Open fractures: it doesn't come out in the wash.

BACKGROUND: Six hours from injury to washout is considered the gold standard in the treatment of open traumatic fractures. Despite this being our hospital policy, the rural nature of our Level I trauma center causes delays in discovery and transport, creating a unique randomization of time to washout. We hypothesized that orthopedic complications after open fractures are related to the severity of the fractures, not the timing of the washout. METHODS: Patients and fractures were reviewed retrospectively over 6.3 years, evaluating for demographics, injury severity, location of fracture, mechan

The adult spinal cord injury without radiographic abnormalities syndrome: magnetic resonance imaging and clinical findings in adults with spinal cord injuries having normal radiographs and computed tomography studies.

BACKGROUND: Spinal cord injury without radiographic abnormalities (SCIWORA) is thought to represent mostly a pediatric entity and its incidence in adults is rather underreported. Some authors have also proposed the term spinal cord injury without radiologic evidence of trauma, as more precisely describing the condition of adult SCIWORA in the setting of cervical spondylosis. The purpose of the present study was to evaluate adult patients with cervical spine injuries and radiological-clinical examination discrepancy, and to discuss their characteristics and current management. METHODS: During

Patterns of maxillofacial injuries related to interaction with horses.

PURPOSE: Studies of trauma patients have described patterns of injuries sustained from unfortunate encounters with large animals. However, the patterns of maxillofacial injuries have yet to be reported. The goal of this investigation was to describe and report on maxillofacial injuries that are associated with interaction with horses. PATIENTS AND METHODS: Charts were selected from the trauma registry by E-code at a level 1 trauma center in Portland, OR. A retrospective review was performed on charts collected from the previous 5 years (1998-2002). Data were collected according to patient, pa

Outcome after traumatic frontal intracerebral haemorrhage: a comparison of unilateral and bilateral haematomas.

Frontal intracerebral haemorrhage (ICH) is a common result of cranial trauma. Outcome differences between bilateral and unilateral frontal ICH are not well studied but would be valuable to predict prognosis in clinical practice. Two aims are proposed in this study: first to compare the risk of developing delayed ICH after bilateral or unilateral frontal ICH, and second to determine the variables helpful to predict outcome according to the Glasgow Outcome Scale (GOS). Between January 1993 and December 1997, 694 consecutive patients with traumatic ICH were admitted to the Chang Gung Medical Cent

Electrical impedance tomography-guided individualized ventilation strategy in patients with trauma-related and postoperative acute respiratory distress syndrome.

OBJECTIVE: Optimal positive end-expiratory pressure (PEEP) selection in acute respiratory distress syndrome (ARDS) is challenging because of marked regional lung heterogeneity. Electrical impedance tomography (EIT) provides bedside assessment of regional ventilation and may support individualized ventilation by informing PEEP titration, but evidence in trauma- and postoperative-associated ARDS remains limited. METHODS: In this single-center randomized controlled trial, 86 adults with moderate-to-severe trauma- or postoperative-associated ARDS were randomized to an EIT-guided individualized ve

Mental illness as an independent risk factor for unintentional injury and injury recidivism.

BACKGROUND: Twelve percent of Americans are diagnosed and treated for mental illness annually. The relationship between mental illness and intentional injuries such as assault and suicide has previously been described. However, unintentional injury among mentally ill adults has not been characterized. The purpose of this study was to identify relationships between mental illness diagnosis and unintentional injury. We hypothesized that diagnosed mental illness is an independent risk factor for unintentional injury and it increases the risk of recidivism. METHODS: In this retrospective cohort s

Early Outcomes in an Emerging Facial Nerve Center: The Oregon Health and Science University (OHSU) Experience.

