A comparison of smartphones to paper-based questionnaires for routine influenza sentinel surveillance, Kenya, 2011-2012.

BACKGROUND: For disease surveillance, manual data collection using paper-based questionnaires can be time consuming and prone to errors. We introduced smartphone data collection to replace paper-based data collection for an influenza sentinel surveillance system in four hospitals in Kenya. We compared the quality, cost and timeliness of data collection between the smartphone data collection system and the paper-based system. METHODS: Since 2006, the Kenya Ministry of Health (MoH) with technical support from the Kenya Medical Research Institute/Centers for Disease Control and Prevention (KEMRI

A genetic fuzzy system for unstable angina risk assessment.

BACKGROUND: Unstable Angina (UA) is widely accepted as a critical phase of coronary heart disease with patients exhibiting widely varying risks. Early risk assessment of UA is at the center of the management program, which allows physicians to categorize patients according to the clinical characteristics and stratification of risk and different prognosis. Although many prognostic models have been widely used for UA risk assessment in clinical practice, a number of studies have highlighted possible shortcomings. One serious drawback is that existing models lack the ability to deal with the intr

Is it possible to identify cases of coronary artery bypass graft postoperative surgical site infection accurately from claims data?

BACKGROUND: Claims data has usually been used in recent studies to identify cases of healthcare-associated infection. However, several studies have indicated that the ICD-9-CM codes might be inappropriate for identifying such cases from claims data; therefore, several researchers developed alternative identification models to correctly identify more cases from claims data. The purpose of this study was to investigate three common approaches to develop alternative models for the identification of cases of coronary artery bypass graft (CABG) surgical site infection, and to compare the performanc

Comparison of Three Different Types of Wrist Pulse Signals by Their Physical Meanings and Diagnosis Performance.

Increasing interest has been focused on computational pulse diagnosis where sensors are developed to acquire pulse signals, and machine learning techniques are exploited to analyze health conditions based on the acquired pulse signals. By far, a number of sensors have been employed for pulse signal acquisition, which can be grouped into three major categories, i.e., pressure, photoelectric, and ultrasonic sensors. To guide the sensor selection for computational pulse diagnosis, in this paper, we analyze the physical meanings and sensitivities of signals acquired by these three types of sensors

A simulation model of colorectal cancer surveillance and recurrence.

BACKGROUND: Approximately one-third of those treated curatively for colorectal cancer (CRC) will experience recurrence. No evidence-based consensus exists on how best to follow patients after initial treatment to detect asymptomatic recurrence. Here, a new approach for simulating surveillance and recurrence among CRC survivors is outlined, and development and calibration of a simple model applying this approach is described. The model's ability to predict outcomes for a group of patients under a specified surveillance strategy is validated. METHODS: We developed an individual-based simulation

Automatic classification of intracardiac tumor and thrombi in echocardiography based on sparse representation.

Identification of intracardiac masses in echocardiograms is one important task in cardiac disease diagnosis. To improve diagnosis accuracy, a novel fully automatic classification method based on the sparse representation is proposed to distinguish intracardiac tumor and thrombi in echocardiography. First, a region of interest is cropped to define the mass area. Then, a unique globally denoising method is employed to remove the speckle and preserve the anatomical structure. Subsequently, the contour of the mass and its connected atrial wall are described by the K-singular value decomposition an

Fall detection based on body part tracking using a depth camera.

The elderly population is increasing rapidly all over the world. One major risk for elderly people is fall accidents, especially for those living alone. In this paper, we propose a robust fall detection approach by analyzing the tracked key joints of the human body using a single depth camera. Compared to the rivals that rely on the RGB inputs, the proposed scheme is independent of illumination of the lights and can work even in a dark room. In our scheme, a pose-invariant randomized decision tree algorithm is proposed for the key joint extraction, which requires low computational cost during

A MDP model for breast and ovarian cancer intervention strategies for BRCA1/2 mutation carriers.

PURPOSE: Women with BRCA1/2 mutations have higher risk for breast and ovarian cancers. Available intervention actions include prophylactic surgeries and breast screening, which vary significantly in cost, cancer prevention, and in resulting death from other causes. We present a model designed to yield optimal intervention strategies for mutation carriers between the ages of 30 and 65 and any prior intervention history. METHODS: A Markov decision process (MDP) model is developed that considers yearly state transitions for the mutation carriers and state dependent intervention actions. State is

Joint application of audio spectral envelope and tonality index in an e-asthma monitoring system.

This paper presents in detail a recently introduced highly efficient method for automatic detection of asthmatic wheezing in breathing sounds. The fluctuation in the audio spectral envelope (ASE) from the MPEG-7 standard and the value of the tonality index (TI) from the MPEG-2 Audio specification are jointly used as discriminative features for wheezy sounds, while the support vector machine (SVM) with a polynomial kernel serves as a classifier. The advantages of the proposed approach are described in the paper (e.g., detecting weak wheezes, very good ROC characteristics, independence from nois

An automatic mass detection system in mammograms based on complex texture features.

