Advancements in Predicting Periodontal Disease Evolution: A Narrative Review of Contemporary Clinical Risk Prediction Systems.

Periodontal diseases are multifactorial chronic inflammatory disorders characterised by progressive destruction of the tooth-supporting apparatus. Predicting the evolution of these diseases remains a significant clinical challenge because microbial, host, behavioral, and systemic determinants interact to produce substantial inter-individual variability. In recent decades, periodontal care has transitioned from a reactive model toward a preventive, risk-oriented approach supported by structured prediction systems. Various clinical risk assessment tools, including the hexagonal periodontal risk

Pilot trial of the Florida-Resources for Enhancing Alzheimer's Caregiver Health (FL-REACH) intervention in an outpatient memory disorders clinic.

OBJECTIVES: Translation of the foundational REACH II intervention for caregivers of persons with Alzheimer's disease and related dementias (ADRD) into practice has been limited. Most interventions generally focus on later-stage caregiving. The FL-REACH intervention was adapted from REACH II, addressing issues of implementation burden and shifting focus to prolonging in-home care through intervention delivered soon after diagnosis. Goals of this single-arm pilot study were to examine effectiveness of a six-session intervention delivered through an outpatient memory disorders clinic to caregiver

Sustainable Solutions for Eye Drop Plastic Waste: Challenges, Innovations, and Environmental Impact.

The widespread use of single-use plastic eye drop bottles in ophthalmology presents a significant environmental challenge. While essential for preventing contamination and ensuring patient safety, these bottles contribute to plastic waste due to their small size, multimaterial composition, and inadequate recycling infrastructure. This review examines the environmental impact of eye drop packaging across various ophthalmic conditions, including dry eye disease, cataract surgery, and glaucoma, highlighting the substantial cumulative burden of plastic waste and carbon emissions. The manuscript ex

Frailty, diabetic foot ulceration and mortality in people diagnosed with type 2 diabetes: Multistate analysis of primary care patients in the UK clinical practice research datalink.

AIMS: Diabetic foot ulceration (DFU) is a serious complication of diabetes. Frailty is common in individuals with type 2 diabetes and has been associated with a higher mortality risk. We investigated the risk of DFU and mortality in relation to frailty in individuals with type 2 diabetes. METHODS: We identified DFU and mortality events between 2000 and 2021 in the Clinical Practice Research Datalink GOLD primary care records linked to hospital and mortality data. Frailty was categorised using the electronic frailty index. We employed parametric multistate modelling to describe the transitions

Early Intervention with a Preformed Eruption Guidance Appliance After the Excision of Cemento-Ossifying Fibroma: A Case Report.

BACKGROUND: Cemento-ossifying fibroma (COF) is a benign, non-aggressive fibro-osseous tumor in which normal bone is replaced by fibrous tissue containing collagen fibers, bone, or cementum-like material. Although COF is rare in children, its occurrence requires careful management due to its potential impact on tooth development and occlusion. Surgical excision is the widely used treatment; however, post-operative occlusal guidance is crucial, particularly in pediatric patients. CASE REPORT: This case report presents the early intervention using a preformed eruption guidance appliance (EGA) fo

A nationwide approach to reduction in anaesthetic gas use: the Dutch Approach to decarbonising anaesthesia.

Anaesthetic gases account for ∼3% of the carbon footprint of the entire healthcare sector and up to 63% of the emissions originating from surgical care. Transitioning to predominant use of total intravenous anaesthesia (TIVA) has been proven a safe and effective strategy to reduce this footprint, yet its adoption has been slow in most countries. Interventions at the national level have been limited to regulatory action (e.g. banning of desflurane) and publication of nonbinding recommendations and best practices. We describe a new approach that we used to drive sustainable change and apply it t

Developing Parental Identity in the Neonatal Intensive Care Unit.

