Anesthetic management of a child with congenital insensitivity to pain with anhidrosis: A case report.

Congenital insensitivity to pain with anhidrosis (CIPA) is a rare, autosomal recessive disease classified as hereditary sensory and autonomic neuropathy type VI. Patients with CIPA are characterized by insensitivity to pain, episodes of unexplained fever, anhidrosis, self-mutilating behavior, intellectual disability, and autonomic nervous system abnormalities. The clinical features may intrinsically pose anesthetic challenges. We present a case of a patient with CIPA who underwent tumor biopsy under general anesthesia using a Supreme laryngeal mask airway without any complications. The anesthe

Zolpidem dependence in an adult with bipolar affective disorder and epilepsy: A case report.

Zolpidem is a short-acting non-benzodiazepine hypnotic agent, commonly recommended for short-term treatment of insomnia. Zolpidem has less dependence potential than benzodiazepines. Patients with mental illnesses often have disturbed sleep, for which zolpidem is often prescribed. Long-term use and self-medication (in more than recommended doses) are more likely to cause dependence. We report here a case of bipolar affective disorder with epilepsy, who developed dependence to zolpidem and had severe withdrawal symptoms. The management issues are also discussed with review of the literature.

Endoscopic therapy of colorectal anastomotic hematoma with hypertonic dextrose.

Anastomotic dehiscence and leak are dreaded complications after a colorectal resection and can often present with rectal bleeding and pelvic abscess or sepsis. Although most cases of bleeding after gastrointestinal anastomoses are minor and self-limited, major bleeding, as defined by hemodynamic instability or the need for blood transfusions, poses a significant challenge for management. Here we report a case in which a patient presenting with profuse rectal bleeding and pelvic hematoma secondary to a colorectal anastomotic leak was treated endoscopically with 50% dextrose spray then enema.

Small vessel vasculitis of the skin.

Small vessel vasculitis in the skin manifests with palpable purpura on the lower extremities. This clinical presentation prompts a complete physical examination, history, and review of systems, as well as biopsies for routine processing and direct immunofluorescence to confirm the diagnosis. The presence of vasculitis in other organs, associated underlying conditions, and the severity of cutaneous manifestations dictate management. The majority of cases are self-limited, and overall the prognosis is favorable. Still, a subset of patients can have serious complications and chronic or recurrent

Psychopharmacologic management of serious behavioral disturbance in ASD.

Individuals diagnosed with autism spectrum disorders (ASD) often exhibit serious behavioral disturbance (irritability) including severe tantrums, aggression, and self-injury that requires pharmacologic management. Research focused on the treatment of severe irritability has primarily involved the atypical antipsychotics, including risperidone and aripiprazole. Anticonvulsants have also been investigated for targeting serious behavioral disturbance; however findings have been mixed. Advances in the pharmacotherapy of irritability in ASD continue to inform practice. Research is needed to develop

Cold-induced urticaria.

Cold-induced urticaria is a form of physical urticaria that develops on cold exposure in susceptible individuals. The majority of cases have an unknown etiology (primary or idiopathic). In rare forms, it is associated with various disease entities (secondary). The workup of a patient includes a history and physical examination, cold challenge, and a battery of laboratory tests. The management of such patients includes precaution and avoidance measures as well as treatment with antihistamines. Life-threatening symptoms necessitate carrying a self-administered injectable epinephrine.

[Management of hemoptysis in children].

Hemoptysis in children are infrequent and often self-limiting. They are a manifestation of the broader spectrum of pulmonary haemorrhage. Diffuse pulmonary haemorrhages are often associated with diseases of other organs (cardiopathies, systemic diseases). Focal haemorrhages have multiple aetiologies, dominated by bronchopulmonary infections and cystic fibrosis. Fiberoptic bronchoscopy allows one to localise the bleeding, look for local causes and diagnose pulmonary hemosiderosis by BAL. For local lesions and if the medical management fails, bronchial arteriography is indicated to perform the e

[Inpatient behavioral medicine in chronic skin diseases].

Behavioral medicine is a newer approach in the management of various diseases. In the last years, special programs have been developed for atopic dermatitis, psoriasis vulgaris and other chronic skin diseases. An increase in the patient's self-control could be achieved by optimizing the patient's skills in dealing with their skin disease. The article summarizes the principles and procedures of in-patient behavioral medicine, especially the strategies employed by dermatological training groups and psychotherapeutical groups to help the patient deal with their skin disorder.

Epidemiology and natural progression of articular temporomandibular disorders.

The reported prevalence of temporomandibular disorders (TMDs) differs from study to study, probably because of methodologic errors and lack of standardized definitions of TMDs and their characteristics. Classification of TMDs should be in accordance with classification of synovial joint disorders as generally used by rheumatologists and orthopedic surgeons, in which articular disorders are distinguished from nonarticular disorders. Articular temporomandibular disorders appear to be self-limiting in a very high percentage of cases. Because of this nonprogressive nature, the outcome of nonsurgic

Application of behavioral principles in the management of suffocation phobia: a case study of chronic respiratory failure.

This case study presents the first application of behavioral techniques in the management of psychological reactions associated with respiratory weaning. A forty-eight-year-old woman, with a history of brain stem dysfunction and repeated respiratory arrests participated in a four-week behavioral treatment program designed to 1) eliminate suffocation fear and depression, 2) promote respiratory self-sufficiency, and 3) facilitate successful ventilatory weaning. At one year follow-up, the patient continued to display no evidence of respiratory difficulty, depressive symptomatology, or agitated mo

Dermatitis artefacta.

