Adaptive functioning in children with seizures: impact of maternal anxiety about epilepsy.

This study evaluated the impact of maternal anxiety about a child's epilepsy on parental overprotection and the child's adaptive functioning. Specific maternal and family characteristics that contribute to elevated maternal anxiety about epilepsy were also studied over a year's time in a group of 56 mothers with children recently diagnosed with epilepsy. Overall, the primary predictor of maternal anxiety about epilepsy was the mother's level of coping resources, although family stress aggravated anxiety at the initial time point. Maternal anxiety about epilepsy was associated with overprotecti…

[Pharmacoresistant temporal-lobe epilepsy. Exploration with foramen ovale electrodes and surgical outcomes].

AIM: To report our experience in the surgical treatment of temporal-lobe epilepsy. PATIENTS AND METHODS: An analysis was performed of the outcomes of 137 patients submitted to surgery between 1990 and 2001, with a follow-up of more than two years. A study was conducted of the percentages of successful detection by the different complementary tests--MRI, EEG, interictal SPECT, video-EEG with foramen ovale electrodes (FOE), neuropsychological study (NPS)--and the precision with which they reported the epileptogenic focus. RESULTS AND CONCLUSIONS: Successful surgical outcomes (Engel grades I-II…

The neurobiology of temporal lobe epilepsy: too much information, not enough knowledge.

Although there are many types of epilepsy of both genetic and acquired forms, temporal lobe epilepsy (TLE) with hippocampal sclerosis is probably the single most common human epilepsy, and the one most intensely studied. Despite a wealth of descriptive data obtained from patient histories, imaging techniques, electroencephalographic recording, and histological studies, the epileptogenic process remains poorly understood. Progress toward understanding the etiology of an acquired neurological disorder is largely dependent on the degree to which experimental animal models reflect the human condit…

Hippocampal sclerosis in severe myoclonic epilepsy in infancy: a retrospective MRI study.

PURPOSE: Severe myoclonic epilepsy in infancy (SMEI; Dravet's syndrome) is a malignant epilepsy syndrome characterized by early prolonged febrile convulsions (PFCs) with secondary psychomotor delay and a variety of therapy-resistant seizures. Although the initial symptoms are repeated PFCs, the MRI performed at the onset of disease shows no hippocampal structural abnormalities. We aimed to assess clinical and serial MRI data of patients with SMEI with a special attention to the temporomedial structures. To our knowledge, this is the first systematic MRI study in this disease. METHODS: Clinica…

Hemodynamic response (BOLD/fMRI) in focal epilepsy with reference to benzodiazepine effect.

We studied a new procedure of BOLD/fMRI acquisition in epilepsy. They use the benzodiazepine effect to achieve a more reliable baseline for statistical analysis. The method works only in the MR domain without EEG correlation. It compares the EPI images during interictal epileptic discharges and the images "inactivated" by benzodiazepine. The results in five out of eight patients show that this procedure in comparison with the EEG/fMRI method gives a net improvement of spatial definition of BOLD areas. These preliminary results seem to confirm the hypothesis that the better BOLD/fMRI procedure …

Subcortical [18F]fluorodeoxyglucose uptake in lesional epilepsy in patients with intracranial tumour.

BACKGROUND: We hypothesized that in patients with intracerebral tumours a subcortical metabolical shift may be present when the underlying pathology can, itself, be the epileptogenic focus. We also assumed that by studying the alterations in glucose metabolism beyond the tumour's borders we could identify a modulator area. METHODS: Sixty-seven patients with supratentorial brain tumour associated epilepsy were investigated interictally, in normoglycaemic conditions, by using [18F]fluorodeoxyglucose positron emission tomography (FDG PET). The studies were analysed semiquantitatively by calculat…

Chess-playing epilepsy: a case report with video-EEG and back averaging.

A patient suffering from juvenile myoclonic epilepsy experienced myoclonic jerks, fairly regularly, while playing chess. The myoclonus appeared particularly when he had to plan his strategy, to choose between two solutions or while raising the arm to move a chess figure. Video-EEG-polygraphy was performed, with back averaging of the myoclonus registered during a chess match and during neuropsychological testing with Kohs cubes. The EEG spike wave complexes were localised in the fronto-central region. [Published with video sequences].

Quantitative measurement of cortical surface features in localization-related temporal lobe epilepsy.

Differences in cortical surface features between healthy controls (n = 48) and patients with temporal lobe epilepsy (n = 46), ages 14-59, were characterized by means of advanced quantitative MRI processing techniques. Cortical surface features of interest included gyral and sulcal curvature, cortical depth, and total cortical surface area. Epilepsy patients and controls differed on measures of gyrification; the abnormalities generalized despite the focal nature of the primary epileptic process. Changes in cortical surface features were associated with increasing chronological age in both group…

Congenital ocular motor apraxia associated with idiopathic generalized epilepsy in monozygotic twins.

Identical female twins (age 11 years) with congenital ocular motor apraxia and generalized idiopathic epilepsy are reported. Their presenting symptoms were a long history of abnormal head and eye movements. One twin developed partial sensory seizures. The patients underwent 16-channel EEG, electro-oculographic recordings, MRI of the brain, and genetic and metabolic investigations. EEG findings were consistent with idiopathic generalized epilepsy. Electrooculographic recordings of the saccades confirmed an inability to elicit horizontal saccades without preceding head movement; saccades to the …

Ontogeny of feline temporal lobe epilepsy in amygdala-kindled kittens: an update.

