Long-Term Survival After Coverage With Prevertebral Fascia for Abdominal Aortic Stump Closure.

Abdominal aortic graft-enteric fistula is an uncommon but grave complication. Acceptable early results of its management have been reported in recent years, but aortic stump disruption remains a dreaded problem in the remote period. This report describes a case of a 71-year-old male with graft-enteric fistula following after a distant abdominal aortic aneurysm repair. The patient underwent 1-stage operation with extra-anatomic bypass preceding the complete removal of the infected aortic graft and intestinal repair. For coverage of the aortic stump closure, the prevertebral fascia was harvested

Growth hormone axis treatments for HIV-associated lipodystrophy: a systematic review of placebo-controlled trials.

BACKGROUND: HIV-associated lipodystrophy is a disorder of fat metabolism that occurs in patients with HIV infection. It can cause metabolic derangements and negative self-perceptions of body image, and result in noncompliance with highly active antiretroviral therapy (HAART). Growth hormone (GH) axis drugs have been evaluated for treatment of this disorder, but no systematic review has been conducted previously. OBJECTIVES: The aim of the review was to compare the effects of GH axis drugs vs. placebo in changing visceral adipose tissue (VAT), subcutaneous adipose tissue (SAT) and lean body ma

Long-term wound morbidity and hernia recurrence in diabetic patients with HbA1c ≥ 8.5% undergoing open transversus abdominis release: a descriptive longitudinal follow-up of a single-center cohort.

BACKGROUND: Preoperative glycemic control, as measured by HbA1c, is widely used in risk stratification for diabetic patients undergoing abdominal wall reconstruction (AWR). In a prior analysis, our group demonstrated that among diabetic patients undergoing open ventral hernia repair (VHR) with transversus abdominis release (TAR), preoperative HbA1c levels did not correlate with short-term wound morbidity. This study evaluates long-term outcomes in patients with poor preoperative glycemic control (HbA1c ≥ 8.5%) undergoing open AWR with TAR. METHODS: Adult diabetic patients with HbA1c ≥ 8.5% wh

Association between the skeletal muscle-to-visceral fat area ratio and liver fibrosis risk in patients with non-alcoholic fatty liver disease: a cross-sectional study.

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is highly prevalent and closely linked to metabolic dysfunction, with progressive liver fibrosis driving adverse outcomes. Conventional obesity metrics such as body mass index (BMI) and waist circumference inadequately capture body-composition imbalance. The skeletal muscle-to-visceral fat area ratio (SVR) integrates muscle reserve and visceral adiposity and may better reflect metabolic risk. However, large-scale evidence on SVR and fibrosis risk in Chinese patients with NAFLD is limited. This study investigated the association between the

Ultrasound guided-transabdominal plane block (UG-TAPB) reduces pain, opioid consumption and PONV, and is associated with faster recovery for patients undergoing bariatric surgery: a retrospective analysis in a high-volume Italian center.

Ultrasound-guided Transversus Abdominis Plane Block (UG-TAPB) reduces post-operative pain better than i.v. painkillers in patients operated with laparoscopic surgery. This study aims to compare the postoperative course of patients undergoing bariatric surgery treated with UG-TABP to that of patients treated with standard analgesic therapy. We retrospectively analyzed patients who have undergone bariatric surgery from November 2021 to April 2023, comparing patients treated with UG-TAPB (Group A) with patients treated with standard i.v. analgesic therapy (Group B). Post-operative numeric-pain ra

Endoscopic enhanced-view totally extraperitoneal prosthetic (eTEP) versus open Rives-Stoppa repair as a treatment of midline abdominal wall hernias with rectus diastasis: comparison of postoperative pain and length of hospital stay in a single-centre surgical cohort.

The Rives-Stoppa (RS) procedure is a gold standard treatment of midline abdominal wall hernias. Comparability of pain control and outcomes to the enhanced-view totally extraperitoneal prosthetic (eTEP) repair remain unclear. A single-centre retrospective surgical cohort was selected including 30 RS repairs (January 2019-November 2021) and 30 consecutive eTEP procedures (September 2021-August 2022) for midline abdominal wall hernia(s) with rectus abdominis diastasis. Postoperative pain and outcomes were compared up to 1 month. Presence and median duration of patient-controlled analgesia were, r

Association of the addition of a transversus abdominis plane block to an enhanced recovery program with opioid consumption, postoperative antiemetic use, and discharge time in patients undergoing laparoscopic bariatric surgery: a retrospective study.

BACKGROUND: Increasing numbers of laparoscopic bariatric surgeries are being performed and enhanced recovery from anesthesia and surgery (ERAS) protocols have been implemented to optimize care for these patients. We evaluated the effects of an anesthesiologist placed preoperative transversus abdominis plane block (TAP) as part of a bariatric surgery ERAS protocol. We hypothesized that an anesthesiologist placed preoperative TAP added to an ERAS protocol following laparoscopic bariatric surgery would reduce total opioid consumption. METHODS: A retrospective cohort of consecutive patients betwe

Review article: prognostic significance of body composition abnormalities in patients with cirrhosis.

BACKGROUND: Recent advances in evaluation of body composition show body mass index to be inadequate in differentiating between body compartments in cirrhosis. Given the limitations of body mass index, body composition evaluation using computed tomography has been increasingly used as a non-invasive clinical tool with prognostic value. Another factor influencing prognosis includes sex-specific differences in body composition that are seen in cirrhosis. AIM: To review current knowledge regarding the frequency and clinical implications of abnormal body composition features in cirrhosis. METHODS

Clinical study of ultrasound-guided transversus abdominis plane block for analgesia after cesarean section.

BACKGROUND: Patient-controlled intravenous analgesia (PCIA) and patient-controlled epidural analgesia are 2 common methods of maintaining analgesia after cesarean section. In recent years, transversus abdominis plane block (TAPB) has been gradually applied clinically to reduce opioid analgesics and has achieved good results. Therefore, we performed this study to compare the efficacy and side effects of TAPB and PCIA in analgesia after cesarean section. METHODS: One hundred patients who underwent cesarean section were randomly classified into 2 groups. Following surgery, one group underwent ul
