Delayed Diagnosis of Dislocation of All Four Ulnar Carpometacarpal Joints. Surgical Treatment Strategy and Challenges.

Background  Carpometacarpal dislocations are rare injuries whose diagnosis is often missed initially. These injuries can be treated conservatively, as long as an anatomical closed reduction is achieved. However, in most cases, open reduction followed by internal or external fixation is necessary to reach a stable anatomical reduction. Case Description  A 32-year-old male was referred to our institution due to excess swelling of his right hand, 2 months after a motorcycle crash he sustained. During our imaging workup, a dorsal carpometacarpal dislocation of all four ulnar metacarpals was found

Medial Patellofemoral Ligament Reconstruction in Traumatic Patellar Dislocation without Patellar Fixation.

Acute traumatic patellar dislocation is a common injury, and spontaneous reduction may occur at the time of injury or may be reduced at the field of the accident by someone. It may be associated with osteochondral fractures and rupture of medial patellar stabilizers leading to recurrent patellar instability. The aim of this prospective study was to evaluate the outcomes of medial patellofemoral (PF) ligament (MPFL) reconstruction in recurrent traumatic patellar dislocation. Forty-five patients presented with PF instability as a result of traumatic rupture MPFL with normal patellar tracking und

Evaluation of a treatment algorithm for acute traumatic osseous Bankart lesions resulting from first time dislocation of the shoulder with a two year follow-up.

BACKGROUND: Studies dealing with acute osseous Bankart lesions and corresponding treatment strategies are rare. The purpose of this study is to analyze the results after applying our treatment algorithm for acute glenoid rim fractures caused by first time traumatic anterior shoulder dislocations. METHODS: 25 patients were included in this retrospective case series. All patients sustained a first time shoulder dislocation caused by ski or snowboard accidents. An osseous Bankart lesion was detected in all shoulders. Operative therapy was performed in patients with osseous defects of 5% or more,

Acute grade III and IV acromioclavicular dislocations: outcomes and pitfalls of reconstruction procedures using a synthetic ligament.

INTRODUCTION: Acromioclavicular dislocation (ACD) is frequent, at 8% of all shoulder traumas. Management in grade III lesions remains controversial. The present study assessed objective and subjective results at medium-term follow-up (mean, 60 months; range, 12-120 mo) in 27 patients managed by Ligastic(®) ligament reconstruction for acute dislocation. PATIENTS AND METHODS: This is a multicenter, multi-surgeon retrospective study. Between 1998 and 2006, 59 patients were operated on for grade III or IV ACD, in one teaching hospital (Lille, France) and one general hospital (Tourcoing, France).

Outcome of longstanding dislocated elbows treated by open reduction and excision of collateral ligaments.

AIM: To study the long term result of open reduction of longstanding dislocated elbows with regard to stability, avascular necrosis of the distal humerus and degenerative changes of the joint. METHODS: Nine patients, aged between nine and 60 years (average 30 years) with longstanding posterior dislocation of the elbow underwent open reduction. The operative procedure featured a medial and lateral incision, excision of the capsule, fibrous adhesions and the collateral ligaments with no attempt to reconstruct the ligaments, as well as anterior transposition of the ulnar nerve. The procedure was

Systematic Analysis of Surgical Management in Traumatic Cervical Spondyloptosis.

BACKGROUND: Traumatic spondyloptosis has been described as the most severe form of vertebral dislocation. It is characterised by the complete subluxation of a vertebral body by more than 100% relative to the adjacent vertebra. In general, traumatic spondyloptosis can occur in all sections of the spine. It is an extremely unstable injury that requires immediate surgical stabilisation. The aim of this study was to conduct a systematic analysis and evaluation of diagnostic procedures, therapeutic concepts, and surgical strategies for this serious injury. METHODS: Due to the limited number of stu

Traumatic subtalar joint dislocation.

Although uncommon, subtalar joint dislocations remain a frequently missed orthopaedic emergency. Detailed soft tissue and neurovascular assessment is important and these should be documented as appropriate. Failure of urgent reduction might lead to increased risk of conversion to an open injury as a result of pressure necrosis of the overlying skin, risk of talar avascular necrosis and risk of neurovascular compromise. A computed tomography scan is needed in all cases following successful closed or open reduction to identify associated occult foot and ankle fractures. The goal of treatment is

Stress fracture of the manubrium sterni during parallel bar dips.

