Modified approach for ethmoid and anterior skull base surgery.

The use of combined craniofacial resection is well established for tumors of the ethmoid bone and the anterior aspect of the base of the skull. Mobilization of the medial fronto-orbital ridge improves the transbasal approach and can be performed with a monobloc bone flap. We describe an en bloc bifrontal craniotomy including the supraorbital ridges and the nasal bones. This provides a wider angle of approach to the anterior aspect of the base of the skull than any other method and avoids retraction of the frontal lobes. This type of bone flap procedure can be performed after a wide periosteal …

Mutism: review, differential diagnosis, and report of 22 cases.

Mutism is a common manifestation of both psychiatric and neurologic illness. Psychiatric disturbances associated with mutism include schizophrenia, affective disorders, conversion reactions, dissociative states, and dementias. Neurologic disorders producing mutism involve the basal ganglia, frontal lobes, or limbic system structures. In psychiatric and neurologic conditions, mutism is often associated with other signs of catatonia. The authors review the literature on mutism, including psychiatric, neurologic, toxic-metabolic, and drug-induced causes. Methods to discriminate among the many cau…

Imaging Diagnosis: Atypical CT Appearance of a Multilobular Osteochondrosarcoma in a Dog.

A 9-year-old female spayed boxer presented for a dorsal cranial mass. Cytology diagnosed a sarcoma. Contrast-enhanced computed tomography (CT) of the head revealed a symmetrical, soft tissue attenuating mass with subtle mineralization and a contrast-enhancing capsule at the dorsal calvarium. There was adjacent frontal bone lysis and contrast enhancement of the parietal lobe. This supported a sarcoma of bone or cartilage origin. Postmortem examination additionally revealed a pulmonary tumor embolus. A final diagnosis of grade III multilobular osteochondrosarcoma (MLO) was made. This is the firs

A Case Report of Early-Onset Alzheimer's Disease Using 18F-FDG PET and 18F-FBB PET.

We describe a 40-year-old female patient who presented with sleep disturbance, intermittent headache, and gradual subjective cognitive decline. 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) showed mild FDG hypometabolism in bilateral parietal and temporal lobes. However, 18F-florbetaben (FBB) amyloid PET revealed diffuse amyloid retention in the lateral temporal cortex, frontal cortex, posterior cingulate cortex/precuneus, parietal cortex, and cerebellum. This finding supports the clinical significance of amyloid imaging in diagnostic work-up of early-onset Alzheimer's diseas

Meningioma and glioma as the first collision brain tumour reported in small animals.

We describe the clinical and pathological features of a brain collision tumour consisting of a fibrous meningioma and an anaplastic oligoastrocytoma in a 14-year-old male neutered French Bulldog. Computed tomography of the brain revealed a poorly defined, intra-axial lesion affecting the left frontal lobe. Following euthanasia, histological examination showed an anaplastic oligoastrocytoma invading the brain parenchyma and an adjacent fibrous meningioma. As synchronous intracranial tumours are rare in animals, the aims of this report are to describe the clinical, imaging and histopathological …

Cerebral Amyloidoma Accompanied by Sjögren's Syndrome: A Case Report and Literature Review.

We present a 69-year-old woman with colorectal cancer and a left frontal lobe tumor that was diagnosed as a cerebral amyloidoma after surgical resection. Further postoperative systemic evaluation revealed another amyloidoma in her hip as well as Sjögren's syndrome. Systemic amyloidosis was not present. To the best of our knowledge, this is the first case of cerebral amyloidoma presenting as one of the multiple localized amyloidomas accompanied by Sjögren's syndrome. We also present a systematic review of 65 cerebral amyloidoma cases reported in the literature over the past 40 years and discuss…

[Symptomatology of Behavioral Variant of Frontotemporal Dementia].

In patients with the behavioral variant of frontotemporal dementia, the core clinical phenotype of frontotemporal lobar degeneration, various social behaviors such as disinhibition, apathy, lack of empathy, stereotypy, and changes in eating behavior occur from the onset of the disease, and progresses slowly with frontal lobe damage. Because there are no disease-specific biomarkers, the diagnosis of frontotemporal dementia is based on the evaluation of behavioral symptoms, with neuroimaging methods and cognitive tests as assisting methods. Although diagnostic criteria are useful, frontotemporal…

Neuroimaging Features of San Luis Valley Syndrome.

A 14-month-old Hispanic female with a history of double-outlet right ventricle and developmental delay in the setting of recombinant chromosome 8 syndrome was referred for neurologic imaging. Brain MR revealed multiple abnormalities primarily affecting midline structures, including commissural dysgenesis, vermian and brainstem hypoplasia/dysplasia, an interhypothalamic adhesion, and an epidermoid between the frontal lobes that enlarged over time. Spine MR demonstrated hypoplastic C1 and C2 posterior elements, scoliosis, and a borderline low conus medullaris position. Presented herein is the fi…

The neuropsychiatric manifestations of Huntington's disease-like 2.