This report describes amygdala-kindled seizure development and the post-kindling course in 58 cats (29 males and 29 females), including 40 preadolescents between 2.5 and 6.5 months of age and 18 adults >1 year of age at the beginning of kindling. The results extend previous findings, as follows: (1) Youngest animals (<5.0 months, n=30) are far more likely than older kittens and adults to exhibit spontaneous epilepsy, indexed by seizures that occur >1 h after stimulus-evoked seizures. (2) The youngest animals also exhibit accelerated kindling rates and rapid post-kindling onset of multifocal sp…

MRI-negative PET-positive temporal lobe epilepsy: a distinct surgically remediable syndrome.

Most patients with non-lesional temporal lobe epilepsy (NLTLE) will have the findings of hippocampal sclerosis (HS) on a high resolution MRI. However, a significant minority of patients with NLTLE and electroclinically well-lateralized temporal lobe seizures have no evidence of HS on MRI. Many of these patients have concordant hypometabolism on fluorodeoxyglucose-PET ([18F]FDG-PET). The pathophysiological basis of this latter group remains uncertain. We aimed to determine whether NLTLE without HS on MRI represents a variant of or a different clinicopathological syndrome from that of NLTLE with…

Effects of vagus nerve stimulation on sleep-related breathing in epilepsy patients.

PURPOSE: To describe the effects of vagus nerve stimulation (VNS) on sleep-related breathing in a sample of 16 epilepsy patients. METHODS: Sixteen adults with medically refractory epilepsy (nine men, seven women, ages 21-58 years) underwent baseline polysomnograms (PSGs). Three months after VNS therapy was initiated, PSGs were repeated. In addition, patient 7 had a study with esophageal pressure monitoring, and patient 1 had a continuous positive airway pressure (CPAP) trial. RESULTS: Baseline PSGs: One of 16 patients had an apnea-hypopnea index (AHI) >5 (6.8). Treatment PSGs: Five of 16 pat…

Nonsyndromic mental retardation and cryptogenic epilepsy in women with doublecortin gene mutations.

DCX mutations cause mental retardation in male subjects with lissencephalypachygyria and in female subjects with subcortical band heterotopia (SBH). We observed four families in which carrier women had normal brain magnetic resonance imaging (MRI) and mild mental retardation, with or without epilepsy. Affected male subjects had SBH or pachygyria-SBH. In two families, the phenotype was mild in both genders. In the first family, we found a tyr138his mutation that is predicted to result in abnormal folding in the small hinge region. In the second family, we found an arg178cys mutation at the init…

Lateralising value of neuropsychological protocols for presurgical assessment of temporal lobe epilepsy.

PURPOSE: To estimate the value of neuropsychological measurements in determining the side of seizure onset for presurgical assessment in patients with temporal lobe epilepsy. The lateralising value of neuropsychological protocols was evaluated for all patients and in subpopulations depending on surgical outcome with regard to seizure control, speech dominance, neuropathology, and need for intracranial EEG recordings. METHODS: A battery of neuropsychological procedures was carried out preoperatively in 125 patients who underwent left (n = 66) or right (n = 59) temporal lobectomies. Binary logi…

Left hemisphere dysfunction affects dichotic listening in patients with temporal lobe epilepsy.

This study evaluated the relative effect of left hemisphere dysfunction and side of seizure onset on dichotic listening performance in patients with temporal lobe epilepsy and left hemisphere speech dominance. Seventeen patients were divided into groups based on side of seizure onset and based on scores on a composite measure revealing left hemisphere dysfunction. The group with left hemisphere dysfunction had more correct responses from the left ear, and a left ear advantage, on dichotic listening. The group with normal left hemisphere function showed the expected right ear advantage. Side of…

[Intrathecal glutamate receptor antibodies in a patient with elderly-onset refractory epilepsy].

Antibodies against a glutamate receptor in the central nervous system are reported to be an important autoimmune factor in infantile or childhood refractory epilepsy and in adult-onset paraneoplasstic cerebellar ataxia. We here reported a patient with glutamate receptor antibodies-positive elderly-onset intractable epilepsy and advanced-stage stomach cancer. A 77-year-old man with a history of resected bladder cancer suddenly developed an attack of epilepsia partialis continua in the right arm, followed by a generalized epileptic status. The ictal EEG showed spikes in the left parietal region …

Clinical and neuropsychological characteristics of pediatric epilepsy patients with atypical language dominance.

Studies on intracarotid amobarbital procedures (IAP) in pediatric patients are rare and mainly focus on practicability aspects. Very few studies have reported characteristics of children with atypical language dominance. We compared children with left-sided focal epilepsy and atypical (i.e., right or bilateral) versus left-sided language representation (n=12 versus 17). Our results indicate a higher incidence of left handedness, extratemporal lesions, an earlier onset of epilepsy, and a neuropsychological "crowding effect" with distinct nonverbal memory deficits in the atypical group. We concl…

Cortical dysplasia in extratemporal lobe intractable epilepsy: a study of 52 cases.

Cortical dysplasias or malformations due to abnormalities of cortical development are a well-recognized cause of intractable seizures. This study retrospectively examines the clinicopathologic features of 52 cases of extratemporal cortical dysplasia (from 135 total resections performed over a 16-year period). The study consists of 52 patients (27 males; 25 females) who underwent extratemporal resection for epilepsy at a mean age of 15.1 years (range, 3 months to 44.1 years). Seizure duration before surgery ranged from 7 to 372 months (mean duration, 129 months). Patterns of cortical dysplasia …