OBJECTIVE: Stress fractures are common in highly active people, such as athletes or those in the military. They occur frequently in the lower extremities but sternal stress fractures are rare injuries. METHODS: We present a case of a young male who reported no pain and a 'click' sound from the front of the chest while training with parallel bar dips with a grip that was wider than shoulder-width apart. RESULTS: In this case, radiological evaluation was the most helpful tool to diagnose manubrium sterni stress fracture. We advised him to rest but he started exercises immediately because he ha

Intraoperative Closed Reduction of Traumatic Lateraloptosis: Operative Nuances.

BACKGROUND AND IMPORTANCE: Traumatic lateral spondyloptosis, or lateraloptosis, is the complete lateral dislocation of the spine. Reduction in these dislocations presents unique challenges, especially in cases of preserved neurological function. Open techniques carry significant risks of cerebrospinal fluid leak and neurological injury. For traditional spondyloptosis, off-table closed techniques have been described but may result in loss of the reduction when the patient is transferred to the operative table. An on-table closed reduction technique has potential advantages over previously descr

Traumatic Obturator Hip Dislocation in a 9-Year-Old Girl: A Case Report.

CASE: We present a case of traumatic anterior obturator hip dislocation in a pediatric patient with a focus on acute management of this injury. The orthopaedic team successfully performed closed reduction of this injury on an emergent basis, and the patient had minimal issues with ambulation and pain at subsequent follow-up. CONCLUSION: Pediatric traumatic hip dislocations are rare injuries with potentially devastating sequelae, particularly if diagnosis and treatment are delayed. Proper technique during closed reduction is essential. Be prepared for potential emergent open reduction. Two yea

Ulnar translation, a commonly overlooked, unrecognized deformity of distal radius fractures: techniques to correct the malalignment.

Distal radius fracture is one of the most common fractures, accounting for up to 15% of all bone injuries. Radial shortening, loss of palmar inclination, and/or radial tilt are commonly recognized and addressed components of radius fracture deformity. However, a common but typically overlooked and unrecognized component of distal radius fracture deformity is ulnar translation of the proximal radius metaphyseal/shaft. Several techniques to address ulnar translation deformity are presented.

Anterior interosseous nerve palsy associated with Galeazzi fracture.

Galeazzi fracture is more common in adults than in children. Associated neurologic deficits are rare and easily missed at the first clinical examination. The authors describe a case of anterior interosseous nerve palsy after closed Galeazzi fracture. Conservative treatment resulted in complete return of normal nerve function, suggesting that this is a neurapraxia type of injury.

[Views necessary for the traumatic wrist].

Wrist injuries are an extremely common occurrence. The origin of these injuries may be due to a fracture of the two bones of the forearm or the wrist bones, which may sometimes require specific X-rays views. They are rarely responsible for a dislocation of the wrist. Strains wrists may be misdiagnosed and may result in instability of the wrist. The initial examination of all wrist injuries should systematically include the following four views: (1) a PAview of the wrist in pronation (with the palm facing downward). (2) a lateral view according to the Meyrueis technique. (3) an oblique view. (4

[Fractures and luxations of the fingers].

Fracture and luxation of the long fingers are particularly frequent in hand trauma. Untreated or poorly treated, they may leave highly disabling sequelae. The aim of treatment is two-fold: to repair the anatomical lesion and to obtain movement as rapidly as possible, the only means of avoiding joint stiffening. It is important to underline that the metacarpo-phalangeal joints usually stiffen in extension and that the proximal interphalangeal joints on the contrary stiffen in flexion. Thus, in case of immobilisation of the hand, an attempt should be made to obtain an "intrinsec plus" immobilisa

Fracture of the index metacarpal base with subluxation of the trapeziometacarpal joint. A case report.

A 40-year-old man fell on his outstretched arm and suffered a fracture of the index metacarpal base with subluxation of the thumb basal joint. The small fracture fragment at the base of the index metacarpal was attached to the base of the thumb metacarpal by a strong ligament, as noted at the time of surgery. This pattern of injury, a ligament-reversed Bennett's fracture, seems not to have been previously reported.

Locked facets with fracture of the neural arch of the axis.

This report describes a rare variant represented by the association of locked facets at C2-3 with a fracture of the neural arch of the axis. Fracture of the neural arch of the axis is a broad term including the Hangman fracture, traumatic spondylolisthesis of the axis, fractures of the arch of the axis, or the more recently described fractures of the ring of the axis.