Self-inflicted dermatoses mainly refer to psychiatric disturbances such as psychoses, mental retardation, and personality disorders. Diagnostic clues are found in the nature and the evolution of the lesions as well as in the ambivalent combination of dependency on doctors and hostility toward them revealed in the patient's medical history. Management of dermatitis artefacta patients means dealing with the complex emotional issue of a basic antagonism in the dermatologist-patient relationship. Both the dermatologist and the dermatologic nursing staff should show an understanding and nonaggressi

Cyanide self-poisoning.

Four cases of cyanide self-poisoning were admitted to one hospital over a period of two years. Two of the patients died. The diagnosis in the unconscious patient may be suggested by the finding of bradycardia and the absence of cyanosis (despite inadequate ventilation). The diagnosis can be confirmed in 5 to 10 minutes by a simple test on gastric aspirate, performed by the casualty officer. Cardiac pacing was used in two patients and may have a place in the supportive management of severe cases.

Tips for successful use of commercial automated insulin delivery systems: An expert paper of the Italian working group on diabetes and technology.

In recent years, automated insulin delivery (AID) systems have transformed diabetes care with demonstrated benefits in glucose control, hypoglycemia risk, and psychosocial outcomes. Given that different systems show peculiarities in terms of components, approved indications of use, type of algorithm, modifiable settings, and additional features, with this expert paper, we aim to provide healthcare professionals with device-specific recommendations for the optimization of insulin therapy and diabetes self-management with the five commercial AID systems most commonly used in Italy. In detail, we…

Understanding pediatric cervicofacial non-tuberculous mycobacterial infection.

Cervicofacial non-tuberculous mycobacterial infection should be a part of the differential diagnosis for immunocompetent children ages 1 to 5 years who present with painless submandibular or preauricular lymphadenopathy. Although a benign and self-limiting disease, patients can develop a chronically draining fistula if not diagnosed and treated promptly. The diagnostic process can be managed with a combination of microbiological studies, cytology, laboratory tests, and imaging studies. Surgical excision is the most definitive curative measure. Incision and curettage, medical management with an

The impact of misdiagnosing Bell's palsy as acute stroke.

Idiopathic Bell's palsy can lead to a serious and, sometimes permanently, disfiguring and emotionally challenging facial palsy. Early diagnosis and treatment with corticosteroids are important, as they significantly improve recovery rates. Bell's palsy is a benign condition that should be diagnosed and managed in primary care. Patients who self-present to the emergency department should be managed and discharged without needing admission. We reviewed all patients referred urgently to our hospital with facial weakness and discharged with a diagnosis of Bell's palsy, to explore whether clinician

Neonatal lupus erythematosus.

Neonatal lupus erythematosus (NLE) refers to a clinical spectrum of cutaneous, cardiac, and systemic abnormalities observed in newborn infants whose mothers have autoantibodies against Ro/SSA and La/SSB. The condition is rare and usually benign and self-limited but sometimes may be associated with serious sequelae. We review the pathophysiology, clinical features, and management of infants with this condition. Neonates with NLE should be managed at a tertiary care center. Multidisciplinary team involvement may also be indicated. In mothers with anti-Ro/SSA and/or anti-La/SSB antibodies and inf

Treatment of internal carotid artery dissections with endovascular stent placement: report of two cases.

Extracranial carotid artery dissection may manifest as arterial stenosis or occlusion, or as dissecting aneurysm formation. Anticoagulation and/or antiplatelet therapy is the first-line treatment, but because it is effective and less invasive than other procedures, endovascular treatment of carotid artery dissection has recently attracted interest. We encountered two consecutive cases of trauma-related extracranial internal carotid artery dissection, one in the suprabulbar portion and one in the subpetrosal portion. We managed the patient with suprabulbar dissection using a self-expandable met

[Self-medication with local anaesthetics by glaucoma patients using teletonometry].

BACKGROUND: Self-tonometry, a supplementary measurement of the intraocular pressure in ophthalmology by glaucoma patients using an automatic tonometer, will become more and more important in the future. As long as the self-tonometry has to work in the contact modus with the ocular surface, home application of a topical anaesthetic by the glaucoma patient will be a requirement for a successful measurement. So far no severe problems within this controlled self-medication have been seen. Nevertheless, public health authorities believe patient health is put at high risk by the application of local

User-Centered Development of the MyCare2 Comprehensive Digital Support Platform for Caregivers of Individuals With Cancer.

PURPOSE: Caregivers of adults with cancer face significant emotional and practical challenges. Digital tools may enhance caregiver preparedness and self-efficacy; however, few are developed through rigorous, user-centered approaches that reflect caregivers' lived experiences and needs. The purpose of this study was to develop and refine a prototype of MyCare2, a comprehensive web-based intervention designed to support caregivers in managing care-related tasks and promote their physical and psychosocial well-being. METHODS: We used an iterative, user-centered qualitative design to guide protot

Qualitative application of the RE-AIM/PRISM framework to an educational intervention for improving the care of persons with behavioral and psychological symptoms of dementia.

BACKGROUND: An academic detailing model has improved self-efficacy of memory clinic clinicians to identify and manage complex behavioral and psychological symptoms in persons with dementia (BPSD). The purpose of this report is to describe a systematic approach to adapting a clinician education program previously delivered in two primary care integrated memory clinics for improving the management of BPSD to also be deliverable outside a memory clinic setting, in generalist primary care clinics. The RE-AIM/PRISM implementation framework guided the approach. METHODS: Application of the RE-AIM/PR
