Drug intolerance due to nonspecific adverse effects related to psychiatric morbidity in hypertensive patients.

BACKGROUND: Poor adherence to antihypertensive drug regimens is common and may increase the risk for cardiovascular morbidity and mortality. Adverse effects of the drugs can contribute to poor adherence, but some patients who discontinue several different antihypertensive drugs may misinterpret nonspecific symptoms as adverse effects of the drug because of psychiatric morbidity. We examined the relationship between intolerance to antihypertensive drugs and the presence of panic disorder, panic attacks, anxiety, and depression. METHODS: We included all patients with hypertension who attended a

Increase of Helicobacter pylori-associated corpus gastritis during acid suppressive therapy: implications for long-term safety.

OBJECTIVES: Helicobacter pylori causes chronic active gastritis with predominant localization in the gastric antrum. This predisposes to development of mucosal atrophy, intestinal metaplasia, and eventually, gastric cancer. The effects of acid suppression on H. pylori infection and associated gastritis are unclear. However rapid development of atrophic gastritis has been consistently observed in a number of studies during low acid output. We therefore studied the histological features of antrum and corpus of the stomach before and during acid suppressive therapy. METHODS: Fifty patients with

Prolonged versus short course of indomethacin for the treatment of patent ductus arteriosus in preterm infants.

BACKGROUND: Indomethacin is a prostaglandin inhibitor used to treat patent ductus arteriosus (PDA) in preterm infants. Although indomethacin produces ductal closure in the majority of cases, it is ineffective in up to 40% of patients. Furthermore, the ductus will re-open in up to 35% of infants who initially respond to the drug. A more prolonged course of indomethacin has been studied regarding the potential to achieve higher rates of ductal closure. OBJECTIVES: To determine if a prolonged course of indomethacin (compared to a short course) reduces the rate of treatment failure in preterm inf

Using academic admissions data to predict PA training attrition and academic difficulties: a survival analysis approach.

BACKGROUND: Research studying health professions' student attrition and academic difficulties generally treat these outcomes as dichotomous variables that obscure important time-varying aspects. Scant attention has been paid to incorporating time-varying methods, such as survival analysis, to model attrition and academic difficulties in health professions trainees. OBJECTIVE: The objectives of this study are to (1) identify when students were most likely to attrit from the program and/or experience academic probation and (2) identify predictors of time to attrition and academic probation usin

Feasibility and early onset outcomes of outpatient sleeve gastrectomy.

BACKGROUND: In the United States, the rate of obesity has nearly tripled over the past 60 years to 41.9% of the national population. Consequently, the need for bariatric surgeries for weight loss management has significantly increased, while access to bariatric surgery has decreased. OBJECTIVES: Our Bariatrics Center has performed over 2000 bariatric procedures during the past decade in an outpatient setting, hence, the study aim was to present our data and prove the feasibility of performing sleeve gastrectomy in an outpatient setting. We examined and reported on outcomes and specifically lo

Patterns of progression in patients with synchronous oligometastatic non-small cell lung cancer after systemic and local radical treatment.

Oligoprogressive disease (OPD) occurs when a limited number of disease sites show progression. Data on OPD incidence during systemic therapy for metastatic non-small cell lung cancer (NSCLC) are increasing, while data on progression patterns after systemic therapy with local radical treatment (LRT) in patients with synchronous oligometastatic disease (sOMD) are limited. We conducted a retrospective multicenter study to assess progression patterns, factors associated with progression and survival outcomes in sOMD NSCLC patients treated with systemic therapy and LRT. Between January 2015 and Dec

Robotic Coronary Artery Bypass Grafting: A Narrative Review of Techniques, Evidence, and Future Directions.

This narrative review summarizes the published literature on robotic coronary revascularization, encompassing robotic-assisted minimally invasive direct coronary artery bypass (RA-MIDCAB), totally endoscopic coronary artery bypass (TECAB), and hybrid coronary revascularization (HCR). These are distinct procedures that differ in operative access, conduit harvesting, use of cardiopulmonary bypass (CPB), and typical patient selection, and are treated as such throughout this review rather than as interchangeable techniques. A structured, non-systematic literature search of PubMed/MEDLINE and major

Population Heterogeneity of Diabetes in Indigenous Peoples of the Americas: A Systematic Scoping Review of the Existing Literature.

Background: In the Americas, the number of people living with diabetes is expected to rise from 92 million in 2024 to 120 million by 2050. Indigenous populations may experience distinct biological, environmental, and sociocultural risk factors; however, they are often treated as a homogeneous group in epidemiological research, and consolidated evidence on diabetes prevalence across diverse Indigenous populations remains limited. This scoping review examines the prevalence of diabetes among Indigenous populations in the Americas. Methods: Following PRISMA-ScR guidelines, we conducted a systemat

Patients' experiences of participating in a nurse-led clinic for bowel dysfunction following rectal cancer: A qualitative interview study.

