We believe that malignant involvement of mediastinum is a contraindication to attempted resection of bronchial carcinoma. It was hoped that computed tomographic (CT) scanning might improve our ability to assess the mediastinum prior to thoracotomy. The accuracy of CT scanning, performed with modern techniques, in assessment of the mediastinum of 22 patients with bronchial carcinoma has therefore been determined. Sixty-three patients who had either previously treated primary tumors or unknown primary tumors and developed metastatic cervical adenopathy in their previously untreated necks received the combination of surgery and megavoltage irradiation. Within two years, 12 patients died of intercurrent disease, nine patients died with distant metastases only, and five patients had disease recur at a primary site. Schwann cell features were found on ultrastructural examination of a neurotropic melanoma (de novo type) of the lip. A neurotropic pattern of growth was retained in metastatic tumors in lymph nodes and lung. Melanin was not demonstrable in extra-epidermal tumor cells.

Results of CT and US examinations in 24 patients with testicular tumors yielded similar sensitivity and specificity in the diagnosis of enlarged retroperitoneal lymph nodes found at laparatomy. Therefore, the choice of either imaging modality may be influenced by other factors such as presence of abundant bowel gas, post-operative wounds, surgical clips in the abdomen, cachexia or obesity.

Eighty patients with negative mammograms and proven breast cancer are described. The incidence of negative mammograms is 5 to 7% of all breast cancer in Northern Alberta. In 30 patients, immediate biopsy because of clinical abnormality, and mastectomy, revealed only 3.3% axillary node involvement. The remaining 50 patients gave a history of finding a lump and then had a negative mammogram. Biopsy was delayed for a mean of 45 weeks. Their incidence of axillary node involvement was 42%. Patients with nonseminomatous germ-cell tumors of the testis can be divided into two broad groups. The first includes patients with negative lymphograms or small-volume metastases confined to the abdominal nodes. The overall cure rate with orchiectomy and nodal irradiation is 80%. Fourteen primary melanomas arising in the nipple and areola of the breast were treated by mastectomy and axillary dissection. Four patients had axillary lymph node metastases and all were dead within 3 years of their operation, while the 10 patients with no axillary node involvement were free from recurrent disease 5 years after their operation. On the basis of clinical and anatomical studies, it is suggested that a wide local excision without mastectomy is adequate for the treatment of nipple and areola melanomas.

The various symptoms of 766 patients with nasopharyngeal malignancies (641 carcinomas and 125 malignant lymphomas, confirmed histologically) were analysed statistically from the viewpoint of early diagnosis. Fifty consecutive patients with verified squamous cell carcinoma in the oesophagus or adenocarcinoma had mediastinal metastases. One of 21 patients scopy. Seven of 29 patients with squamous cell carcinoma and mediastinal metastases. One of 21 patients with adenocarcinoma had this finding. It is concluded that patients with squamous cell carcinoma of the oesophagus should be mediastinoscopied and that it is also a worthwhile method in patients with adenocarcinoma.

Fifty-three patients with pulmonary neoplasm were studied by radioaerosol inhalation and perfusion lung scans, bronchoscopy and spirometry. The probability of lymph node metastases could be estimated by the amount of inhaled aerosol deposited at the carina. On thoracotomy, actual mediastinal lymphadenopathy was minimum, if any, in 4 patients without carinal aerosol deposition. Application of the aerosol inhalation lung scan to pulmonary neoplasm provides useful diagnostic and prognostic information regarding mediastinal lymph nodes metastases.

He died having a pancreatic carcinoma. At autopsy, a supravalvular aortic stenosis and a hypoplastic aorta found clinically were confirmed. The end results of 264 patients with vocal cord carcinoma treated by either surgery or radiation therapy at the University of Illinois from 1955 to 1972 are analyzed. Stage I glottic carcinoma can be cured by either surgery or radiation alone (86 percent and 80 percent). Stage II glottic cancer treated by surgery has achieved survival rates of 70 percent. The five-year end results of Stage III glottic cancer are 49 percent for surgery and 30 percent for radiation. The management of glottic carcinoma, according to anatomic site and indications for various modalities are discussed.
