Motor vehicle crash-related injury causation scenarios for spinal injuries in restrained children and adolescents.

OBJECTIVE: Motor vehicle crash (MVC)-related spinal injuries result in significant morbidity and mortality in children. The objective was to identify MVC-related injury causation scenarios for spinal injuries in restrained children. METHODS: This was a case series of occupants in MVCs from the Crash Injury Research and Engineering Network (CIREN) data set. Occupants aged 0-17 years old with at least one Abbreviated Injury Scale (AIS) 2+ severity spinal injury in vehicles model year 1990+ that did not experience a rollover were included. Unrestrained occupants, those not using the shoulder por

COVID-19 vaccination and short-term mortality risk: a nationwide self-controlled case series study in The Netherlands.

Excess mortality during the COVID-19 pandemic partly exceeded COVID-19-related deaths, indicating that other causes may have contributed. We conducted a retrospective data-linkage study including all Dutch inhabitants to investigate the impact of COVID-19 vaccination on excess mortality using a modified self-controlled case series method. We found a 44% lower relative incidence of all-cause deaths in the first three weeks after the primary vaccination compared to more than three weeks after vaccination (IRR 0.56, 95%CI 0.54-0.57). This lower incidence was consistent across vaccine types, doses

Hypothermia on trauma center arrival has a much greater impact on outcomes at the extremes of age.

BACKGROUND: Hypothermia is a potentially modifiable risk factor for poor outcomes in trauma; differences in impact of hypothermia on different age groups are not well-defined. This study characterized the effects of varying levels of hypothermia on injured patients by age. We hypothesized that pediatric and geriatric trauma patients would be disproportionately impacted by hypothermia. METHODS: This retrospective cohort study included all patients with a valid arrival temperature from the 2020 to 2023 Trauma Quality Improvement Program database. Patients were categorized by arrival temperature

Excess All-Cause Mortality by Age and Gender During the COVID-19 Pandemic in the Federation of Bosnia and Herzegovina, Bosnia and Herzegovina: 2020-2022.

OBJECTIVES: COVID-19 has had a profound impact on global mortality and morbidity, yet only a fraction of deaths was confirmed and reported. We estimated all-cause excess mortality from 1 January 2020 to 31 December 2022 in the Federation of Bosnia and Herzegovina (FBiH) to assess the true magnitude of the pandemic. METHODS: Data for this analysis was sourced from the FBiH mortality register and supplemented with population statistics and official COVID-19 death counts (i.e., cases where COVID-19 was registered as the cause of death). Using a Poisson model, all-cause excess number of deaths an

Risk factors for adverse short-term and long-term outcomes in children with severe acute kidney injury requiring continuous kidney replacement therapy.

BACKGROUND: Severe acute kidney injury (AKI) requiring continuous kidney replacement therapy (CKRT) is common in critically ill children. METHODS: We conducted a single-center, retrospective cohort study of children with AKI requiring CKRT from 2011 to 2020 to determine the incidence of and risk factors for adverse short- and long-term outcomes using multivariable analysis. Patients with chronic kidney disease (CKD) and, for long-term analysis, the lack of post-discharge data, were excluded. RESULTS: Of 217 patients, 47% died in hospital. Hospital, intensive care unit (ICU), and ventilator d

Early-Life Disease Exposure and Its Heterogeneous Effects on Mortality Throughout Life: Sweden, 1905-2016.

Exposure to infectious diseases in early life has been linked to increased mortality risk in later life in high-disease settings, such as eighteenth- and nineteenth-century Europe. Less is known about the long-term effects of early-life disease exposure in milder disease environments. This study estimates heterogeneous effects from disease exposure in infancy on later-life mortality in twentieth-century Sweden, by socioeconomic status at birth and sex. Using historical population data for southern Sweden, we study 11,515 individuals who were born in 1905-1929 from age 1 until age 85. We measur

Epidemiology and outcome analysis of facial burns: A retrospective multicentre study 2011-2015.

BACKGROUND: The head and neck regions are frequent sites of burns, but few studies have analysed and reported the epidemiology of facial burns. As the face is the centre of one's identity and persona, facial injuries often result in physical and psychological morbidity. The aim of this article is to describe the epidemiology and outcome of facial burns in China and to suggest future preventive strategies. METHODS: This retrospective analysis included all patients with facial burns in a database at eight institutions from 2011-2015. The data collected included sex, age, month distribution, aet

Risk Factors for Mortality in Severely Ill Children Admitted to a Tertiary Referral Hospital in Malawi.

In low-resource settings, many children are severely ill at arrival to hospital. The risk factors for mortality among such ill children are not well-known. Understanding which of these patients are at the highest risk could assist in the allocation of limited resources to where they are most needed. A cohort study of severely ill children treated in the resuscitation room of the pediatric emergency department at Queen Elizabeth Central Hospital in Malawi was conducted over a 6-month period in 2017. Data on signs and symptoms, vital signs, blood glucose levels, and nutritional status were colle

Hyperchloremia is independently associated with mortality in critically ill children who ultimately require continuous renal replacement therapy.

BACKGROUND: The optimal fluid management in critically ill children is currently under investigation with several studies suggesting that hyperchloremia, chloride load, and the use of chloride-rich fluids contribute to worse outcomes. METHODS: This is a single-center retrospective cohort study of Pediatric Intensive Care Unit patients from 2008 to 2016 requiring continuous renal replacement therapy (CRRT). Patients were excluded if they had end-stage renal disease, a disorder of chloride transport, or concurrent provision of extracorporeal membrane oxygenation therapy. RESULTS: Patients (n =

Cuban Application of Two Methods for Analyzing Multiple Causes of Death.

INTRODUCTION Mortality analysis based on a single cause of death is not, in most cases, fully informative. There are several more illuminating procedures using a multiple cause of death approach; these are little known and rarely used in Cuba. The simplest of these methods, while methodologically limited, consists of summing all deaths from a specific cause mentioned on death certificates, regardless of whether the cause is listed as underlying or contributing. OBJECTIVE Using Cuban data, critically assess and implement two of the most recognized approaches to analyzing multiple causes of deat

National disability-adjusted life years (DALYs) for 257 diseases and injuries in Ethiopia, 1990-2015: findings from the global burden of disease study 2015.

BACKGROUND: Disability-adjusted life years (DALYs) provide a summary measure of health and can be a critical input to guide health systems, investments, and priority-setting in Ethiopia. We aimed to determine the leading causes of premature mortality and disability using DALYs and describe the relative burden of disease and injuries in Ethiopia. METHODS: We used results from the Global Burden of Diseases, Injuries, and Risk Factors Study 2015 (GBD 2015) for non-fatal disease burden, cause-specific mortality, and all-cause mortality to derive age-standardized DALYs by sex for Ethiopia for each