BACKGROUND: Parents of neonatal intensive care unit (NICU) patients become parents in a stressful, unexpected environment. These parents are at risk of impaired bonding with their infants and may experience worse mental health outcomes compared with parents of infants born healthy. A better understanding of how parents develop their identity as a parent in the NICU may facilitate improved parent support. METHODS: This qualitative study analyzed semistructured interviews with parents of NICU patients to explore parental identity development in the NICU. We applied phenomenological and narrativ

Oral Contraceptive-Induced Cerebral Venous Thrombosis: A Case Report.

Cerebral venous thrombosis (CVT) is a rare, potentially life-threatening condition that occurs when blood clots form in the venous sinuses of the brain, leading to impaired venous drainage, increased intracranial pressure, and neurological deficits. We discuss the case of a 32-year-old female who presented with a four-day history of worsening headache, nausea, vomiting, and right-sided weakness. She was on oral contraceptive pills (OCPs) for two years for menstrual regulation. Neuroimaging, including non-contrast CT, CT venography, and MRI with MR venography, confirmed the diagnosis of CVT inv

Preventing the progression of cirrhosis to decompensation and death.

Two main stages are differentiated in patients with advanced chronic liver disease (ACLD), one compensated (cACLD) with an excellent prognosis, and the other decompensated (dACLD), defined by the appearance of complications (ascites, variceal bleeding and hepatic encephalopathy) and associated with high mortality. Preventing the progression to dACLD might dramatically improve prognosis and reduce the burden of care associated with ACLD. Portal hypertension is a major driver of the transition from cACLD to dACLD, and a portal pressure of ≥10 mmHg defines clinically significant portal hypertensi

The role of cangrelor in acute and high-risk PCI settings.

Dual antiplatelet therapy with acetylsalicylic acid (ASA) and an oral P2Y12 inhibitor is the standard of care to prevent thrombotic complications in patients with acute coronary syndromes undergoing percutaneous coronary intervention (PCI). However, the oral administration of P2Y12 inhibitors bears significant limitations in acute and high-risk PCIs, particularly in ST-elevation myocardial infarction (STEMI) patients, especially those presenting with cardiogenic shock (CS) and cardiac arrest (CA). In these cases, factors such as active vomiting, altered physiology, sedatives, mechanical ventil

How do people with a history of sexual abuse experience talking therapies for sexual trauma? A systematic review of qualitative findings.

OBJECTIVES: This systematic review investigated the qualitative experiences of sexual abuse survivors in talking therapy treatments with respect to the helpful and hindering aspects of therapy. METHODS: Searches were conducted on five databases; PubMed, Web of Science, PyscInfo, PsycArticles and PsycExtra and through searching reference lists. Papers were included where they provided qualitative primary data of client experiences of therapy, did not involve specialist clinical populations and were written in English. Papers were included irrespective of whether study participants had experien

Ethical considerations in access to heart transplant for adults with congenital heart disease.

PURPOSE OF REVIEW: Heart failure is a final common pathway for many patients with adult congenital heart disease (ACHD). As such, ACHD heart transplantation is increasingly being considered and performed. Ethical considerations exist at each step in the transplantation process. RECENT FINDINGS: ACHD patients face many challenges to successful heart transplantation. A dearth of ACHD cardiologists and many barriers to transplant consideration highlight inequities related to recognition of end-stage disease and timely referral for advanced therapies. Certain vulnerable ACHD sub-groups may more e

Maternal and Neonatal Factors Associated with Delayed Closure of Ductus Arteriosus in Term-Born Neonates: Insights from the Copenhagen Baby Heart Cohort Study.

INTRODUCTION: The prenatal shunt, ductus arteriosus (DA), typically closes during the cardio-pulmonary transition at birth. We evaluated maternal and neonatal factors associated with delayed closure of DA in term-born neonates. METHODS: We conducted a retrospective cohort study that included full-term neonates from the prospective observational Copenhagen Baby Heart cohort study. We assessed the association between maternal and neonatal factors and delayed ductal closure. RESULTS: We included 19,566 neonates, of whom 48% were female. Echocardiography was performed at a median age of 12 (IQR:

Modeling potential drugs for Zika virus in animal and in vitro platforms: what is the current state of the art?

