An 89-year-old woman was admitted for intestinal obstruction. Her abdominal radiograph showed typical features of sigmoid volvulus. Decompression was done successfully through careful flexible sigmoidoscopy with minimum air insufflation. The patient's symptoms were relieved and she was discharged two days post-procedure. No recurrence was noted at extended follow-up. Rapid diagnosis and early decompression are key to optimal management of sigmoid volvulus.

The imaging appearances of three patients with a giant sigmoid diverticulum are described. The prominent feature was a large gaseous lucency noted in the lower abdomen on plain radiographs. Computed tomography (CT) was undertaken in two cases and in these a large gas filled collection was identified containing a small quantity of fluid. In the third case the collection was aspirated, contrast medium injected and a communication with the large bowel demonstrated. The condition is uncommon and needs to be distinguished from sigmoid and caecal volvulus.

An abdominal ultrasound examination performed on two patients with abdominal pain showed segmental wall thickening of the colon that proved to be due to ischemic colitis. In one case, the typical "target lesion" was recognized. These findings could be confirmed with computed tomography, whereas a single contrast enema was unable to demonstrate the ischemic disease. In the second case, a typical five-layered structure of the bowel wall could be demonstrated. It is concluded that US can be useful in the early diagnosis of colonic ischemia.

A case of a young man who developed a Streptococcus milleri liver abscess secondary to an occult colonic foreign body, which presented as fulminant community acquired pneumonia, is reported. The isolation of Streptococcus milleri, a normal member of the gastrointestinal flora, from an abscess, blood or sputum should prompt a search for pathology, foreign bodies or silent perforation in the gastrointestinal tract.

The use of magnetic resonance imaging (MRI) for abdominal and gastrointestinal imaging has been quite limited, when compared to its utilization in other organ systems. We present an interesting case of extraperitoneal bowel perforation, which was diagnosed by magnetic resonance (MR). Extraperitoneal bowel perforation is frequently a confusing and difficult diagnosis to make. The multiplanar imaging capability and unique sensitivity of MR to fluid enabled a specific diagnosis to be made in this challenging case.

Two hundred and twenty-three cases of intussusception admitted over a 9½-year period are analysed. Six deaths occurred. Sigmoidorectal intussusception occurred on 5 occasions and in each case was referred as a case of rectal prolapse, and the diagnosis and management of this particular form of intussusception are discussed.
