Influence of age on the surgical outcome after endoscopic sinus surgery for chronic rhinosinusitis with nasal polyposis.

OBJECTIVES: To determine whether patient age is associated with the surgical outcome after endoscopic sinus surgery (ESS) with polypectomy. STUDY DESIGN: A prospective, controlled case series. MATERIALS AND METHODS: This study consisted of 60 patients who were diagnosed as having chronic rhinosinusitis (CRS) with nasal polyposis (NP) that was refractory to medical treatment. Three groups were classified according to patient age: pediatric (5-18 yr), adult (19-65 yr), and geriatric (over 65 yr). We collected 20 patients in each age group after applying the exclusion criteria. The extent of th

In vitro antiseptic properties of an ammonium compound combined with denture base acrylic resin.

BACKGROUND: Denture base acrylic resin is easily colonised by oral endogenous bacteria and Candida spp., and eventually by extra-oral species such as Staphylococcus spp., Pseudomonadaceae or members of Enterobacteriaceae. This microbial reservoir can be responsive for denture related stomatitis and aspiration pneumonia, a life-threatening infection especially in geriatric patients. However, the oral and denture hygiene of dependent elderly individuals is extremely poor. OBJECTIVE: This in vitro study aimed to determine the per cent of a quaternary ammonium compound heat-polymerised in acrylic

Is pain a significant factor in elder self-neglect?

PURPOSE: Pain is a worldwide health concern leading to cognitive impairments, depression, and decline in activities of daily living when poorly controlled. Self-neglect is also a serious public health issue being the most common allegation reported to Adult Protective Services (APS). The purpose of this analysis is to determine whether self-reported pain is associated with validated cases of self-neglect compared with matched controls. METHODS: This is a cross-sectional study of 80 (APS) validated cases of self-neglect aged 65 years and older and 80 control participants recruited from a geria

[Aortic valve replacement in patients over 70 years old: determinants of early death].

INTRODUCTION AND OBJECTIVES: There is controversy regarding the risk factors associated with early death in geriatric patients undergoing aortic valve replacement. We analyzed the risks in these patients and established an accurate model for predicting in-hospital mortality. PATIENTS AND METHOD: Univariate and multivariate analyses were made of the risk factors associated with early death in a group of 129 patients older than 70 years who underwent aortic valve replacement (May 1994-June 2001). The variables obtained by multivariate logistic regression were combined to produce an equation for

Posterior approaches in the management of cervical spondylosis and ossification of the posterior longitudinal ligament.

BACKGROUND: If the cervical lordotic curvature has been well preserved, spondylostenosis or ossification of the posterior longitudinal ligament, with or without instability, may be approached posteriorly in selected older patients (over 65 years of age). Posterior surgical alternatives include the laminectomy with or without fusion, or laminoplasty. However, in younger patients or in geriatric patients with predominantly anterior disease with kyphosis, direct anterior surgical procedures yield better results. METHODS: Laminectomy with medial facetectomy and foraminotomy is classically perform

The Hospital Elder Life Program: a model of care to prevent cognitive and functional decline in older hospitalized patients. Hospital Elder Life Program.

OBJECTIVES: To describe the Hospital Elder Life Program, a new model of care designed to prevent functional and cognitive decline of older persons during hospitalization. PROGRAM STRUCTURE AND PROCESS: All patients aged > or =70 years on specified units are screened on admission for six risk factors (cognitive impairment, sleep deprivation, immobility, dehydration, vision or hearing impairment). Targeted interventions for these risk factors are implemented by an interdisciplinary team-including a geriatric nurse specialist, Elder Life Specialists, trained volunteers, and geriatricians--who wor

A profile of four patterns of vulnerability to functional decline in older general medicine patients in Victoria, Australia: a cross sectional survey.

BACKGROUND: There are limited published data reporting Australian hospitalized elders' vulnerability to functional decline to guide best practice interventions. The objectives of this study were to describe the prevalence of vulnerability to functional decline and explore profiles of vulnerability related to the performance of physical activity in a representative group of elders in a single centre in Victoria, Australia. METHODS: A cross-sectional survey of patients aged ≥ 70 years (Mean age 82.4, SD 7 years) admitted to a general medical ward of an Australian tertiary-referral metropolitan

Prescribing of GLP-1 and GIP/GLP-1 Receptor Agonists and Other Glucose-Lowering Drugs Requiring Special Caution in Elderly Japanese People With Type 2 Diabetes: A Preliminary Report.

BACKGROUND: Adverse drug events are more common in elderly people, prompting the Japan Geriatrics Society to update its "Guidelines for Safe Pharmacotherapy in the Elderly" in 2025. In this revision, glucagon-like peptide-1 receptor agonist (GLP-1RA) and glucose-dependent insulinotropic polypeptide/glucagon-like peptide-1 receptor agonist (GIP/GLP-1RA) were newly added to the list of drugs requiring special caution. This study aimed to descriptively investigate real-world prescribing patterns of GLP-1RAs and their concomitant use with other glucose-lowering drugs (GLDs) requiring special cauti…

From Mission to MOSAIC: Growth and Resilience of an Academic Diversity, Equity, and Inclusion Program.

BACKGROUND: Academic medicine programs face compounded challenges from public health crises, health inequities, and evolving federal policies pertaining to Diversity, Equity, and Inclusion (DEI). Older adults and individuals living with serious illness-particularly those from marginalized communities-experienced greater morbidity and mortality during the COVID-19 pandemic and may continue to see disproportionately negative outcomes amid 2025 federal regulatory changes. METHODS: Within a Department of Geriatrics and Palliative Medicine, a DEI initiative evolved into the MOSAIC Council (Mission…

Cultural Adaptation and Validation of the Oral Frailty Index-8 for Turkish Older Adults.

INTRODUCTION: Oral frailty, a multidimensional decline in oral function involving chewing, swallowing, and oral behaviours, is a recognised precursor of dysphagia, malnutrition, and physical frailty. The Oral Frailty Index-8 (OFI-8) is a brief self-report screening tool for assessing oral frailty. This study's aim is to adapt the OFI-8 into Turkish (OFI-8-TR) and evaluate its psychometric properties among older adults. METHODS: A cross-sectional study was conducted in 341 adults aged ≥ 65 years attending a geriatrics outpatient clinic. Internal consistency (KR-20), test-retest reliability (IC…
