Lumbar spinal stenosis is a common degenerative condition that causes pain, disability, and reduces quality of life, especially in older adults. Techniques such as Unilateral Biportal Endoscopy (UBE) and Open Unilateral Laminotomy with Bilateral Decompression (ULBD) aim to preserve soft tissues while achieving adequate decompression of neural tissue. The study included 194 patients with symptomatic degenerative lumbar spinal stenosis, of whom 177 patients underwent UBE and 17 patients underwent Open ULBD. Clinical outcomes were measured with the visual analog scale (VAS) for pain, the Oswestry

BACKGROUND: Lumbar spinal chondroma (LSC) is a rare benign tumor. Most LSCs are located in the epidural space, and some exhibit a dumbbell-shaped growth pattern. Its presentation mimics lumbar disc herniation, leading to frequent misdiagnosis. While hemilaminectomy has been the conventional treatment, we report a case successfully managed with a novel technique: Arthroscopic-assisted Uni-portal Spinal Surgery (AUSS) via a far-lateral extraforaminal approach. CASE PRESENTATION: We report a 42-year-old male with recurrent left lower limb radicular pain for one year. Preoperative MRI revealed an L

OBJECTIVE: In patients with severe central canal stenosis or posterior vertebral body osteophytes, endoscopic techniques often face challenges such as limited decompression range, endoscopic visual blind spots, and inadequate decompression. To address these limitations, our team innovatively developed a modified posterior cervical endoscopic decompression technique termed "Cross-over top Decompression (COTD)", drawing on clinical experience in lumbar endoscopic decompression and biomechanical principles. This study aims to compare its biomechanical performance with endoscopic unilateral lamino

OBJECTIVE: In unilateral biportal endoscopic (UBE) decompression surgery, image quality reduces potential risks in the surgical field and allows the surgical procedure to be performed in a shorter time. This study aimed to investigate the relationship between visualization and blood pressure during surgery and to determine the optimal blood pressure to maintain good visualization during surgery.
METHODS: A senior surgeon and an assistant surgeon independently evaluated intraoperative visualization during UBE entry, laminotomy, and foraminotomy. They assessed image quality based on blood press

BACKGROUND: Achondroplasia is the most prevalent form of skeletal dysplasia and is characterized by rhizomelia, short stature, and distinctive facial features. Achondroplasia is frequently accompanied by spinal canal stenosis because of the distinctive morphology of the spine. For pediatric lumbar spinal canal stenosis, a meticulously constructed surgical plan is needed to prevent complications such as the development of thoracolumbar kyphosis.
CASE PRESENTATION: An 11-year-old Asian boy with achondroplasia presented with bilateral lower limb numbness and intermittent claudication, which limi

We present a unique case of a man in his 60s with a knee intra-articular loose body causing a compression neuropathy on his common peroneal (fibular) nerve (CPN). This neuropathy caused a reduction in function of his leg's motor function causing a foot drop. Through a strong clinical interview and focused clinical examination, the neuropathy was localised, and the diagnosis was reinforced through nerve conduction studies and imaging. The intra-articular loose body was surgically excised after the careful isolation and decompression of the peroneal nerve. This resulted in a marked improvement i

INTRODUCTION: Sigmoid volvulus is a rare cause of bowel obstruction in young adults and is classically seen in elderly males. In woman of reproductive age, sigmoid volvulus is easily misdiagnosed as a gynecologic emergency, which will contribute to delayed recognition and increased morbidity. Reports of sigmoid volvulus in healthy young females without predisposing factors remain extremely limited in literature. Against this backdrop of established knowledge, we present a unique case that challenges the typical demographic profile of sigmoid volvulus.

BACKGROUND: To spare important anatomical structures from damage during treatment of cystic odontogenic lesions, decompression has become a conservative alternative to enucleation. Marsupialization, in combination with the use of a custom-made decompression prosthesis, is an effective method of inducing reduction and bony infill of cystic lesions of the jaw.
METHODS: In this case series and review of the literature, we describe three patients with odontogenic cysts of the mandible who were treated with custom-made decompression obturator prostheses (DOP) at the Memorial Sloan Kettering Cancer

Thoracolumbar fractures are common injuries that usually result from high energy trauma. They can lead to significant morbidity due to neurologic impair - or mortality - if not managed according to strict and rapid intervention rules in terms of decompression of the spinal cord, and rigid fixation of the fracture. This manuscript reviews emergency treatment protocols, imaging modalities, and classification systems used for thoracolumbar fractures. The emergency treatment is discussed, specific classifications are compared and indications for surgeries are compared.

