Medical students increasingly engage in community-based initiatives to address health inequities among marginalized populations, yet often lack structured guidance for translating intent into feasible practice. This Practice Note describes how the Exploration, Preparation, Implementation, and Sustainment framework was used to guide a student-led foot and wound care initiative serving people experiencing homelessness or housing insecurity in northern Minnesota. In a setting marked by extreme cold and rising homelessness, students partnered with community organizations and volunteer registered n

There is a growing awareness among dermatology providers of ocular comorbidities in patients with the chronic inflammatory skin disease atopic dermatitis (AD). For example, the prevalence of ocular surface diseases (OSD) such as conjunctivitis is higher in patients with AD than in the general population, and the use of some AD treatments may be associated with OSD. In a recent review published in the Journal of the American Academy of Dermatology, dermatologists and ophthalmologists provided an overview of the different types, etiology, pathophysiology, and practical management of OSD associat

Children in pediatric long-term care (LTC) facilities are commonly infected with respiratory tract viruses as they have many high-risk co-morbidities and require significant interactions with the healthcare team. From previous studies, we know that infected staff can often be the source of transmission of infection to the children. If instituted quickly, infection control practices can help mitigate the spread of infection. We will describe how Sunshine Children's Home and Rehabilitation Center responded to federal and state infection control and prevention mandates in LTC for COVID-19. We wil

This scientific statement summarizes the current state of knowledge related to interstage home monitoring for infants with shunt-dependent single ventricle heart disease. Historically, the interstage period has been defined as the time of discharge from the initial palliative procedure to the time of second stage palliation. High mortality rates during the interstage period led to the implementation of in-home surveillance strategies to detect physiologic changes that may precede hemodynamic decompensation in interstage infants with single ventricle heart disease. Adoption of interstage home m

Schizophrenic patients have traditionally suffered from high rates of cardiovascular disease and early mortality. NICE guidelines suggest that several physical health measures be monitored regularly in these patients, and particularly those on antipsychotic medication, which has a wide side-effect profile that may potentiate the risk of cardiovascular disease and other comorbidities. This general practice audit aimed to determine the rates of physical health monitoring in primary care in patients on antipsychotic medication for over a year for psychotic symptoms or schizophrenia. The search wa

Exertional heat illness (EHI) is a leading cause of preventable death among student athletes. While causes and preventative measures for EHI are known, school districts may not be implementing evidence-based practices. This descriptive, exploratory study explored school policies, resources, and practices of coaches in a mid-Atlantic state in the prevention and identification of EHI; 397 responded. Ninety-three percent knew of EHI, but only 52% scheduled acclimatization. Coaches reporting a heat emergency plan (56%) were significantly more likely to follow other evidence-based recommendations:

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Shared decision-making is a concept originating in the medical field, and it is ideally based on a trustful relationship between the patient and the health professionals involved. Shared decision-making shows potential to strengthen patient autonomy and encourages patients to become involved in decisions regarding their treatment. WHAT DOES THE PAPER ADD TO EXISTING KNOWLEDGE?: A universal concept and understanding of shared decision-making with relevance specifically to psychiatric clinical practice could not be identified in the analysed literature.

OBJECTIVE: Tyrosine kinase inhibitors (TKIs) have transformed the prognosis of chronic myeloid leukemia (CML) into a manageable chronic condition, but TKI intolerance remains a significant issue. This study aimed to quantify the economic burden incurred by oncology practices to manage a CML treatment intolerance episode.
METHODS: This mixed-methods study leveraged interviews with US oncology practice managers (e.g. physicians, nurse leaders, advanced practice providers) and cost estimation analysis to estimate average time and cost per CML patient treatment intolerance episode, defined as an

OBJECTIVES: To explore the reasons for the 40-fold variation in diagnostic testing for genital Chlamydia trachomatis by general practices.
METHODS: A qualitative study with focus groups. We randomly selected urban and rural high and low testing practices served by Bristol, Hereford, and Gloucester microbiology laboratories. Open questions were asked about the investigation of C trachomatis in men and women in different clinical contexts.
RESULTS: The high and low testing practices did not differ in their age/sex make-up or by deprivation indices. There were major differences between high and

