Relationship of the subperiosteal bone collar to metaphyseal lesions in abused infants.

We studied the relationship between the subperiosteal bone collar and forty metaphyseal lesions in specimens obtained at autopsy from ten infants who died with evidence of abuse. The fracture specimens were studied with high-detail radiography and light microscopy. The typical morphological pattern was a fracture extending through the primary spongiosa adjacent to the chondro-osseous junction. As the fracture line approached the cortex, it veered away from the growth plate, undercutting a fragment of bone that was thicker peripherally than it was centrally. Histological examination showed that

Pathological fracture following minimal trauma as the initial presentation of parathyroid carcinoma-associated hyperparathyroidism in a young man: a case report.

BACKGROUND: Parathyroid carcinoma (PC) is a rare malignant endocrine tumor, accounting for approximately 0.005% of all malignancies and representing an uncommon cause of primary hyperparathyroidism. Excessive secretion of parathyroid hormone leads to severe hypercalcemia, resulting in multisystem complications, including skeletal fragility, nephrolithiasis, neuromuscular manifestations, and cardiovascular and neurocognitive disturbances. Preoperative diagnosis remains challenging, as PC frequently mimics benign parathyroid disease both clinically and radiologically. CASE PRESENTATION: A 36-ye

Assessing Bone Mineral Density Using Radiographs of the Hand: A Multicenter Validation.

PURPOSE: Osteoporosis and fragility fractures incur substantial costs to quality of life and global health care systems. The current gold standard for quantifying bone density is dual-energy X-ray absorptiometry. Our hypothesis is that bone density can be accurately predicted with a simpler method using plain radiographs of the hand. METHODS: This retrospective cohort study was undertaken in two centers in the United Kingdom, where the second metacarpal cortical percentage (2MCP) scores were calculated from radiographs. This study included patients who had sustained a distal radius fracture b

Bone imaging in hypoparathyroidism.

Hypoparathyroidism (HypoPT) is an uncommon endocrine disorder characterized by chronic deficiency or absence of parathyroid hormone (PTH), which leads to a profound reduction in bone remodeling. Subjects with HypoPT typically have bone mineral densities (BMDs) by dual-energy X-ray absorptiometry (DXA) above average at all skeletal sites, with greatest scores observed at the lumbar spine. Trabecular bone score (TBS), an indirect measure of bone microarchitecture, also appears to be normal in HypoPT. By peripheral quantitative computed tomography (pQCT) of the radius, volumetric BMD at cancellou

Medical management of osteoporosis and the surgeons' role.

Osteoporosis is a worldwide public health issue and with the aging population the resultant increase in fragility fractures has generated a significant socioeconomic impact. Robust scientific research has increased our knowledge of the endocrine mechanisms and pathophysiology of osteoporosis. This information has led to Level 1 randomized clinical trials which demonstrate the beneficial effects of appropriate regimens in reducing the fracture risk and the coincident mortality. Despite these contributions the public health problem remains and has stubbornly failed many public awareness campaign

Epihyseal Separation of Lower end Humerus in A Neonate-Diagnostic and Management Difficulty.

INTRODUCTION: Birth injuries are common phenomenon. But commonly encountered birth injuries are clavicle fracture, humerus fractures and uncommonly femur fracture. Incidence of Distal humerus Epiphyseal injuries as birth injury are rare. As the first secondary centre around elbow-capitellum appears at 3-9 months, its radilological assessment of alignment with elbow or the radius shaft is not possible in neonates. It is difficult to differentiate elbow dislocation from epiphyseal injury in neonates. CASE REPORT: Here we are reporting a case of lower end humerus epiphyseal injury, as birth inju

Pulsed electromagnetic field during cast immobilization in postmenopausal women with Colles' fracture.

INTRODUCTION: Although Colles' fracture i.e. a dorsally displaced distal radius fracture (DRF) is one of the most common fractures, there is no enough evidence to determine the best form of rehabilitation. OBJECTIVE: To assess whether the use of pulsed electromagnetic field (PEMF) therapy during cast immobilization of DRF provides beneficial effects on pain, edema, wrist range of motion and function, as well as on the frequency of complications immediately after cast removal. METHODS: The prospective randomized controlled study included 60 women over the age of 55 years with extra-articular

[Magnetic resonance imaging of the scaphoid bone].

INTRODUCTION: Can junior doctors in orthopaedic surgery interpret magnetic resonance imaging (MRI) scans of the scaphoid bone and thereby avoid that patients undergo unnecessary immobilization and repeated controls? MATERIAL AND METHODS: Patients suspected of having a fracture of the scaphoid bone and with a negative x-ray were included in the study. The MRI was performed within a few days, and the patient received the results of the MRI the same day in the ambulatory. The junior doctor had received training in MRI diagnosis and was not allowed to confer with another doctor. Subsequently, the

Changes in pediatric fracture patterns presenting to US emergency departments before, during, and after the COVID-19 pandemic.

The purpose of this study was to analyze the demographics of pediatric fracture patients before, during, and after the COVID pandemic using US national emergency department (ED) data. The National Electronic Injury Surveillance System (NEISS) data for the years 2018 through 2021 was extracted for those <16 years of age, and organized into 24 consecutive bimonthly groups. There was a decrease in the number of ED visits for fractures in 2020 and returned to pre-COVID levels by March/April of 2021, except for small hospitals which demonstrated an earlier rebound beginning in late 2020. During the

Functional and radiologic outcomes of non-displaced scaphoid waist fractures in adolescents approaching skeletal maturity: Comparison between conservative treatment and percutaneous screw fixation.

BACKGROUND: Treatment of scaphoid waist fractures is generally conservative in children but surgical in adults, given the relatively high risk of non-union in adults. In adolescents, the required therapeutic strategy is less well defined. The objective of this study was to compare the radiographic and clinical parameters, and the rate of complications, between non-surgical orthopedic treatment (OT) and surgical treatment (ST) by percutaneous screw fixation of these fractures in adolescents approaching skeletal maturity. HYPOTHESIS: ST of non-displaced scaphoid waist fractures in adolescents a

Introductory remarks.

The cause of osteoporosis, a condition in which bone mass is decreased to a point where structural failure may occur, is unknown; many factors that contribute to the development of osteoporosis are known.Bone mass increases until the late twenties, the time when people attain peak bone mass. For a time after peak bone mass is reached, bone loss and formation are approximately equal. Soon after that, probably in the early thirties, an uncoupling of bone synthesis and bone resorption occurs, and a net loss of bone mass begins, a process that can ultimately result in osteoporosis.Bone loss occurs