BACKGROUND: An increasing number of patients survive rectal cancer, leading to a rise in the number of patients with late sequelae following surgical and oncological treatments. Consequently, the demand for information, treatment and counselling among these patients is on the rise. Nurse-led clinics, staffed by nurses with specialised expertise in Low Anterior Resection Syndrome and following structured treatment programs, are being established as part of specialised hospital departments in Denmark and other countries in an attempt to increase patient survivorship. AIM: This study aimed to ga

Growth Hormone and Brain Regeneration: Evidence from Clinical Studies in Dementia, Traumatic Brain Injury, and Stroke: A Systematic Review.

Growth hormone (GH) and insulin-like growth factor 1 (IGF-1) play essential roles in the brain, influencing neuronal and dendritic growth, as well as neurotransmission. These effects persist throughout life. Numerous studies in animals and humans have demonstrated the beneficial effects of GH therapy on memory and cognitive function, as well as on the restoration of neuronal function following injury. All nerve cells, including neurons, glia, endothelial, epithelial, and perivascular cells, are affected by the actions of GH/IGF-1. IGF-1, in particular, has been associated with cognitive functi

Does stereotactic ablative body radiotherapy (SABR) added to continued systemic therapy improve time to treatment failure compared with physician's choice of systemic therapy in oligoprogressive ER-positive, HER2-negative advanced breast cancer? Study protocol for a multicentre, randomised, open-label phase II trial in Australian tertiary cancer centres (AVATAR-II).

INTRODUCTION: Oligoprogressive disease (OPD) is a clinically significant pattern of progression observed in patients with oestrogen receptor (ER)-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced breast cancer treated with endocrine therapy (ET) and cyclin-dependent kinase (CDK) 4/6 inhibitors. Stereotactic ablative body radiotherapy (SABR) is an emerging strategy that may ablate resistant subclones and prolong the benefit of systemic therapy. The AVATAR-II trial investigates whether the addition of SABR to continued systemic therapy can delay the need to change treat

Older Kidney Transplant Patients Are Over Immunosuppressed Using Standard Protocols With Differential Sex-Based Complications.

BACKGROUND: Increasing numbers of older patients are undergoing kidney transplantation. While there is evidence for both sex- and age-related immunological variations increasing the risks of immunosuppression (IS), few centers enforce age- or sex-specific IS adjustments. METHODS: We investigated outcomes of 148 kidney transplants performed in our center between April 2009 and March 2019 in recipients aged > 60 years and compared them to outcomes in 272 younger recipients (divided into age groups 18-34, 35-49, and 50-60 years), matched for degree of human leukocyte antigen (HLA) sensitization

Comparative Efficacy of Regenerative Therapies for Diabetic Foot Ulcers: A Network Meta-analysis.

BACKGROUND: Diabetic foot ulcer (DFU) is one of the most dreaded complications of diabetes mellitus and is associated with a high risk of lower limb amputation and mortality. Because the efficacy of conventional treatments for DFUs is often limited, regenerative medicine therapy, such as growth factors and stem cells, may provide a solution to chronic wound healing. In this study, we compared several kinds of regenerative therapy for treating DFUs in terms of efficacy. METHODS: We systemically searched relevant studies on PubMed, Cochrane Library, and EMBASE. Randomized clinical trials on con

Vancomycin levels for Bayesian dose-optimization in critical care: a prospective cohort study.

BACKGROUND: Vancomycin dosing in critically ill patients typically requires monitoring the area under the concentration-time curve/minimum inhibitory concentration (AUC/MIC), often using at least two vancomycin levels (VLs). However, the optimal number of VLs needed for accurate AUC/MIC estimation in this population remains uncertain. This study aimed to determine the minimum number of VLs required to accurately estimate the AUC/MIC in critically ill patients treated with intermittent infusion of vancomycin. METHODS: A prospective cohort study was conducted in critically ill patients, where V

Lower-limb muscle synergies in musician's dystonia: a case study of a drummer.

Musician's dystonia (MD) is a movement disorder characterized by involuntary muscle contractions specifically triggered by playing an instrument. This condition often leads to a loss of fine motor control, threatening the careers of affected musicians. While MD is commonly associated with the hands, it can also affect the lower limbs, particularly in drummers. Understanding the muscle coordination involved in MD is crucial for comprehending its neurological mechanisms, yet the muscle coordination of lower-limb dystonia has not been thoroughly explored. This study aimed to investigate the diffe
