Sleep plays a crucial role in cognitive processes, particularly in memory consolidation, with Non-Rapid Eye Movement (NREM) and Rapid Eye Movement (REM) sleep phases playing distinct roles. Different classes of anxiolytics and sedatives influence sleep architecture and, consequently, memory function, with varying effects on learning, synaptic plasticity, and neurophysiological activity. While some drugs promote sedation and anxiolysis via GABAergic modulation, others act on serotonergic, dopaminergic, or orexinergic pathways, producing divergent effects on memory function. Rodent studies indic…

Postoperative delirium (POD) is a common occurrence for patients in the postanaesthesia care unit (PACU) or intensive care units. A growing body of literature suggests sleep habits may be related to one's risk of developing delirium. However, these sleep habits have yet to be objectively measured via actigraphy during a lengthy follow-up. We analysed 6 days of actigraphy data collected from 54,845 UK Biobank participants who had at least one hospitalisation during an 8-year follow-up. Nap duration (sleep between 9 AM and 7 PM), nap frequency, overnight sleep duration (between 7 PM and 9 AM) an…

Obstructive sleep apnoea-hypopnea syndrome (OSAHS) is characterized by the recurrent occurrence of apnoea and hypopneas during sleep. OSAHS is an independent risk factor for cardiovascular diseases, including hypertension, coronary artery disease, arrhythmias, and stroke. The high prevalence of OSAHS among patients with psychiatric disorders, along with the reciprocal links between these two categories of conditions, deserve emphasis. This association indeed carries diagnostic, prognostic, and therapeutic implications. Diagnosis poses specific challenges, including difficulties to perform slee…

Postoperative sleep disturbance (PSD) is a common yet underrecognized complication after breast cancer surgery and is associated with pain, fatigue, affective symptoms, impaired quality of life, and delayed postoperative recovery. Its management remains challenging because symptom presentations are clinically heterogeneous, and treatment decisions must balance therapeutic efficacy with safety within the broader context of breast cancer care and survivorship. This narrative review summarizes the current evidence on PSD after breast cancer surgery, with particular emphasis on pharmacologic manag…

Obstructive sleep apnea (OSA) is a heterogeneous disorder with varying clinical presentations and pathophysiologic mechanisms. Multiple studies demonstrated increased cardiovascular morbidity and mortality, impaired daytime function and quality of life in moderate to severe OSA. New sleep study metrics are being developed to better predict OSA-related cardiovascular risk that may help to target therapy to patients most likely to benefit. Novel treatments, including hypoglossal nerve stimulation, as well pharmacologic approaches, provide options to personalize therapy.

PURPOSE: Empirical evidence suggests that the duration of the Cutaneous Silent Period (CuSP) is significantly extended in pathological states such as Idiopathic Parkinson's Disease (IPD), Restless Legs Syndrome (RLS), and dystonia, all of which are correlated with the degeneration of dopaminergic neurons. This neurodegenerative process is a critical factor in the etiology of Obstructive Sleep Apnea (OSA). In light of this active participation, the present study sought to assess the CuSP in patients with OSA.
METHODS: A cohort comprising forty individuals diagnosed with OSA and forty healthy c…

OBJECTIVE: As fundamental primary processes in maintaining the body's health, sleep and eating habits influence reciprocally, and this relationship is also modulated by circadian rhythms. Consistently, the literature reports various levels of alterations in sleep parameters and circadian preferences among individuals exhibiting dysfunctional eating behaviors. The present review aims to provide an up-to-date overview of case-control studies conducted to date on this topic.
METHOD: A systematic literature search was conducted to detect case-control studies investigating both subjective and obje…

Obstructive sleep apnea (OSA) is a sleep-related breathing disorder characterized by recurrent collapse of a structurally vulnerable upper airway during sleep. Airway caliber and stability are influenced by multiple anatomical structures, including the nasal cavity, soft palate, lateral pharyngeal walls, tongue base, hyoid complex, and craniofacial skeleton. Detailed knowledge of these structures is therefore essential for rational surgical planning. This narrative review summarizes key anatomical features relevant to upper pharyngeal surgery, including nasal obstruction and the internal nasal…