Forced displacement has reached unprecedented levels, with over 120 million people uprooted globally and more than 100,000 refugees resettled in the United States in 2024. Refugees arrive with diverse cultural identities, prior professional roles, and experiences of trauma, yet often face systemic barriers, clinician bias, and unmet health needs. This article offers a practical framework grounded in holistic, person-centered values for delivering palliative nursing care to refugee communities. It outlines the complex resettlement journey and highlights how structural inequities, institutional

BACKGROUND: Surgical care increasingly shifts pre- and postoperative care responsibility to patient self-care at home. By using the capability, opportunity, motivation-behaviour (COM-B) framework, patient determinants influencing self-care can be explained.
AIM: The aim was to describe patients' experienced facilitators of and barriers to self-care in surgical fast-track programmes related to capability, opportunity and motivation.
METHODS: A qualitative design study with semi-structured interviews was conducted among 27 general and orthopaedic surgery patients at three Swedish hospitals. Da

The care of patients with pediatric and congenital heart disease is intrinsically complex and relies on coordinated multidisciplinary teamwork. While advances in diagnostics and intervention have improved survival, variability in outcomes persists and is increasingly linked to team performance, decision-making, and organizational culture. This Perspective draws on evidence from pediatric cardiology, healthcare systems research, and organizational science to examine the role of team-based care in pediatric and congenital cardiology. Emphasis is placed on multidisciplinary decision-making, team

Hypertrophic cardiomyopathy (HCM) is a primary myocardial disease that is genetically transmitted as an autosomal dominant trait. Even apical HCM (ApHCM) induces atrial fibrillation (AF) based on underlying left ventricle (LV) diastolic dysfunction, where anticoagulation therapy is recommended. However, anticoagulation for AF in patients at high risk of bleeding is a double-edged sword. A 98-year-old woman living in a nursing home presented to our hospital with sudden-onset dyspnea and palpitation persisting for two hours. The patient had a history of apical HCM and bronchiectasis. The Tübingen Palatal Plate (TPP) is a minimally invasive yet highly effective functional orthodontic treatment for upper airway obstruction in infants with Robin Sequence (RS). It consists of a palatal plate to cover the cleft and a velar extension that shifts the root of the tongue forward. We review our practical experience with this approach. First, upon admission, our local orthodontists perform an (3-D) intraoral scan of the maxilla. Based on the scan data, the TPP is manufactured in a semi-digital workflow. The length and angulation of its extension is checked via awake laryngoscopy and

OBJECTIVE: Those with chronic critical illness (CCI) remain dependent on life-sustaining treatments and increasingly reside in long-term care facilities equipped to meet their needs. The nature of family involvement in care remains undetermined thwarting approaches to mitigate poor family outcomes. The research objective was to explicate family involvement in the care of an individual with CCI who resides in long-term care.
METHODS: In this qualitative research, we used thematic analysis and constant comparative techniques to analyze data from interviews with 38 participants: 11 family member

OBJECTIVE: Alopecia is a frequently occurring side effect of chemotherapy and has high impact on many patients. Currently, scalp cooling is the only method to prevent this chemotherapy-induced alopecia (CIA) and it is effective in about half of the patients. Since determinants of the success are largely unknown, all patients should be prepared for potential hair loss. The objective was to provide up-to-date online information about CIA and scalp cooling to support patients in coping with CIA and in their choice regarding scalp cooling.
METHODS: Essential aspects of delivering information and

Healthcare workers are at risk of occupationally-acquired viral infections such as human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV). HCV is parenterally transmitted and has been found in every part of the world. This cross-sectional study was conducted to establish the awareness and attitude about HCV infections among 210 full-time and part-time nursing students of the Medical College in Bitola, Macedonia from May to June 2009. For this purpose we used a self-administered questionnaire which consisted of three parts. The first included demographic data, the second i
