tDCS modulates effective connectivity during motor command following; a potential therapeutic target for disorders of consciousness.

Transcranial direct current stimulation (tDCS) is attracting increasing interest as a potential therapeutic route for unresponsive patients with prolonged disorders of consciousness (PDOC). However, research to date has had mixed results. Here, we propose a new direction by directly addressing the mechanisms underlying lack of responsiveness in PDOC, and using these to define our targets and the success of our intervention in the healthy brain first. We report 2 experiments that assess whether tDCS to the primary motor cortex (M1-tDCS; Experiment 1) and the cerebellum (cb-tDCS; Experiment 2) a

ENTF Neuromodulation Yields Reduced Disability After Stroke: An Individual Participant-Level Data Meta-Analysis.

BACKGROUND: Electromagnetic network-targeted field (ENTF) brain stimulation therapy is a promising approach to reduce poststroke disability. Two pilot, randomized, sham-controlled trials showed safety and signals of efficacy. The aim of this study was to perform a pooled analysis with greater statistical power to characterize with precision the effect of ENTF in promoting recovery and reducing disability. METHODS: We pooled individual patient-level data from 2 double-blind, randomized, sham-controlled studies, BQ3 (BrainQ3 Trial; Unique identifier: NCT04039178) and EMAGINE 1 (Electromagnetic

Accelerated intermittent theta burst stimulation targeting personalized fronto-parietal control network improves core symptoms of autism spectrum disorder: a double-blind, randomized controlled trial.

Autism Spectrum Disorder (ASD) presents a substantial global challenge, yet no pharmacological treatments effectively target its core symptoms, especially in individuals with severe cognitive or adaptive impairments. This double-blind, sham-controlled, randomized clinical trial assessed accelerated intermittent theta burst stimulation (iTBS) targeting the personalized fronto-parietal network (FPN) in ASD. Participants (6-30 years) were randomized in a 2:1 ratio to active or sham iTBS (three daily sessions, 1800 pulses/session) over 12 weeks (324k pulses) alongside behavioral training. The prim

Affective dimensions of emotional memory shape corticospinal excitability in the upper and lower limbs in young males.

Emotional memory influences motor control, potentially via the corticospinal tract, yet the factors causing this modulation remain poorly understood. This study investigated how recalling emotional memories modulates corticospinal excitability in upper- and lower-limb muscles and whether these effects are better captured by categorical (e.g., happiness) or dimensional (valence and arousal) models of emotion. In two experiments, transcranial magnetic stimulation was applied over the primary motor cortex while healthy male participants recalled memories associated with five emotional conditions:…

Effects of 52 weeks of precuneus rTMS in Alzheimer's disease patients: a randomized trial.

BACKGROUND: Personalized repetitive transcranial magnetic stimulation (rTMS) of the precuneus (PC) is emerging as a new non-invasive therapeutic approach in treating Alzheimer's disease (AD). Here we sought to investigate the effects of 52 weeks of rTMS applied over the PC on cognitive functions in patients with mild-to-moderate dementia due to AD. METHODS: Forty-eight patients with mild-to-moderate dementia due to AD were enrolled for the study. Of those 31 patients were extended to 52 weeks after being included in a 24-week trial (NCT03778151) with the same experimental design. The trial in…

Simulating tDCS-induced electric fields in stroke patients: Realistic-lesion head models are needed.

INTRODUCTION: Transcranial direct current stimulation (tDCS) is tested as tool for post-stroke rehabilitation in aphasia, and individualized simulations of tDCS-induced electric fields (E-fields) can guide its application. However, the accuracy of simulations is challenged by complex and variable tissue properties of stroke lesions. Here, we assessed the impact of stroke lesions on tDCS-induced E-fields realistically in terms of lesion size, shape, and conductivity. METHODS: Structural and diffusion MRI datasets of stroke patients with aphasia (n = 13, six females, age = 38-70 years) and age-

Effects of atorvastatin-loaded PEGylated liposomes delivered by magnetic stimulation for stroke treatment.

BACKGROUND: Focused magnetic stimulation (MagStim) can temporarily and safely open the blood-brain barrier (BBB) for target delivery. We investigated whether opening the BBB with MagStim and delivering atorvastatin-loaded PEGylated liposomes (LipoStatin) would work synergistically for subacute post-stroke treatment. METHODS: Two weeks after middle cerebral artery occlusion (MCAO), an injection of 15 mg/ml magnetic nanoparticles (MNPs) was performed, followed by 30 min of MagStim, in subacute stroke models. The procedure was conducted over a week, during which MagStim and MNPs were administere

iTBS Improves Behavioral Abnormalities and Synaptic Defects in Fmr1 KO Rats by Regulating the Autophagy Pathway.

Synaptic dysfunction resulting from abnormal autophagy may contribute to the pathogenesis of fragile X syndrome (FXS). Intermittent theta burst stimulation (iTBS) is a novel magnetic stimulation mode that has shown promising therapeutic potential in various neurological disorders. However, the therapeutic effects of iTBS on FXS-related behavior abnormality and its underlying mechanism remain unclear. In this study, we investigated whether iTBS could ameliorate behavioral abnormalities and synaptic dysfunction associated with FXS by modulating the autophagy pathway. This study utilized Fmr1 kno

Comparisons of Accelerated Continuous and Intermittent Theta Burst Stimulation for Treatment-Resistant Depression and Suicidal Ideation.

