Nocardiosis is an infectious disease that occurs in both normal and immunocompromised hosts. In North America, the most common presentation is that of primary subacute pneumonia. In Central and South America, primary cutaneous infections are more common. The diagnosis is often missed because of contamination of cultures and the need to culture specimens for extended periods. An epidemiological survey for scabies was conducted in a rural community of 2771 persons among 404 households living in 238 houses. Prevalence rates were 13% by population and 30.9% by household. Each index case was responsible, on average, for the spread of infection to 1.9 persons in affected households. One hundred and forty-eight cases of acute epiglottitis in children treated at the University of Rochester's Strong Memorial Hospital and two affiliated community hospitals between 1965 and 1983 are reviewed retrospectively. Patients seen prior to 1975 were usually managed with tracheotomy and intravenous antibiotics, while those after 1975 with nasotracheal intubation, antibiotics and steroids. An outbreak of Shigellosis due to Shigella sonnei, S. flexnerii and S. boydii in an ultra-religious Jewish community in Southern Israel is described. The source of epidemic was traced to vegetables bought from a single source. The importance of epidemic and endemic modes of spread of Shigella is discussed.
