Crisis leadership and power dynamics in Portuguese hospitals: organisational learning from COVID-19.

PURPOSE: This paper aims to examine how power struggles, competing leadership logics and governance failures shaped organisational learning and crisis response in Portuguese public hospitals during the COVID-19 pandemic. By analysing tensions between managerial, clinical and political actors, this study explores how fragmented authority and informal leadership networks conditioned hospitals' adaptive capacity under sustained systemic pressure. DESIGN/METHODOLOGY/APPROACH: This study draws on 41 semi-structured interviews with hospital managers, clinical leaders and frontline coordinators acro

Assessing the integration of social protection into infectious disease policies: a multi-country analysis of TB, HIV, and malaria strategies in Sub-Saharan Africa.

BACKGROUND: Tuberculosis (TB), HIV, and malaria continue to impose major economic hardship on households in Sub-Saharan Africa. While global frameworks such as the WHO End TB Strategy and Universal Health Coverage (UHC) emphasize social protection, evidence remains limited on how national policies translate these commitments into practice. OBJECTIVES: This study assessed how national TB, HIV, and malaria policies and strategic plans in Kenya, Malawi, Mozambique, Nigeria, and Zambia define and operationalize social-protection mechanisms to reduce household economic burden. It also examined ali

Restless nights, demanding days: assessment of the prevalence and associated factors of poor sleep quality among medical students in Mogadishu, Somalia.

BACKGROUND: Sleep is a fundamental biological process essential for cognitive performance, emotional regulation, and academic functioning. Medical students are particularly vulnerable to poor sleep quality due to demanding coursework, irregular schedules, and heightened psychological stress. Although global studies have documented a high prevalence of sleep disturbances in this group, evidence from low-resource educational settings such as Somalia remains limited. This study aimed to determine the prevalence of poor sleep quality, identify associated factors, and evaluate whether depressive sy

Synergizing the Behavior Change Wheel and a Cocreative Approach to Design a Physical Activity Intervention for Adolescents and Young Adults With Intellectual Disabilities: Development Study.

BACKGROUND: There is a need for physical activity promotion interventions in adolescents and young adults with intellectual disabilities. Current interventions have shown limited effectiveness, which may be attributed to the absence of theory and a population-specific development. Combining a planning model (including theory) and cocreation with the target audience during intervention development could potentially address this gap. OBJECTIVE: This study aimed to report the systematic development of the Move it, Move ID! intervention by describing how the 8 different steps of the Behavior Chan

Social support, psychological capital, multidimensional job burnout, and turnover intention of primary medical staff: a path analysis drawing on conservation of resources theory.

BACKGROUND: Job burnout is a prevalent and emerging challenge in the primary medical system, causing mass turnover, especially of primary medical staff. Little attention has been paid to the different dimensions of job burnout (emotional exhaustion, personality disintegration, and reduced sense of achievement), which may hinder efforts to tackle high turnover intention among primary medical staff. From the perspective of conservation of resources theory, social support and psychological capital are basic resources with potential to diminish job burnout and thus lower turnover intention. Traffic-related particulate matter affects behavior, inflammation, and neural integrity in a developmental rodent model.

Recent studies indicate that exposure to airborne particulate matter (PM) is associated with cognitive delay, depression, anxiety, autism, and neurodegenerative diseases; however, the role of PM in the etiology of these outcomes is not well-understood. Therefore, there is a need for controlled animal studies to better elucidate the causes and mechanisms by which PM impacts these health outcomes. We assessed the effects of gestational and early life exposure to traffic-related PM on social- and anxiety-related behaviors, cognition, inflammatory markers, and neural integrity in juvenile male rat

Clarifying Associations between Childhood Adversity, Social Support, Behavioral Factors, and Mental Health, Health, and Well-Being in Adulthood: A Population-Based Study.

Previous studies have shown that socio-demographic factors, childhood socioeconomic status (CSES), childhood traumatic experiences (CTEs), social support and behavioral factors are associated with health and well-being in adulthood. However, the relative importance of these factors for mental health, health, and well-being has not been studied. Moreover, the mechanisms by which CTEs affect mental health, health, and well-being in adulthood are not clear. Using data from a representative sample (n = 12,981) of the adult population in Tromsø, Norway, this study examines (i) the relative contribu

Making psychiatric epidemiology useful: the contribution of epidemiology to government policy.

