Supramolecular immunomodulatory hydrogelator potentiates CAR-T therapy with long-lasting endogenous immunity toward solid tumor eradication.

The success of chimeric antigen receptor (CAR)-T cell therapy in hematologic malignancies has not been translated to solid tumors, primarily due to inadequate T cell infiltration and an immunosuppressive tumor microenvironment that drives T cell exhaustion. To address these challenges, we developed a tailored immunomodulatory drug-drug conjugate-based hydrogelator for the localized delivery of CAR-T cells targeting solid tumors. This hydrogel forms an in situ scaffold that serves as a sustained-release reservoir, enabling continuous co-delivery of CAR-T cells along with immunomodulatory agents

Repeatable Battery for the Assessment of Neuropsychological Status: Preliminary Utility for Subacute Cognitive Monitoring after Immune Effector Cell-Associated Neurotoxicity Syndrome after Chimeric Antigen Receptor T-Cell Therapy.

Cognitive dysfunction is a common manifestation of immune effector cell-associated neurotoxicity syndrome (ICANS) following chimeric antigen receptor (CAR) T-cell therapy. With CAR-T therapy indications rapidly expanding beyond traditional indications, it has become increasingly important to understand CAR-T therapy-related neurotoxicity and cognitive impairment to optimize treatment safety and to improve monitoring and management. This study examined the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) as a tool for monitoring subacute cognitive recovery. In this pro

Peripheral blood neutrophils contribute to Claudin18.2-specific CAR-T cell treatment resistance in advanced gastric cancer.

BACKGROUND: Claudin18.2 (CLDN18.2)-specific chimeric antigen receptor (CAR)-T cell treatment holds promise for advanced gastric cancer (GC) but has variable efficacy. This study investigates the prognostic value of the neutrophil-to-lymphocyte ratio (NLR) in CAR-T cell treatment and elucidates the molecular mechanisms of treatment resistance. METHODS: GC patients treated with CLDN18.2-specific CAR-T cell treatment were analyzed. Outcomes included objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), and overall survival (OS). Survival analyses utilized Ka

Population Cellular Kinetics of Idecabtagene Vicleucel in Patients with Triple-Class-Exposed Relapsed/Refractory Multiple Myeloma.

BACKGROUND: Idecabtagene vicleucel (ide-cel, ABECMA) is an autologous, B-cell maturation antigen (BCMA)-directed, chimeric antigen receptor (CAR) T-cell therapy. The current modeling analyses aim to characterize the cellular kinetics (CK) of ide-cel and to assess the covariate effects impacting ide-cel cellular kinetics in patients with relapsed/refractory multiple myeloma. METHODS: A modified piecewise model was developed to characterize ide-cel CK through the nonlinear mixed effects method. The model parameterization was conducted using the ide-cel whole-blood transgene data in CK-evaluable

24-Nor-ursodeoxycholic acid improves intestinal inflammation by targeting TH17 pathogenicity and transdifferentiation.

BACKGROUND: 24-Nor-ursodeoxycholic acid (NorUDCA) is a novel therapeutic bile acid for treating immune-mediated cholestatic liver diseases, such as primary sclerosing cholangitis (PSC). OBJECTIVE: Since PSC strongly associates with T helper-type-like 17 (TH17)-mediated intestinal inflammation, we explored NorUDCA's immunomodulatory potential on TH17 cells. DESIGN: NorUDCA's impact on TH17 differentiation was assessed using a CD4+TNaive adoptive transfer mouse model, and on intraepithelial TH17 pathogenicity and transdifferentiation using an αCD3 stimulation model combined with interleukin-17

Cord blood-derived iNK T cells as a platform for allogeneic CAR T cell therapy.