Obstructive sleep apnea (OSA) syndrome is a severe, debilitating, and pervasive sleep disorder. OSA mainly affects people with obesity, type 2 diabetes mellitus (T2DM), hypertension, and dyslipidemia and is strongly associated with cardiovascular complications. Based on the bidirectional relationship between T2DM and OSA, the latter represents a risk factor for the former, and, vice versa, people with T2DM have a high risk of OSA. Mechanical and hormonal factors, inflammatory mediators, and a dysregulated autonomic nervous system contribute to the mechanisms underlying the disease. Treatment o…

To evaluate the correlation between various sleep and the risk of NAFLD\MAFLD\MASLD. This study included 4772 subjects from the National Health and Nutrition Examination Survey data from 2017 to 2020. Poor sleep factors were defined as: ①abnormal sleep duration (< 7 h or > 8 h); ②snoring; ③sleep apnea; ④self-reported sleep disorder; ⑤ daytime sleepiness. The frequency of each sleep factor was scored, and the scores of all components were summed to obtain a sleep score ranging from 0 to 12. The higher the score, the less healthy the sleep pattern. Then we divided the overall sleep pattern into …

As the brain transitions from wakefulness to sleep, processing of external information diminishes while restorative processes, such as glymphatic removal of waste products, are activated. Yet, it is not known what drives brain clearance during sleep. We here employed an array of technologies and identified tightly synchronized oscillations in norepinephrine, cerebral blood volume, and cerebrospinal fluid (CSF) as the strongest predictors of glymphatic clearance during NREM sleep. Optogenetic stimulation of the locus coeruleus induced anti-correlated changes in vasomotion and CSF signal. Furthe…

A dysregulated circadian system is independently associated with both Obstructive Sleep Apnea (OSA) and cardiovascular disease (CVD). OSA and CVD coexistence is often seen in patients with prolonged untreated OSA. However, the role of circadian dysregulation in their relationship is unclear. Half of the human genes, associated biological pathways, and physiological functions exhibit circadian rhythms, including blood pressure and heart rate regulation. Mechanisms related to circadian dysregulation and heart function are potentially involved in the coexistence of OSA and CVD. In this article, w…

UNLABELLED: In the management of obstructive sleep apnea, the integrative concept of care is gathering momentum. This is an encouraging development to prevent deficiencies or errors due to a lack of professional collaboration since risks, benefits and alternatives of various techniques can be discussed candidly. However, while the deployment of a multidisciplinary team is encouraged, a scientific, evidence-based approach needs to be rigorously engendered to encompass the phenotypic heterogeneity of obstructive sleep apnea, including neurologic, metabolic, craniofacial, and myopathic endotypes.…

The interrelationship between asthma, obesity, and obstructive sleep apnea (OSA) presents a critical area of investigation within sleep medicine, given the rising prevalence of these conditions globally. This article explores the multifactorial interactions among these three disorders that contribute to significant morbidity. Asthma, a chronic inflammatory condition of the airways, is one of the most common chronic respiratory conditions globally. Asthma in people with obesity is associated with poor asthma control, increased asthma severity, and an increased frequency of exacerbations. Obesit…

A primary purpose of sleep for humans is to remove toxins and metabolic wastes from the brain (e.g., Aβ, tau, lactate) that would otherwise build up and compromise brain functionality. There are currently no drugs or devices that have been clinically shown in humans to enhance brain toxin removal, either during sleep or wakefulness. This perspective article focuses on a recently (re)discovered major route of toxin drainage from the human brain through meningeal lymphatic vessels (mLVs) and the primary enhancer of their flow-the cytokine Vascular Endothelial Growth Factor (VEGF). The purpose of…