This paper aims to discuss the contribution of epidemiology to aspects of public policy that have either a direct influence on mental health and mental disorders, or an indirect effect by influencing environmental factors which influence mental health. Both kinds of public policy will need to be considered by governments wishing to protect, promote, and improve the mental health of their populations. The paper draws on information from both relatively wealthy and low-income countries. The paper defines epidemiology and mental health policy, sets out the range of government policies which may h

User Needs Assessment for Gestational Diabetes Care and Management Digital Tool: A Qualitative Study.

Gestational diabetes mellitus (GDM) is one of the most common pregnancy complications, with both short- and long-term consequences. We conducted a user needs assessment of GDM care and management through semi-structured interviews with healthcare professionals and women with GDM. Examining current GDM care, we found that time management, number and attendance of appointments, information, resources and education, and cultural social and language differences were affecting factors. Furthermore, participants suggested a digital tool to include notification and prompts, risk stratification, and i

Migraine and Sleep-An Unexplained Association?

Migraine and sleep disorders are common chronic diseases in the general population, with significant negative social and economic impacts. The association between both of these phenomena has been observed by clinicians for years and is confirmed by many epidemiological studies. Despite this, the nature of this relationship is still not fully understood. In recent years, there has been rapid progress in understanding the common anatomical structures of and pathogenetic mechanism between sleep and migraine. Based on a literature review, the authors present the current view on this topic as well

Preparing families to care for ventilated infants at home.

Advances in neonatal care have led to increased survival of infants with complex medical needs and technology dependence. Transition of the ventilator-dependent infant from hospital to home is a complex process that requires extensive coordination between the medical team and family. Home caregivers must be prepared to provide routine care for the ventilator-dependent child and respond to life-threatening emergencies. Families should be counseled on the need for home nursing, medical equipment and an adequate home environment to ensure a safe transition to home. Throughout the process, the fam

Antiviral Agents in Development for Zika Virus Infections.

In 1947, Zika virus (ZIKV), a mosquito-borne flavivirus was identified in Uganda and subsequently spread to Asia and the Pacific regions. In 2015, it was introduced in Brazil causing an important social and sanitary alarm due to its increased virulence and rapid dissemination. Importantly, ZIKV infections have been associated with severe neurological complications such as Guillain-Barré syndrome and microcephaly in fetuses and newborns. Although enormous efforts were made by investigators in the development of effective countermeasures against ZIKV, there is still no approved specific antivira

Parent and teacher ratings of temperamental disposition to social anxiety: the BIS 3-6.

Behavioral inhibition (BI) discourages children from interacting, predisposing them to isolation, social anxiety, and depression. We have created the Behavioral Inhibition Scale for Children Aged 3 to 6 (BIS 3-6) to detect this trait early. Parents and teachers of 365 preschoolers completed different versions of the BIS 3-6 and provided measures of psychopathology. Both versions are structured into 1 factor that explains over 40% of total variability and displays excellent internal consistency (α = .95). The moderate correlations with measures of internalizing symptoms (r = .50) and nonexisten

Eight year follow-up dental treatment in a patient with Treacher Collins syndrome.

Treacher Collins syndrome (TCS) is an autosomal dominant disorder of craniofacial development characterized by variable involvement of the craniofacial structures derived from the first and second branchial arches. Occurrence of this syndrome is relatively rare with wide variations in the clinical expression. Aspects of speech, appearance, and social well-being along with psychological issues are hampered in these patients. Treating such disabled children poses a great challenge not only in the medical field but also in the dental meadow. We report a case of a 5-year-old boy who presented with

Offering to share: how to put heads together in autism neuroimaging.

Data sharing in autism neuroimaging presents scientific, technical, and social obstacles. We outline the desiderata for a data-sharing scheme that combines imaging with other measures of phenotype and with genetics, defines requirements for comparability of derived data and recommendations for raw data, outlines a core protocol including multispectral structural and diffusion-tensor imaging and optional extensions, provides for the collection of prospective, confound-free normative data, and extends sharing and collaborative development not only to data but to the analytical tools and methods

The price of adherence: qualitative findings from HIV positive individuals purchasing fixed-dose combination generic HIV antiretroviral therapy in Kampala, Uganda.

Contrary to early expectations, recent studies have shown near-perfect adherence to HIV antiretrovirals in sub-Saharan Africa We conducted qualitative interviews with patients purchasing low-cost, generic antiretroviral therapy to better understand the social dynamics underlying these findings. We found that concerns for family well-being motivate adherence, yet, the financial sacrifices necessary to secure therapy may paradoxically undermine family welfare. We suggest that missed doses may be more due to a failure to access medication rather than a failure to adhere to medications, and that s
