Alterations in trabecular bone microarchitecture in the ovine spine and distal femur following ovariectomy.

Osteoporosis is a bone disease resulting in increased fracture risk as a result of alterations in both quantity and quality of bone. Bone quality is a combination of metabolic and microarchitectural properties of bone that can help to explain the increased susceptibility to fracture. Translational animal models are essential to understanding the pathology and for evaluating potential treatments of this disease. Large animals, such as the ovariectomized sheep, have been used as models for post-menopausal osteoporosis. However, long-term studies have not been carried out to observe the effects o

Effect of Modic changes on spinal canal stenosis and segmental motion in cervical spine.

PURPOSE: Few studies have reported the characteristics of Modic changes (MCs) in the cervical spine in contrast to the lumbar spine. The purpose of this study was to identify the prevalence of MCs in the cervical spine and to elucidate the relationship of MCs with spinal canal stenosis and angular motion. METHODS: 437 consecutive, symptomatic patients with neck pain with or without neurogenic symptoms were included in this study. MRI in multiple positions was performed with dynamic motion of the cervical spine in upright, weight-bearing neutral, flexion and extension positions. Type of MC, in…

Intraoperative neurophysiologic monitoring in spine surgery. Developments and state of the art in France in 2011.

Intraoperative spinal cord monitoring consists in a subcontinuous evaluation of spinal cord sensory-motor functions and allows the reduction the incidence of neurological complications resulting from spinal surgery. A combination of techniques is used: somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), neurogenic motor evoked potentials (NMEP), D waves, and pedicular screw testing. In absence of intraoperative neurophysiological testing, the intraoperative wake-up test is a true form of monitoring even if its latency long and its precision variable. A 2011 survey of 117 Fre

Dendritic arborization and spine dynamics are abnormal in the mouse model of MECP2 duplication syndrome.

MECP2 duplication syndrome is a childhood neurological disorder characterized by intellectual disability, autism, motor abnormalities, and epilepsy. The disorder is caused by duplications spanning the gene encoding methyl-CpG-binding protein-2 (MeCP2), a protein involved in the modulation of chromatin and gene expression. MeCP2 is thought to play a role in maintaining the structural integrity of neuronal circuits. Loss of MeCP2 function causes Rett syndrome and results in abnormal dendritic spine morphology and decreased pyramidal dendritic arbor complexity and spine density. The consequences …

Tuberculosis of the lower cervical spine (C5-C6) in a 24-month-old infant.

BACKGROUND: Tuberculosis (TB) of the cervical spine occurs rarely in younger children whereas the presentation and outcome are different from those of adult cervical spine. Because cervical spinal TB in younger children is rarely reported, the clinical characteristics, the treatments, and the expected outcome of treatments in younger children are still unknown. METHODS: We present a case of cervical spine TB in a 24-month-old boy that grounds severe vertebral destruction and an extradural abscess. This child presented with neurological deficit in the form of quadraparesis. We performed anteri…

Kinematics and muscle activities of the lumbar spine during and after working in stooped postures.

Existing biomechanical evidence suggests mechanisms of low back injuries and disorders associated with prolonged stooping. However, no research has tested realistic and more natural stooped work conditions with human subjects in the investigation of the biomechanical responses of the low back in prolonged stooping. The current study was aimed to explore various biomechanical responses of the low back in more realistic and work-related loading and posture conditions of prolonged stooping. Twenty two subjects performed stooped work tasks for 7min with periodic micro-breaks in upright standing, a

Complications of upper cervical spine trauma in elderly subjects. A systematic review of the literature.

UNLABELLED: The frequency of cervical spine trauma in elderly patients is increasing with most injuries occurring in the upper cervical spine. These fractures are associated with a risk of sometimes life-threatening complications, although very few studies have specifically analyzed this. The goal of this study was to identify the incidence of complications in the literature (mortality and morbidity) following upper cervical spine trauma in elderly patients. METHODS: A systematic search was performed on the MEDLINE database without limiting the search by language or date to identify all studi…

Complications due to the use of BMP/INFUSE in spine surgery: The evidence continues to mount.

