Chronic pruritus is a cardinal symptom of the inflammatory skin disease atopic dermatitis (AD). Pathogenic mechanisms in the periphery, spinal cord and the brain have been implicated in AD-related pruritus. Therefore, both systemic and topical administration of drugs could potentially provide relief. Despite efforts to elucidate the mechanisms behind AD-related pruritus and the relative contribution of peripheral nervous system and central nervous system (CNS), specific and successful treatment options have not yet been developed. Several small molecule drugs are currently being investigated t…

Recent studies have demonstrated significant molecular parallels between atopic dermatitis and alopecia areata as predominantly TH2-mediated diseases. This case report highlights the potential for Dupilumab as a targeted biologic therapy in both atopic dermatitis and alopecia areata.

HISTORY AND FINDINGS: A 25-year-old female patient was treated with the anti-IL-4/IL-13 receptor antibody Dupilumab due to severe atopic dermatitis (AD). In addition, the patient had been suffering from alopecia areata (AA) for 5 years.
EXAMINATION AND DIAGNOSIS: At first presentation a severe AD (Scoring Atopic Dermatitis [SCORAD] 76.0; Eczema Area and Severity Index [EASI] 37.4) with a total-IgE of 1108 kU/l and an Alopecia areata was observed.
THERAPY AND COURSE: One year ago, treatment with the anti-IL-4/IL-13 receptor antibody Dupilumab was initiated. After 5 months, there was a marked …

The role of the innate immune system in allergic contact dermatitis (ACD) has traditionally been confined to the initial antigen sensitization phase. However, more recent findings have shown the role of innate immunity in additional aspects of ACD, including the effector phase of the classic type IV hypersensitivity reaction. As a result, the precise immunologic mechanisms mediating ACD are more complex than previously believed. The aim of this review is to provide insight into recent advances in understanding the role of the innate immune system in the pathogenesis of ACD, including novel mec…

Fiberglass dermatitis is a common form of occupational irritant contact dermatitis. Individuals experience a negative reaction to fiberglass fragments because of mechanical irritation. This review comprehensively analyzes the information in the existing literature on fiberglass dermatitis. Recognizing the different environmental risks factors for acquiring fiberglass dermatitis is necessary for better diagnosis and prevention. In addition to the identification of high-risk populations exposed to fiberglass particles, the common clinical presentation and preventive measures that can reduce the …

BACKGROUND: Atopic dermatitis (AD) is a heterogeneous disease with a multifactorial aetiology and complex pathophysiology. This heterogeneity translates into different trajectories of disease progression with respect to severity, persistence and risk of development of atopic comorbidities. Determining which possible disease trajectories or comorbidities any individual child might develop is challenging in clinical practice. Tools that help identify paediatric patients at higher risk of disease progression would greatly aid clinicians.
METHODS: We reviewed recent cohort studies to synthesize a…

INTRODUCTION: Paraphenylenediamine (PPDA) is a chemical with strong sensitizing properties used for dyeing of hair and textiles. Paraphenylenediamine can cross-react, resulting in allergy to other related compounds. The prevalence of PPDA sensitization varies widely. The objectives were to assess the frequency of positive patch test reactions to PPDA and related chemicals among patients with allergic contact dermatitis (ACD) and to analyze them regarding their clinical pattern, occupation and cross-reactions.
MATERIAL AND METHODS: The sociodemographic and clinical data of patients with positi…

BACKGROUND: There is debate as to whether psoriasis and atopic dermatitis (AD) belong to the same disease spectrum.
OBJECTIVES: To describe and compare disease characteristics, lifestyle factors and disease burden in adult patients with psoriasis and AD.
METHODS: We linked registry data with clinical and patient-reported outcomes from the Danish Skin Cohort, containing 3348 and 3834 adults with dermatologist-verified psoriasis or AD respectively, and 2946 adults from the general population.
RESULTS: The participants were predominantly women and middle-aged. Patients with psoriasis mostly re…
