Diphenhydramine is an antihistamine available in numerous over-the-counter preparations. Often used for its sedative effects in adults, it can cause paradoxical central nervous system stimulation in children, with effects ranging from excitation to seizures and death. Reports of fatal intoxications in young children are rare. We present five cases of fatal intoxication in infants 6, 8, 9, 12, and 12 weeks old. Clinically significant lingual tonsil hypertrophy is a rare entity in children. We describe a child, status postadenotonsillectomy, with recurrent hypertrophy of the lingual tonsil after carbon dioxide laser ablation. Workup was negative for immunodeficiency, cystic fibrosis, and allergies. The findings of examination of multiple biopsy specimens of the tissue suggested follicular hyperplasia, and cytometery showed no evidence of a lymphoproliferative disorder. A multiwavelength spectrophotometric (WApH) titration method was applied to study several multi-protic histamine H2-receptor antagonists which involved four acid dissociation constants (pKa values) over the pH range of 2-10. OBJECTIVE: This article reviews the use of nonprescription histamine2-receptor antagonists (H2RAs) in the management of secretory disorders of the upper gastrointestinal tract.
DATA SOURCES: The scientific and consumer literature pertaining to the over-the-counter availability and therapeutic uses of H2RAs was reviewed.
STUDY SELECTION: References related to safety, efficacy, comparative efficacy with antacids, and economic impact of H2RA therapy were emphasized. Additional importance was placed on information describing the pharmacist's role in nonprescription H2RA therapy.

The efficacy of the H2 receptor antagonist, cimetidine, in the treatment of a patient with porphyria cutanea tarda (PCT) was evaluated. After administration of cimetidine for 2 weeks, urinary excretion of uroporphyrin (UP) and coproporphyrin (CP) was significantly decreased. Urinary porphyrin levels remained low even after the cessation of cimetidine for 1 week.