OBJECTIVES: Nerve transfer (NT) and free gracilis muscle transfer (FGMT) are procedures for reanimation of the paralyzed face. Assessing the surgical outcomes of these procedures is imperative when evaluating the effectiveness of these interventions, especially when establishing a new center focused on the treatment of patients with facial paralysis. We desired to discuss the factors to consider when implementing a facial nerve center and the means by which the specialist can assess and analyze outcomes. METHODS: Patients with facial palsy secondary to multiple etiologies, including cerebello

Endoscopic Third Ventriculostomy in 250 Adults With Hydrocephalus: Patient Selection, Outcomes, and Complications.

BACKGROUND: Endoscopic third ventriculostomy (ETV) has been used predominantly in the pediatric population in the past. Application in the adult population has been less extensive, even in large neurosurgical centers. To our knowledge, this report is one of the largest adult ETV series reported and has the consistency of being performed at 1 center. OBJECTIVE: To determine the efficacy, safety, and outcome of ETV in a large adult hydrocephalus patient series at a single neurosurgical center. In addition, to analyze patient selection criteria and clinical subgroups (including those with ventri

The impact of fathers and male role models on recovery and resilience in pediatric victims of violent trauma.

BACKGROUND: Despite the growing awareness of social determinants of health in pediatric trauma care, the influence of fathers and male caregivers on recovery following violent injury remains underexplored. Understanding this potential protective factor is essential to developing wraparound service models and resilience-building strategies for high-risk youth. METHODS: We conducted a retrospective cohort analysis of a prospectively maintained database of patients aged 0 to 17 enrolled in a hospital-based violence intervention program at an ACS-verified Level I pediatric trauma center. Updating Traumatic Brain Injury Classification for Surgeons: Integrating the CBI-M Framework Into Trauma and Acute Care Practice.

BACKGROUND: Traumatic brain injury (TBI) remains a major cause of death and disability worldwide. Traditional classification based on the Glasgow Coma Scale (GCS) provides a shared clinical language but insufficiently captures the biological heterogeneity, imaging variability, and contextual modifiers that influence outcomes. In January 2024, the National Institute of Neurological Disorders and Stroke (NINDS) convened a multidisciplinary working group to propose the Clinical-Biomarkers-Imaging-Modifiers (CBI-M) framework. This review aims to summarize the rationale, structure, and potential cl

Pilot Simulation Task Trainer for Prehospital Management of Neck Hemorrhage.

INTRODUCTION: Traumatic injuries are the leading cause of death in the U.S. of persons < 45 years of age, with 5-10% of all traumas caused by penetrating neck injuries (PNI). The neck contains several large blood vessels that supply the brain; thus, exsanguination is the leading cause of fatality in PNI. Vascular neck trauma is common in assaults, motor vehicle accidents, battlefields, and sporting events, particularly in ice hockey. There is a general lack of guidance on prehospital management of these injuries, and educating first responders, medics, and sports trainers on how to manage thes

Prevalence of Substance Use and Sleep-Related Problems Among Driver Victims Involved in Road Traffic Accidents Presenting to a Tertiary Care Hospital in India.

BACKGROUND: Road traffic accidents (RTAs) are a critical public health problem leading to significant morbidity, mortality, and socioeconomic losses. Despite known risk factors like substance use and sleep-related problems, there is limited research on the prevalence of these factors among drivers who met with RTAs. Hence, this study aimed to gain insight into the prevalence of substance use and sleep-related problems among this population attending a trauma center in the northern State of India. METHODOLOGY: A cross-sectional study was conducted among 383 driver victims (DVs) who presented t

Timing of regional analgesia in elderly patients with blunt chest-wall injury.

BACKGROUND: Rib fractures represent a typical injury pattern in older people and are associated with respiratory morbidity and mortality. Regional analgesia modalities are adjuncts for pain management, but the optimal timing for their initiation remains understudied. We hypothesized that early regional analgesia would have similar outcomes to late regional analgesia. METHODS: We retrospectively reviewed the American College of Surgeons Trauma Quality Improvement Program database from 2017 to 2019. We included patients ≥65 years old admitted with blunt chest wall trauma who received regional a