CD1d-restricted invariant Natural Killer (iNK) T cells are a suitable candidate for allogeneic Chimeric Antigen Receptor (CAR) T cell therapy as they do not cause graft-versus-host disease (GvHD) due to the monomorphic nature of CD1d proteins. However, the phenotypic and functional heterogeneity of iNK T cells from adult donors (AD) may lead to the inconstant CAR-iNK T cell products. Cord blood-derived (CB) iNK T cells, in contrast, exhibit inter-donor homogeneity in phenotype including uniform CD4 expression and are enriched in memory iNK T cell populations. Thus, we evaluated the preclinical

Present challenges impacting the management of psychological symptoms in patients receiving CAR-T therapy or stem cell transplantation.

INTRODUCTION: Hematopoietic stem cell (HSCT) and chimeric antigen receptor T-cell (CAR-T) therapy are crucial in the treatment of hematologic malignancies. While HSCT has been long established as a mainstay of the treatment of these diseases, CAR-T is a newer therapy with the potential to change the treatment landscape. Both cellular therapies are time and resource intensive, which has put stressors on patients that are unique from those associated with other forms of oncologic treatment. AREAS COVERED: In this review, we discuss the current state of knowledge of psychological symptoms associ

Thermal potentiation improves IFN-γ production but not cytotoxicity in human CAR-T cells.

OBJECTIVE: Body temperature plays an important role in cancer, with febrile temperature generally associated with improved cancer resistance. In murine models, this resistance has been linked to the cytotoxic T cells, whose differentiation into cancer-killing effector cells is poor at lower but robust at elevated temperatures. If conserved, temperature-mediated potentiation of patient-derived T cells could be implemented to improve the existing cancer treatments, including the chimeric antigen receptor T-cell therapy (CAR T-cell therapy). Here, we tested this possibility using human STEAP1 CAR

Novel PAP-targeted CAR-T therapy enhances antitumor efficacy through CoupledCAR approach.

BACKGROUND: The challenges that remain in the treatment of solid tumors with chimeric antigen receptor (CAR)-T cells include limited solid tumor-specific targets and poor CAR-T cell expansion and function due to limited availability of solid tumor antigens outside the tumor microenvironment. Prostate cancer is the second most common cancer among men worldwide. Current CAR-T therapies for prostate cancer lack specific targets, posing safety risks. To overcome these problems, we identified prostatic acid phosphatase (PAP, also known as ACPP or ACP3) as a feasible CAR-T target for prostate cancer

[Pseudoprogression after CAR-T cell therapy: friend or foe?].

INTRODUCTION: Pseudoprogression is a complication observed following CAR-T therapy that can mimic disease progression; however, its incidence is not well defined. This phenomenon is driven by a robust inflammatory response due to the recognition of CAR-T cells targeting the lymphoma. Misinterpreting pseudoprogression as true disease progression could result in unnecessary alterations to the treatment regimen. CLINICAL CASE: Here, we present a case of a 69-year-old patient with diffuse large B-cell lymphoma (DLBCL) who received axicabtagene ciloleucel as fourth line therapy and exhibited pseud

CD19 -targeted CAR T therapy treating hematologic malignancies: hidden danger is the next neighbor to security?

CD19-targeted chimeric antigen receptor (CAR) T-cell therapy has achieved marvelous results in the treatment of patients with relapsed and/or refractory B-cell lymphomas, B-cell acute lymphoblastic leukemia, and multiple myeloma. As a new treatment method that has changed the existing treatment paradigm, there has been a short time from its emergence to FDA approval. However, with the increasing number of cases and the passage of time, hidden problems have gradually been exposed. In this review, we summarize the short- and long-term toxicity, such as secondary T-cell tumors and lethal CAR tumo

RORing CAR T Cells in Solid and Hematologic Cancers: Same but Different.

Social factors strongly contribute to drug use and relapse, and epidemiological studies have found that members of peer groups influence each other to use drugs. However, previous animal models mostly failed to incorporate social factors and demonstrate the effects of social partners on drug addiction and relapse. In the present study, we investigated the transfer of relapse to cocaine seeking between drug-addicted partners in rats. Male Sprague-Dawley rats were pair-housed and subjected to training and extinction of cocaine self-administration and conditioned place preference (CPP). 24 h afte

Comparative Analysis of Lymphodepletion Regimens in CART-19 Treatment for Relapsed/Refractory Diffuse Large B Cell Lymphoma.