Increasingly, adverse events (AE) attributed to utilizing BMP/INFUSE (Bone Morphogenetic Protein, Medtronic, Memphis, TN, USA) "off-label" in spine surgery are being reported. In 2008, the Food and Drug Administration (FDA) issued a warning that in anterior cervical spine surgery, the "off-label" use of BMP/INFUSE contributed to marked dysphagia, hematoma, seroma, swelling, and/or the need for intubation/tracheostomy. Subsequent studies have cited the following AE; heterotopic ossification (HO), osteolysis, infection, arachnoiditis, increased neurological deficits, retrograde ejaculation, and …

The stability of osseous metastases of the spine in lung cancer--a retrospective analysis of 338 cases.

BACKGROUND: The objective of this retrospective analysis is to systematically assess osseous lesions on the basis of a validated scoring system in terms of stability and fractures prior to and following radiotherapy in 338 lung cancer patients with bone metastases in the vertebral column. METHODS: The stability of 338 patients with 981 osteolytic metastases in the thoracic and lumbar spine was evaluated retrospectively on the basis of the Taneichi-Score between January 2000 and January 2012. RESULTS: 64% (215 patients) were classified stable prior to radiotherapy. Of the stable osseous metas…

Interobserver agreement for the spine instability neoplastic score varies according to the experience of the evaluator.

OBJECTIVES: To evaluate the interobserver agreement for the Neoplastic Spine Instability Score (SINS) among spine surgeons with or without experience in vertebral metastasis treatment and physicians in other specialties. METHODS: Case descriptions were produced based on the medical records of 40 patients with vertebral metastases. The descriptions were then published online. Physicians were invited to evaluate the descriptions by answering questions according to the Neoplastic Spine Instability Score (SINS). The agreement among physicians was calculated using the kappa coefficient. RESULTS: …

Degenerative alterations of the spine in an Early Mediaeval population from Mannheim-Seckenheim, Germany.

Palaeopathological and palaeoepidemiological analyses of human skeletal remains are some of the most important bases for the reconstruction of life of past populations. The assessment of frequency and degree of pathological alterations contributes to conclusions of a population's health status, labour conditions, and environmental influences. Degenerative diseases of the spine are among the most common lesions observed in archaeological human remains. The large number of excavated Early Mediaeval cemeteries in Germany enables the comparison of contemporary populations increasing the reliabilit…

[Cervicogenic dysphagia: swallowing difficulties caused by functional and organic disorders of the cervical spine].

Cervical spine disorders which can cause swallowing difficulties (cervicogenic dysphagia; CD) are: chronic multisegmental/MS dysfunction (dysfunction=functional blockade) of the facet joints, changes in physiological curvature of the cervical spine, degenerative changes (anterior osteophytes, anterior disc herniation, osteochondrosis, osteoarthritis), inflammatory rheumatic diseases, diffuse idiopathic skeletal hyperostosis, injuries, conditions after anterior cervical spine surgery, congenital malformations and tumors. According to our clinical observations, degenerative changes in the cervic…

The immediate effects of sigmoid colon manipulation on pressure pain thresholds in the lumbar spine.

Visceral manual therapy is increasingly used by UK osteopaths and manual therapists, but there is a paucity of research investigating its underlying mechanisms, and in particular in relation to hypoalgesia. The aim of this study was to investigate the immediate effects of osteopathic visceral mobilisation on pressure pain thresholds. A single-blinded, randomised, within subjects, repeated measures design was conducted on 15 asymptomatic subjects. Pressure pain thresholds were measured at the L1 paraspinal musculature and 1st dorsal interossei before and after osteopathic visceral mobilisation

Accuracy of prognostic scores in decision making and predicting outcomes in metastatic spine disease.

INTRODUCTION: Management of metastatic spinal disease has changed significantly over the last few years. Different prognostic scores are used in clinical practice for predicting survival. The aim of this study was to assess the accuracy of prognostic scores and the role of delayed presentation in predicting the outcome in patients with metastatic spine disease. METHODS: Retrospectively, four years of data were collected (2007-2010). Medical records review included type of tumour, duration of symptoms, expected survival and functional status. The Karnofsky performance score was used for functi…
