Gender and Racial Disparities in Life-Space Constriction Among Older Adults.

PURPOSE OF THE STUDY: "Life-space" is the spatial area through which a person experiences and interacts with the world. Life-space constriction, the shrinking of the spatial area that a person traverses, is associated with negative health outcomes in later life. Racial and gender disparities in mobility as indicated by life-space constriction are thought to contribute to broader disparities in health and functioning among older adults. DESIGN AND METHODS: Data come from the 5-year follow-up of the Advanced Cognitive Training for Independent and Vital Elderly (ACTIVE) Study (N = 2,765; mean ag…

Elder abuse and dementia: a review of the research and health policy.

Older adults with dementia may be at high risk for abuse, but the topic has not been well studied. We conducted a literature review to examine the relationships between elder abuse and dementia. We found that psychological abuse was the most common form of abuse among older adults, with estimates of its prevalence ranging from 27.9 percent to 62.3 percent. Physical abuse was estimated to affect 3.5-23.1 percent of older adults with dementia. We also found that many older adults experienced multiple forms of abuse simultaneously, and the risk of mortality from abuse and self-neglect may be high

Impaired implicit sequence learning in Parkinson's disease patients with freezing of gait.

OBJECTIVE: Freezing of gait (FOG) in Parkinson's disease (PD) may involve specific impairments in acquiring automaticity under working memory load. This study examined whether implicit sequence learning, with or without a secondary task, is impaired in patients with FOG. METHOD: Fourteen freezers (FRs), 14 nonfreezers (nFRs), and 14 matched healthy controls (HCs) performed a serial reaction time (SRT) task with a deterministic stimulus sequence under single-task (ST) and dual-task (DT) conditions. The increase in reaction times (RTs) for random compared with sequenced blocks was used as a mea

Perceived fitness protects against stress-based mental health impairments among police officers who report good sleep.

OBJECTIVES: This study examined a cognitive stress-moderation model that posits that the harmful effects of chronic stress are decreased in police officers who perceive high levels of physical fitness. It also determined whether the stress-buffering effect of perceived fitness is influenced by officers' self-reported sleep. METHODS: A total of 460 police officers (n=116 females, n=344 males, mean age: M=40.7; SD=9.7) rated their physical fitness and completed a battery of self-report stress, mental health, and sleep questionnaires. Three-way analyses of covariance were performed to examine wh

Epitopes of microbial and human heat shock protein 60 and their recognition in myalgic encephalomyelitis.

Myalgic encephalomyelitis (ME, also called Chronic Fatigue Syndrome), a common disease with chronic fatigability, cognitive dysfunction and myalgia of unknown etiology, often starts with an infection. The chaperonin human heat shock protein 60 (HSP60) occurs in mitochondria and in bacteria, is highly conserved, antigenic and a major autoantigen. The anti-HSP60 humoral (IgG and IgM) immune response was studied in 69 ME patients and 76 blood donors (BD) (the Training set) with recombinant human and E coli HSP60, and 136 30-mer overlapping and targeted peptides from HSP60 of humans, Chlamydia, My

Video Rating in Neurodegenerative Disease Clinical Trials: The Experience of PRION-1.

BACKGROUND/AIMS: Large clinical trials including patients with uncommon diseases involve assessors in different geographical locations, resulting in considerable inter-rater variability in assessment scores. As video recordings of examinations, which can be individually rated, may eliminate such variability, we measured the agreement between a single video rater and multiple examining physicians in the context of PRION-1, a clinical trial of the antimalarial drug quinacrine in human prion diseases. METHODS: We analysed a 43-component neurocognitive assessment battery, on 101 patients with Cre

Sociodemographic markers and behavioral correlates of sexually transmitted infections in a nonclinical sample of adolescent and young adult women.

BACKGROUND: We examined sociodemographic markers and 3-month behavioral correlates of sexually transmitted infections (STIs) in a nonclinical cross-section of adolescent and young adult women. METHODS: All women (N=2288) enrolled in recruit training for the US Marine Corps during a 1-year period were asked to voluntarily participate in either a cognitive-behavioral, skills-building intervention to prevent STIs and unintended pregnancies or a nutrition and fitness program. Participants (94.2%) completed a self-administered questionnaire and were screened for Chlamydia trachomatis, Neisseria go

Experiences of Identity-Related Vigilance in Surgical Trainees: A Qualitative Study.

IMPORTANCE: Experiences with identity-related mistreatment are a critical concern in surgical residency training. Prior research demonstrates those who experience identity-related mistreatment or scrutiny can experience subsequent vigilance for additional encounters with associated stress. It is unknown whether surgical trainees who experience identity-related mistreatment or scrutiny also experience identity-related vigilance. OBJECTIVE: To determine whether identity-related vigilance impacts surgical trainees in the learning environment. DESIGN, SETTING, AND PARTICIPANTS: This qualitative

Agilik@home: A randomized controlled trial protocol to evaluate the effects of home-based training with the Agilik powered KAFO in children with cerebral palsy and crouch gait.