INTRODUCTION: The Zika virus (ZIKV) poses a significant public health threat due to its association with congenital Zika syndrome (CZS) and severe neurological disorders. Since its discovery, ZIKV has transitioned from sporadic outbreaks to a major epidemic in Brazil in 2015, which highlighted the urgent need for effective therapies, especially for vulnerable groups like pregnant women and newborns. AREAS COVERED: This review provides a comprehensive overview of recent advancements in ZIKV drug discovery and their current stage of development, with a particular focus on those tested in animal

Acute renal cortical necrosis following postpartum hemorrhage - successful discontinuation of peritoneal dialysis: A case report.

Renal cortical necrosis (RCN) is a rare but severe cause of acute kidney injury primarily observed in obstetric complications. We herein present a case of RCN in a 32-year-old Mongolian primigravida transferred to our hospital with uncontrolled massive postpartum hemorrhage. Contrast-enhanced computed tomography revealed ongoing uterine hemorrhage, prompting endovascular intervention for control. It also showed the loss of cortical perfusion in the kidneys, while preserving medullary blood flow, which is consistent with RCN. The patient remained anuric from transfer and required continuous hem

Living Young With Lupus: A Case Report on the Challenges of Multi-System Disease in a Young Adult.

Systemic erythematosus lupus (SLE) is a multisystem autoimmune disease that can present aggressively within young people. The presence of severe multi-organ disease poses multiple challenges including physical, psycho-social, and, in this case, medico-legal considerations. We present the case of a young person's journey in navigating these complexities. A 16-year-old girl, whilst on holiday abroad, presented with polyarthralgia, weight loss, severe fatigue and lower limb myalgia. Immunological markers revealed high titres of antinuclear antibody (ANA), anti-double-stranded DNA (anti-dsDNA) and

A case of advanced NUTc of the nasal cavity: case report and review of the literature.

Nuclear protein in testis carcinoma (NUTc) is a rare and highly aggressive salivary gland tumor predominantly affecting young patients. It typically involves midline head and neck structures, progresses rapidly, and is associated with a median survival of 6 to 9 months, resulting in an 80% mortality rate within the first year post-diagnosis. In this study, we present a rapidly progressing case of a 28-year-old male diagnosed with NUTc localized to the right nasal cavity (clinical stage cT4bN2M0, IVb). The patient underwent four cycles of cetuximab (anti-EGFR) combined with albumin-bound paclit

SurgflowNet: Leveraging unannotated video for consistent endoscopic pituitary surgery workflow recognition.

Surgical workflow recognition has the potential to accelerate training initiatives through the analysis of surgical videos, improve intraoperative efficiency, and support preemptive postoperative care. Unlike well-explored minimally invasive surgeries, where surgical workflows are consistent across patients, automating endoscopic pituitary surgery workflow recognition is challenging. Pituitary surgery involves a large number of steps, diverse sequences, optional steps, and frequent transitions, making it challenging for current state-of-the-art (SOTA) methods, which struggle with transferabili

Cisnormativity, human rights and vulnerability of young transgender women to HIV: An analysis based on their youth trajectories.

In Brazil, young transgender women (YTW) bear a disproportionate burden of HIV, which persists despite technological advances in biomedical prevention. This study explores how rights violations across different areas of life shape YTW's vulnerability to HIV through an intersectional lens. We conducted 13 in-depth interviews and one group interview with YTW participating in an HIV pre-exposure prophylaxis (PrEP) cohort. Thematic analysis with intersectional sensibility focused on six dimensions: family, schooling, romantic-sexual relationships, friendships, work, and health. The results show th