Huntington's disease-like 2 (HDL2) is a rare neuropsychiatric disorder that resembles HD but results from a distinct mutation. The authors present a patient with HDL2, hospitalized for psychiatric management, and they review the neuropsychiatric manifestations of this disorder. Depression, irritability/aggression, and frontal lobe personality changes are common presentations of HDL2 and are comparable to classic HD. Patients with HDL2 may differ from those with HD in having a lower incidence of obsessive-compulsive acts, known suicides, antisocial acts, and changes in sexuality. Clinicians sho…

Development of extensive brain lesions following fingolimod (FTY720) treatment in a patient with neuromyelitis optica spectrum disorder.

We report the case of a patient who developed extensive brain lesions during fingolimod (FTY720) treatment in the TRANSFORMS study. His initial diagnosis was multiple sclerosis, but after encephalopathy anti-aquaporin4 antibody (anti-AQP4 Ab) was detected, it was changed to neuromyelitis optica spectrum disorder. After treatment with fingolimod, he developed bilateral extensive brain lesions. The brain MRI showed lesions predominantly involving the right frontal and parietal lobes, with vasogenic edema and enhancement. He had residual encephalomalacia and no recurrence with steroid treatment o

Imaging diagnosis--polioencephalomalacia in a calf.

A 2-month-old mix-breed calf developed acute blindness and ataxia. Serum thiamine concentration was deficient. In antemortem magnetic resonance imaging there were laminar T2-hyperintense regions extending along the cerebral cortex that primarily affected the gray matter. The lesions were relatively symmetric between the left and right hemispheres but no abnormalities were present at the frontal lobes. At necropsy, laminar autofluorescence of the cerebral cortex was observed under ultraviolet exposure at 365 nm, consistent with a diagnosis of polioencephalomalacia. Polioencephalomalacia in the …

Effect of prostaglandin E1 on cerebral blood flow in patients with chronic cerebral infarction.

The effect of prostaglandin E1 (PGE1) on cerebral blood flow (CBF) was studied in 10 patients with chronic cerebral infarction. Regional cerebral blood flow (rCBF) was measured by single photon emission computed tomography before and after they received PGE1 120 microg daily for 2 weeks. The rCBF of the brainstem, cerebellum, and frontal, temporal, and parietal lobes increased significantly after PGE1 administration. PGE1 also significantly increased the rCBF of the non-infarcted area adjacent to infarction. The results indicate that PGE1 increases the CBF of patients with chronic cerebral inf

["Silent" areas of the brain--a challenge in family practice].

Based on two medical histories found among complaints dealt with by the Norwegian Board of Health the author points out the main symptoms of frontal or temporal lobe failure. Following a history of three weeks one of the patients died as a result of an overlooked cerebral abscess. During the history three different doctors had made wrong diagnostic suggestions, mainly because of the coincidence of an influenza epidemic in the neighbourhood. The other patient died from a cerebral tumour. In his history epilepsy was the main symptom for many months. This diagnosis was overlooked because the doct

Brain SPECT of Creutzfeldt-Jakob disease.

SPECT imaging with I-123 IMP or Tc-99m HMPAO was performed in two patients with Creutzfeldt-Jakob disease to evaluate abnormalities in brain perfusion. In the first patient, there was decreased perfusion of the left frontal, parietal, and temporal lobes. CT and MRI were negative. In the second patient, there was patchy perfusion and slightly decreased perfusion of the right hemisphere. MRI revealed slight brain atrophy and bilateral high intensity areas in the basal ganglia on T2 weighted images. Subsequent SPECT imaging revealed severely decreased perfusion of the entire brain. Subsequent MRI

[Otogenic and rhinogenic brain abscesses in children].

Otogenic and rhinogenic abscesses of the brain in children (25 cases) constitute 4.4% of neurosurgical pathologies and 26.9% of all brain abscesses in children. Rhinogenic abscesses (10 cases) were located in the frontal lobes and otogenic abscesses (15 cases) were found supra- or subtentorially. It is difficult to diagnose brain abscesses in children. In this respect computer tomography is very valuable. All the above patients underwent surgical intervention, which included such methods as aspiration, drainage and block removal. The lethality rate was 36%, although during the last 5 years (9 …

A non-traumatic approach to ventral subarachnoid space near circle of Willis.

A non-traumatic approach to the subarachnoid space near the circle of Willis is demonstrated in this study. The approach is made between the olfactory bulb and frontal lobe. Following perfusion with Evans blue dye no leakage was observed into the parenchyma indicating no tissue damage had occurred. No gross bleeding or cerebral vasospasm was visible in the middle meningeal artery (dural), the surface pial blood vessels and the vessels of the circle of Willis. In conclusion, this technique can be successfully used for studies on experimental subarachnoid hemorrhage simulated on the ventral aspe…
