Vitamin E and other endogenous antioxidants in the central nervous system.

The nervous system is protected from free-radical-induced oxidative damage by a number of antioxidants. These include small-molecular-weight substances such as vitamins C and E as well as the large protein molecules superoxide dismutase, glutathione peroxidase, and others. These antioxidants are often localized in specific portions of the cell. They also show considerable biological interactions with one another. These factors modulate their antioxidant activity. Oxidative stress has been implicated in the pathogenesis of some disorders of the brain; hence, antioxidants have become attractive

The role of allergy in the etiopathogenesis of laryngeal mucosal lesions.

The results of this study showed that allergy is an important factor in the etiopathogenesis of laryngeal mucosal lesions. Despite adequate treatment, no other unfavourable factors appeared to have significant influence on the results of the treatment. It seems that hypersensitivity to different inhalatory and nutritional allergens make laryngeal mucosa more susceptible for adverse action of other factors: vocal misuse, gastroesophageal reflux (GER), smoking, irritants in the surrounding microclimate, endocrinologic disorders, etc. Acting together, all these factors cause the development of la

Wernicke's encephalopathy--causes to consider.

Wernicke's encephalopathy (WE) is a thiamine deficiency disorder and is characterized clinically by the triad of ocular abnormalities, ataxia and disturbances of consciousness. We report on 3 patients with WE, of whom 2 had insufficient thiamine substitution. In the first patient symptoms disappeared during thiamine substitution. In the second patient acute WE was the terminating event in the sequence of parenteral nutrition, lactic acidosis and cardio-pulmonary decompensation. Possibly due to hereditary deficits WE developed in the third patient despite sufficient thiamine substitution. Atten

Wernicke's encephalopathy complicating schizophrenia.

Wernicke's encephalopathy producing an altered mental state may occur in malnourished psychiatric patients even in the absence of alcohol abuse. A case is reported of a woman, aged 61 years, with schizophrenia who refused her medication for four months, had the delusion she was dying from a tumour, and withdrew to her bed with a neglect of her nutrition. She was committed to hospital in a mute unresponsive state and after taking some food and fluids for four days lapsed into coma. She responded within three hours to the administration of thiamine. Clinicians need to remain vigilant to the poss

Management of toxic epidermal necrolysis in a pediatric burn center.

Toxic epidermal necrolysis (TEN) is an acute severe exfoliative skin and mucosal membrane disorder with a clinical picture similar to a total-body scald injury. Toxic epidermal necrolysis shares features with severe erythema multiforme seen in Stevens-Johnson syndrome, and is thought by some to be a maximal expression of this syndrome. Drug-related TEN is uncommon in children. Mortalities of 70% have been reported, and death is usually secondary to the bacterial and metabolic consequences of a large open wound. Over the past two years, four children with probable drug-induced TEN were treated

Use of essential amino acid/dextrose solutions in the nutritional management of patients with acute renal failure.

A review of the literature on the effectiveness of essential amino acid and hypertonic dextrose in the nutritional management of patients with acute renal failure is evaluated critically. Earlier case reports and studies published indicated that the administration of these solutions could lead to lower blood urea nitrogen concentrations, positive nitrogen balance, and various other clinical and metabolic improvements. Recent studies, however, show that various combinations of essential and nonessential amino acids can provide similar results. Some investigators have suggested increasing the co

[The frequency of physiologic vaginal bleeding of newborn infants].

A study of 350 newborn girls revealed vaginal (uterine) bleeding in 25,4%, mostly demonstrable only by chemical micro-methods, but also macroscopically visible in 3,3%. This kind of bleeding appears always in the first week, with its highest frequency on the 5th day of life and must be regarded as a physiologic event. Analogous histologic and cytologic controls confirmed these observations, pointing toward the fact that, as a rule, no menstrual withdrawal bleeding, but degenerative, regressional changes upon the endometrium occur. Due to the rapid withdrawal of maternal hormones, nutritive dis

Infantile Tremor Syndrome and Subdural Hemorrhage in an Infant with Cystic Fibrosis.

Cystic fibrosis (CF), an autosomal recessive disease with multi-system involvement, may present with bleeding in infancy owing to vitamin K malabsorption. Infantile tremor syndrome (ITS) is an obscure condition associated with vitamin B12 and other micronutrient deficiencies, described predominantly in Indian subcontinent. We describe an infant presenting with ITS and chronic subdural hemorrhage. He was subsequently diagnosed to have CF. The ITS and subdural hemorrhage is rarely reported in children with CF. In the background of increasing recognition of CF in Indian children, this case demons

Influence of visual acuity on anxiety, panic and depression disorders among young and middle age adults in the United States.

