemic bone tissue necrosis but also by cartilage degeneration, which plays an essential role in the pathogenesis of ONFH. The molecular communication between tissues contributes to disease progression, however the communication between cartilage and subchondral bone in the progression of ONFH remains unclear. In this study, we integrated transcriptomic data from ONFH cartilage and subchondral bone, exploring common differentially expressed genes (DEGs), pathway and function enrichment analyses, the protein-protein interaction…

3D printed sodium alginate/gelatin/tannic acid/calcium chloride scaffolds laden bone marrow mesenchymal stem cells to repair defective thyroid cartilage plate.

Due to the absence of blood vessels, cartilage exhibits extremely limited self-repair capacity. Currently, repairing laryngeal cartilage defects, resulting from conditions such as laryngeal tumors, injury, and congenital structural abnormalities, remains a significant challenge in the Department of Otolaryngology, Head and Neck Surgery. Previous research has often focused on enhancing the mechanical properties of synthetic materials. However, their low biological activity and weak cell adhesion necessitate compensatory measures. This study aims to capitalize on the advantages of natural materi…

Leptin Induces MMP-1 Expression Through the RhoA/ERK1/2/NF-κB Axis in Human Intervertebral Disc Cartilage Endplate-Derived Stem Cells.

PURPOSE: Intervertebral disc (IVD) degeneration, associated with aging, may cause low back pain and disability, with obesity as a significant risk factor. In a prior study, we found a positive correlation between IVD degeneration and levels of matrix metalloproteinase-1 (MMP-1) and leptin. Yet, the interaction between MMP-1 and leptin in IVD degeneration is unclear. Our research seeks to explore leptin's influence on MMP-1 expression and the underlying mechanisms in human intervertebral disc cartilage endplate-derived stem cells, specifically SV40 cells. METHODS: The mRNA and protein expressi…