BACKGROUND: Suicidal ideation is a substantial clinical challenge in treatment-resistant depression (TRD). Recent work demonstrated promising antidepressant effects in TRD patients with no or mild suicidal ideation using a specific protocol termed intermittent theta burst stimulation (iTBS). Here, we examined the clinical effects of accelerated schedules of iTBS and continuous TBS (cTBS) in patients with moderate to severe suicidal ideation. METHODS: Patients with TRD and moderate to severe suicidal ideation (n = 44) were randomly assigned to receive accelerated iTBS or cTBS treatment. Treatm

Repeated anodal high-definition transcranial direct current stimulation over the left dorsolateral prefrontal cortex in mild cognitive impairment patients increased regional homogeneity in multiple brain regions.

Transcranial direct current stimulation (tDCS) can improve cognitive function. However, it is not clear how high-definition tDCS (HD-tDCS) regulates the cognitive function and its neural mechanism, especially in individuals with mild cognitive impairment (MCI). This study aimed to examine whether HD-tDCS can modulate cognitive function in individuals with MCI and to determine whether the potential variety is related to spontaneous brain activity changes recorded by resting-state functional magnetic resonance imaging (rs-fMRI). Forty-three individuals with MCI were randomly assigned to receive

The functional implications and modifiability of resting-state brain network complexity in older adults.

The dynamics of the resting-state activity in brain functional networks are complex, containing meaningful patterns over multiple temporal scales. Such physiologic complexity is often diminished in older adults. Here we aim to examine if the resting-state complexity within functional brain networks is sensitive to functional status in older adults and if repeated exposure to transcranial direct current stimulation (tDCS) would modulate such complexity. Twelve older adults with slow gait and mild-to-moderate executive dysfunction and 12 age- and sex-matched controls completed a baseline resting

Repeated Concussions Impair Behavioral and Neurophysiological Changes in the Motor Learning System.

Background. Concussions affect nearly 3 million people a year and are the leading cause of traumatic brain injury-related emergency department visits among youth. Evidence shows neuromotor regions are sensitive to concussive events and that motor symptoms may be the earliest clinical manifestations of neurodegenerative traumatic brain injuries. However, little is known about the effects repeated concussions play on motor learning. Namely, how does concussion acuity (time since injury) affect different behavioral and neurophysiological components of motor learning? Methods. Using a 3-pronged ap

The effect of frontoparietal paired associative stimulation on decision-making and working memory.

Previous single-site neurostimulation experiments have unsuccessfully attempted to shift decision-making away from habitual control, a fast, inflexible cognitive strategy, towards goal-directed control, a flexible, though computationally expensive strategy. We employed a dual-target neurostimulation approach in 30 healthy participants, using cortico-cortical paired associative stimulation (ccPAS) to target two key nodes: lateral prefrontal cortex (LPFC) and intraparietal sulcus (IPS), to test whether decision-making can be artificially shifted from habitual toward goal-directed control. Inhibition of left anterior intraparietal sulcus shows that mutual adjustment marks dyadic joint-actions in humans.

Creating real-life dynamic contexts to study interactive behaviors is a fundamental challenge for the social neuroscience of interpersonal relations. Real synchronic interpersonal motor interactions involve online, inter-individual mutual adaptation (the ability to adapt one's movements to those of another in order to achieve a shared goal). In order to study the contribution of the left anterior Intra Parietal Sulcus (aIPS) (i.e. a region supporting motor functions) to mutual adaptation, here, we combined a behavioral grasping task where pairs of participants synchronized their actions when p

The role of the human cerebellum in linguistic prediction, word generation and verbal working memory: evidence from brain imaging, non-invasive cerebellar stimulation and lesion studies.

Lesion studies emphasize the role of the human cerebellum in a variety of cognitive processes. To date, most evidence comes from studies investigating language-related functions, such as verbal short-term/working memory, word generation, or linguistic/semantic predictions. This review summarizes brain imaging, non-invasive cerebellar stimulation and lesion studies in this field. Converging evidence suggests a cerebellar role in error processing and memory encoding although findings are partly contradictory. Future research should focus on common principles of cerebellar processing across diffe

Advances in neurocognitive rehabilitation research from 1992 to 2017: The ascension of neural plasticity.

OBJECTIVE: The last 25 years have seen profound changes in neurocognitive rehabilitation that continue to motivate its evolution. Although the concept of nervous system plasticity was discussed by William James (1890), the foundation for experience-based plasticity had not reached the critical empirical mass to seriously impact rehabilitation research until after 1992. The objective of this review is to describe how the emergence of neural plasticity has changed neurocognitive rehabilitation research. METHOD: The important developments included (a) introduction of a widely available tool that

Detection of proximal conduction blocks using a triple stimulation technique improves the early diagnosis of Guillain-Barré syndrome.

OBJECTIVE: Current diagnostic electrophysiological criteria can miss the early stages of Guillain-Barré syndrome (GBS). We evaluated the diagnostic efficiency of the triple stimulation technique (TST) in highlighting proximal conduction blocks (CBs) in patients who do not meet the electrophysiological criteria for GBS. METHODS: All patients with a diagnosis of clinical GBS referred to our center between September 2014 and January 2016 were included in the study. For patients who did not fulfill the electrophysiological criteria of GBS, we performed the TST examination. RESULTS: Among the 44