BACKGROUND: Previous research, albeit limited, has demonstrated an association of visual acuity with depression and anxiety. However, these studies are limited in that they have focused on older adults, used a convenient sample, and/or used a subjective assessment of visual function. As a result, the purpose of this study was to examine the association of objectively-measured visual acuity with depression and anxiety (and panic disorder) among a national sample of young- and middle-age U.S. adults (20-39 years). METHODS: Using data from the 2003-2004 NHANES (n=602), the presence of anxiety, d

Neuropsychological outcomes at midlife following moderate to severe malnutrition in infancy.

OBJECTIVE: To compare neuropsychological profiles of adults who had experienced an episode of moderate to severe protein-energy malnutrition confined to the first year of life with that of a healthy community comparison group. METHOD: We assessed neuropsychological functioning in a cohort of Barbadian adults, all of whom had birth weight >2268 g. The previously malnourished group (N = 77, mean age = 38 years, 53% male) had been hospitalized during the first year of life for moderate to severe protein energy malnutrition and subsequently enrolled in a program providing nutrition education, hom

Two sisters with prenatal growth failure, disproportionate short stature, and feeding difficulties.

A familial short stature syndrome is described in two sisters. Clinical features include severe pre- and post-natal growth failure, stridor, feeding difficulties in the first 2 years requiring nasogastric feeding and facial dysmorphism reminiscent of Three M syndrome. Intellectual function is normal. Skeletal surveys show short long bones, small square iliac bones, short femoral necks and vertebral bodies which are short in the antero-posterior diameter with narrowing of the interpedicular distance inferiorly.

[Epidemic neuropathy: proposal and arguments to rename Strachan disease as Strachan and Madan disease].

INTRODUCTION: Strachan's disease is a condition which mainly affects the nervous system. It is characterized by optic, auditory and peripheral neuropathies and lesions of the skin and mucous membranes. In 1955 Miller Fisher gave it this name, since the clinical condition described by Henry Strachan in Jamaica during the nineteenth century was similar to that seen in Canadian prisoners-of-war in Japanese concentration camps during the Second World War. DEVELOPMENT: Since there are similarities between these clinical disorders and the major neuropathic epidemic seen recently in Cuba, we have re

Medical manifestations of alcoholism in the elderly.

Alcoholism may lead to a great many physical and mental problems in individuals of any age. Elderly alcoholics often have additional problems resulting from the interaction of age related changes in physiology and "heavy" alcohol intake. Some of the more important problems are: Impairment of the immune system with decreased ability to deal with infection or cancer. Increased incidence of hypertension, cardiac arrhythmia, myocardial infarction, and cardiomyopathy. Increased incidence of stroke. Alcohol dementia. Increased incidence of esophageal and other cancers. Cirrhosis and other liver dise

A comparative evaluation of thiol/disulfide homeostasis and ischemia-modified albumin levels in patients with heroin use disorder in remission and detoxification.

INTRODUCTION: Chronic heroin use disturbs body's redox (oxidant-antioxidant) balance, together with dynamic thiol-disulfide balance and ischemia-modified albumin; which are two distinct blood markers that may reflect the oxidative and ischemic load in substance dependence. It is still unclear whether these markers change across the clinical phases of heroin use disorder (HUD), or whether they relate to the affective and behavioral symptoms that dominate the clinical picture. METHODS: In this cross-sectional study, 101 adults meeting DSM-5 criteria for HUD were split into a remission group (n

A Specialized Multinutrient Formula Attenuates High-Fat-Diet-Associated Muscle Dysfunction and Sarcopenic Obesity-Related Features in Adult Mice.

Background/Objectives: Sarcopenia is an age-associated skeletal muscle disorder that may be aggravated by a high-fat diet (HFD). Given the multifactorial pathogenesis of sarcopenia, single-nutrient interventions may be insufficient. This study investigated whether a specialized multinutrient formula could attenuate muscle dysfunction and intramuscular lipid accumulation in adult mice under standard-diet and HFD-fed conditions. Methods: Male C57BL/6J mice were assigned to five groups: young control (YC), adult control (OC), adult control supplemented with the nutritional formula (OCN), adult HF

Comprehensive geriatric assessment identifies multidimensional vulnerabilities in older adults with heart failure.

BACKGROUND/AIM: Heart failure (HF) in older adults is frequently accompanied by geriatric syndromes that remain unrecognized in conventional cardiac evaluations. The vulnerabilities of older adults with and without HF in terms of muscle strength, physical performance, functional capacity, frailty, nutritional status, cognition, mood, anxiety, polypharmacy, and comorbidity were investigated using comprehensive geriatric assessment (CGA). MATERIALS AND METHODS: A cross-sectional study was conducted in two academic outpatient clinics, enrolling 420 adults aged ≥60 years, including 92 (21.9%) wit