BACKGROUND: Cerebral palsy (CP) is the most common neuromotor disability in children and often results in spastic diplegia, which can lead to crouch gait. This gait pattern, resulting from weakness predominantly in the extensor muscles and spasticity mainly affecting the flexors, is characterized by excessive knee flexion, insufficient hip extension, and excessive ankle dorsiflexion, which together can progressively impair walking. The Agilik powered knee-ankle-foot orthosis is designed to enhance knee extension and potentially improve gait in children with CP. Here we present the protocol of

Benchmarking publicly accessible large language models for high-myopia multiple-choice question generation in digital ophthalmic education and public health training.

BACKGROUND: Digital tools are reshaping public health education and training, yet evidence on whether large language models (LLMs) can generate specialist ophthalmic teaching materials remains limited. High myopia (HM), a vision-threatening condition with long-term management needs and public health relevance, provides a suitable setting for evaluating this capability. This study compared five LLMs in generating HM-related multiple-choice questions (MCQs) for ophthalmic education. METHODS: Five LLMs (ChatGPT-5.4, Gemini 3, DeepSeek, Kimi K2.5, and Doubao) completed 60 predefined HM MCQ genera

Aging knowledge, geriatric care attitudes, and career choice motivation among nursing students: A latent profile analysis and mediation analysis.

BACKGROUND: The global aging population has intensified the demand for geriatric care, yet a critical shortage of professionals persists, partly due to nursing students' low motivation to pursue careers in this field. Knowledge of aging and attitudes towards geriatric care are key antecedents to career choice motivation, but their complex interrelationships and the heterogeneity in attitudes remain underexplored. AIM: To identify latent profiles of geriatric care attitudes, examine differences in knowledge of aging and career choice motivation across profiles, and investigate knowledge of agi

Lecture, model, or both? Optimizing fracture education with 3D printing: a longitudinal interventional study.

BACKGROUND: Traditional anatomical training methods in surgery, like cadaver dissection and didactic lectures, face limitations in availability, cost, and translating 2D to 3D understanding. Three-dimensional (3D) printing offers an innovative solution for medical education, particularly in orthopedics and trauma surgery, where complex bone structures are crucial. 3D-printed models show promise for improving anatomical learning, spatial understanding, and student satisfaction. METHODS: This longitudinal interventional study investigated the impact of different teaching methods on medical stud

Scoping review on orofacial pain management in older adults with dementia.

BACKGROUND/OBJECTIVES: Orofacial pain in older adults with dementia is often overlooked due to cognitive decline and impaired communication, impairing patients' quality of life and exacerbating neuropsychiatric behavioural symptoms. This scoping review aimed to map available evidence regarding prevalence, assessment tools, clinical management strategies and research gaps, with the aim of informing clinical practice and guiding future research. METHODS: Systematic literature searches were performed in PubMed, Web of Science, Embase and Cochrane Library from database inception to December 2025.

Brief intermittent intense exercise as interoceptive exposure for panic disorder: a randomized controlled clinical trial.

BACKGROUND: Interoceptive exposure (IE) to feared bodily sensations is a core component of cognitive-behavioral therapy for panic disorder (PD), but standard office-based IE can be perceived as aversive and tedious, potentially limiting engagement. Vigorous physical exercise may provide a more acceptable and health-promoting way to elicit interoceptive cues. Objective: To examine the feasibility and efficacy of a brief intermittent intense exercise (BIE) program, used as an IE strategy, compared with Jacobson's relaxation training (RT) in treatment-free patients with PD. METHODS: In this pros

High-intensity interval training ameliorates Alzheimer's disease-like pathology by regulating astrocyte phenotype-associated AQP4 polarization.

Background: Alzheimer's disease (AD), one of the most common forms of dementia, is a widely studied neurodegenerative disease characterized by Aβ accumulation and tau hyperphosphorylation. Currently, there is no effective cure available for AD. The astrocyte AQP4 polarized distribution-mediated glymphatic system is essential for Aβ and abnormal tau clearance and is a potential therapeutic target for AD. However, the role of exercise on the AQP4 polarized distribution and the association between the AQP4 polarized distribution and astrocyte phenotype polarization are poorly understood. Methods:

Study in Parkinson's disease of exercise phase 3 (SPARX3): study protocol for a randomized controlled trial.

BACKGROUND: To date, no medication has slowed the progression of Parkinson's disease (PD). Preclinical, epidemiological, and experimental data on humans all support many benefits of endurance exercise among persons with PD. The key question is whether there is a definitive additional benefit of exercising at high intensity, in terms of slowing disease progression, beyond the well-documented benefit of endurance training on a treadmill for fitness, gait, and functional mobility. This study will determine the efficacy of high-intensity endurance exercise as first-line therapy for persons diagnos

Hypnosis on patients treated with adjuvant chemotherapy for breast cancer: A feasibility study.

BACKGROUND: Acceptability and tolerance of chemotherapy on patients treated for breast cancer remain challenging. Complementary approaches such as hypnosis may have a favorable impact both at the time of announcing and during chemotherapy, due to the notorious anxiety, distress, and self-perceived dysfunction. The objective of the study was that the patients complied with at least four self-hypnosis sessions out of the six cycles of chemotherapy. METHODS: This open, prospective longitudinal study assessed feasibility of compliance to self-hypnosis during chemotherapy in an outpatients setting