Surgical treatment of celiac artery (CA) compression syndrome (CACS) is to release the median arcuate ligament (MAL) by removing the abdominal nerve plexus surrounding CA. In laparoscopic surgery of CACS, objective intraoperative assessment of blood flow in CA is highly desirable. We herein demonstrate a case of laparoscopic surgery of CACS with use of intraoperative transabdominal ultrasound. A 52-year-old woman was presented with epigastric pain and vomiting after eating. Contrast-enhanced computed tomography demonstrated significant stenosis at the origin of CA. Doppler study of CA was also

BACKGROUND The degree of postoperative symptom improvement in patients with lumbar spinal stenosis (LSS) is crucial to their postoperative rehabilitation process and functional exercise. Corticosteroids have certain anti-inflammatory effects. This study aimed to explore whether small doses of corticosteroids would improve postoperative neurological symptoms in patients with lumbar spinal stenosis. MATERIAL AND METHODS Patients with lumbar spinal stenosis who underwent open surgery were divided into a corticosteroid therapy group (CTG) and a non-corticosteroid therapy group (NCTG). They were fo

Cervical spinal stenosis most commonly occurs at the subaxial spine. C1-C2 stenosis is relatively unusual but can be present in certain congenital or syndromic conditions such as rheumatoid arthritis. In this manuscript, we highlight the case of a 42-year-old lady who presented with neck pain and signs of high cervical myelopathy. Imaging studies showed severe stenosis at C1-C2 caused by posterior osteophytes originating from the C1 arch and C2 lamina. Decompression and stabilization of the segment were performed. This case is unique as this pathology occurred at an uncommon location, leading

BACKGROUND: Carpal tunnel release (CTR) surgery is the most common surgery billed to Medicare by hand surgeons. As such, the purpose of this study was to evaluate trends for CTR surgeries billed to Medicare from 2000 to 2020.
METHODS: The publicly available Medicare Part B National Summary File from 2000 to 2020 was queried. For both open carpal tunnel release (OCTR) and endoscopic carpal tunnel release (ECTR), the number of procedures and total Medicare reimbursement were extracted. For year 2020, the specialty of the performing surgeon was recorded. Descriptive statistics were reported.

BACKGROUND: In patients with Chiari malformation type 1 (CMI) presenting with low intracranial compliance (ICC), foramen magnum decompression (FMD) often fails and the complication rate may be higher. We therefore routinely perform a preoperative assessment of ICC from intracranial pressure measurement. Patients with low ICC are treated with ventriculoperitoneal shunt (VPS) before FMD. In this study, we assess the outcome of patients with low ICC, compared with patients with high ICC treated with FMD alone.
METHODS: We reviewed the clinical and radiologic data of all consecutive patients with

INTRODUCTION: Affective disorders (AD) have been shown to influence patient outcomes and healthcare resource utilization across several pathologies, though this relationship has not been described in patients with Chiari I malformations (CM-I). The aim of this study was to determine the impact of comorbid AD on postoperative events and healthcare resource utilization in adults following suboccipital decompression for CM-I.
METHODS: A retrospective study was performed using the 2016-2019 National Inpatient Sample database. Adults who underwent suboccipital decompression for CM-I were identifie

Context/Objective: Providing accurate counseling on neurological recovery is crucial after traumatic spinal cord injury (TSCI). The early neurological changes that occur in the subacute phase of the injury (i.e. within 14 days of early decompressive surgery) have never been documented. The objective of this study was to assess peri-operative neurological improvements after acute TSCI and determine their relationship with long-term neurological outcomes, measured 6-12 months following the injury. Methods: A retrospective cohort study of 142 adult TSCI patients was conducted. Early peri-operative