Patients with relapsed/refractory (r/r) diffuse large B-cell lymphoma (DLBCL) undergoing chimeric antigen receptor T cell therapy targeting CD19 (CART-19) receive preinfusion lymphodepletion (LD), most commonly with fludarabine/cyclophosphamide (flu/cy). During the fludarabine shortage, bendamustine was used as an alternative. There are only limited data on how outcomes with flu/cy and bendamustine LD compare. We aimed to compare outcomes and toxicity profiles in r/r DLBCL patients lymphodepleted with flu/cy or bendamustine prior to CART-19 therapy. We conducted a retrospective single-center st

Case Report: Remission of a patient with complex combination of autoimmune diseases by anti-CD19 CAR-T cell therapy.

CAR-T cell therapy has been proven effective in various autoimmune diseases, with most studies utilizing lentiviral-transduced CAR-T cells. In recent years, retroviral vector-transduced CAR-T cells-characterized by a high positivity rate, stable cell lines, and lower plasmid requirements-have attracted increasing attention. This article presents a complex case of a patient with SLE combined with APS and TBIRS. For four years following the diagnosis, the patient underwent conventional steroid therapy and immunotherapy, which yielded unsatisfactory and relapse-prone results. After receiving anti

Chimeric Antigen Receptor T Cell Therapy in Systemic Lupus Erythematosus: Mechanisms, Clinical Advances, and Future Directions a Comprehensive Review.

Systemic lupus erythematosus (SLE) is a chronic, multisystem autoimmune disorder characterized by loss of self-tolerance, immune complex deposition, and progressive organ damage. Despite advances in immunosuppressive therapy, a subset of patients develops treatment-resistant or refractory manifestations; terms used variably in the literature to describe inadequate response to multiple standard immunosuppressants. Chimeric antigen receptor T cell (CAR-T) therapy, a revolutionary modality in oncology, is now emerging as a promising approach in severe autoimmune diseases including SLE. By redirec

Advancing MSC therapy: The next generation of potent mesenchymal stromal cells for systemic autoimmune rheumatic diseases.

Fields that deal with systemic autoimmune diseases such as rheumatology, gastroenterology, and endocrinology have adopted the principles of immune modulation, including shifting immune activation sates, from advances originally developed in oncology. Most clinical trials to date have demonstrated efficacy of cell therapies primarily in in hematologic and solid tumors, largely driven by chimeric antigen receptor T (CAR-T) cells. In contrast, mesenchymal stromal cells (MSCs) have shown limited success in oncology applications. In this article we review the most recent clinical trials involving M

First reported case of a spontaneous and healthy pregnancy in a woman with persistent CAR T-cells 5 years after treatment for diffuse large B-cell lymphoma.

Chimeric antigen receptor T-cell (CAR T-cell) therapy has significantly advanced cancer treatments and remission rates; however, questions exist regarding the impacts on both fertility and immune effects on infants born to mothers who have undergone CAR T-cell therapy. There are no known reported cases of persistence of CAR T-cells after cancer therapy in pregnancy. Here, we present a case of a woman with relapsed refractory diffuse large B-cell lymphoma who undertook an experimental CAR T-cell therapy, had persistence of CAR T-cells 5 years after achieving remission, spontaneously became preg

Safety and efficacy of autologous humanized CD19 CAR-T cell therapy for relapsed/refractory B-cell non-Hodgkin lymphoma.

Chimeric antigen receptor (CAR)-T cells have made great progress in hematological malignancies and optimization of CAR structure is a critical area of exploration. Limited research has evaluated humanized CD19-targeted CAR-T cells (hCART19) in relapsed/refractory (R/R) B-cell non-Hodgkin lymphoma (B-NHL). We conducted a clinical trial to determine the safety of hCART19 for patients with R/R B-NHL. Successful infusion of hCART19 were achieved for 26 patients. Twenty-one (80.8%) patients had grade 1-2 cytokine release syndrome (CRS), with only 1 patient having grade 3 CRS. Besides, no immune eff