It is difficult for radiologists to identify the masses on a mammogram because they are surrounded by complicated tissues. In current breast cancer screening, radiologists often miss approximately 10-30% of tumors because of the ambiguous margins of lesions and visual fatigue resulting from long-time diagnosis. For these reasons, many computer-aided detection (CADe) systems have been developed to aid radiologists in detecting mammographic lesions which may indicate the presence of breast cancer. This study presents an automatic CADe system that uses local and discrete texture features for mamm

Electrodermal activity in bipolar patients during affective elicitation.

Bipolar patients are characterized by a pathological unpredictable behavior, resulting in fluctuations between states of depression and episodes of mania or hypomania. In the current clinical practice, the psychiatric diagnosis is made through clinician-administered rating scales and questionnaires, disregarding the potential contribution provided by physiological signs. The aim of this paper is to investigate how changes in the autonomic nervous system activity can be correlated with clinical mood swings. More specifically, a group of ten bipolar patients underwent an emotional elicitation pr

Analysis of protein interaction networks for the detection of candidate hepatitis B and C biomarkers.

Hepatitis B virus (HBV) and hepatitis C virus (HCV) infection are the major causes of chronic liver disease, cirrhosis and hepatocellular carcinoma (HCC). The resolution or chronicity of acute infection is dependent on a complex interplay between virus and innate/adaptive immunity. The mechanisms that lead a significant proportion of patients to more severe liver disease are not clearly defined and involve virus induced host gene/protein alterations. The utilization of protein interaction networks (PINs) is expected to identify novel aspects of the disease concerning the patients' immune respo

Variability of the impact of adverse events on physicians' decision making.

BACKGROUND: Physicians frequently differ in their treatment recommendations. However, few studies have examined the reasons underlying these differences. The objective of this study was to examine whether physicians vary in the importance they attach to specific adverse events for two treatment options found in recent randomized controlled trials to have equivalent efficacy and overall toxicity. METHODS: A Max-Diff survey was administered to physicians attending a national scientific conference to quantify the influence of 23 specific adverse events on decision making related to two treatment

Diabetic retinopathy risk prediction for fundus examination using sparse learning: a cross-sectional study.

BACKGROUND: Blindness due to diabetic retinopathy (DR) is the major disability in diabetic patients. Although early management has shown to prevent vision loss, diabetic patients have a low rate of routine ophthalmologic examination. Hence, we developed and validated sparse learning models with the aim of identifying the risk of DR in diabetic patients. METHODS: Health records from the Korea National Health and Nutrition Examination Surveys (KNHANES) V-1 were used. The prediction models for DR were constructed using data from 327 diabetic patients, and were validated internally on 163 patient

The effectiveness of a graphical presentation in addition to a frequency format in the context of familial breast cancer risk communication: a multicenter controlled trial.

BACKGROUND: Inadequate understanding of risk among counselees is a common problem in familial cancer clinics. It has been suggested that graphical displays can help counselees understand cancer risks and subsequent decision-making. We evaluated the effects of a graphical presentation in addition to a frequency format on counselees' understanding, psychological well-being, and preventive intentions. DESIGN: Multicenter controlled trial. SETTING: Three familial cancer clinics in the Netherlands. PARTICIPANTS: Unaffected women with a breast cancer family history (first-time attendees). INTERV

A randomized controlled study about the use of eHealth in the home health care of premature infants.

BackgroundOne area where the use of information and communication technology (ICT), or eHealth, could be developed is the home health care of premature infants. The aim of this randomized controlled study was to investigate whether the use of video conferencing or a web application improves parents' satisfaction in taking care of a premature infant at home and decreases the need of home visits. In addition, nurses' attitudes regarding the use of these tools were examined.MethodThirty-four families were randomized to one of three groups before their premature infant was discharged from the hosp

Analyzing the impact of social factors on homelessness: a fuzzy cognitive map approach.

BACKGROUND: The forces which affect homelessness are complex and often interactive in nature. Social forces such as addictions, family breakdown, and mental illness are compounded by structural forces such as lack of available low-cost housing, poor economic conditions, and insufficient mental health services. Together these factors impact levels of homelessness through their dynamic relations. Historic models, which are static in nature, have only been marginally successful in capturing these relationships. METHODS: Fuzzy Logic (FL) and fuzzy cognitive maps (FCMs) are particularly suited to

Accelerometry-based berg balance scale score estimation.

The objective of the study was to investigate the validity of 3-D-accelerometry-based Berg balance scale (BBS) score estimation. In particular, acceleration patterns of BBS tasks and gait were the targets of analysis. Accelerations of the lower back were measured during execution of the BBS test and corridor walking for 54 subjects, consisting of neurological patients, older adults, and healthy young persons. The BBS score was estimated from one to three BBS tasks and from gait-related data, separately, through assessment of the similarity of acceleration patterns between subjects. The work al

The usability and preliminary effectiveness of a web-based physical activity intervention in patients with knee and/or hip osteoarthritis.

BACKGROUND: A large proportion of patients with knee and/or hip osteoarthritis (OA) do not meet the recommended levels of physical activity (PA). Therefore, we developed a web-based intervention that provides a tailored PA program for patients with knee and/or hip OA, entitled Join2move. The intervention incorporates core principles of the behaviour graded activity theory (BGA). The aim of this study was to investigate the preliminary effectiveness, feasibility and acceptability of Join2move in patients with knee and/or hip OA. METHODS: A non-randomized pilot study was performed among patient
