BACKGROUND: Fatal gunshot injury to the brain can cause significant alterations in the neuroendocrine state and myocardial dysfunction. Therefore heart allografts from these donors may result in graft failure following orthotopic heart transplantation (OHTx). We evaluated whether receiving a heart from a donor who died from fatal gunshot wound to the brain independently affected the outcome of transplantation.

Data from 699 cardiac and 290 heart-lung transplants has been analysed to determine the importance of the lymphocytotoxic crossmatch result and panel reactive antibody (PRA) status on graft survival. Donor reactive crossmatching was performed for 636 cardiac transplants. A relationship between the emergence of ventricular arrhythmias and other signs of myocardial ischemia was assessed in 96 coronary patients on the basis of comparative results of the physical and psychoemotional stressful tests, and the nitroglycerin and aspirin tests. Ventricular rhythm disorders may develop in the course of mental exercise in some coronary patients with low hemodynamic values as the only manifestation of myocardial ischemia. When direct measurement is not possible, indirect methods utilising heart rate prediction equations are commonly used, but their validity in children has been challenged.
