EEG dynamics under anesthetics-induced loss and return of responsiveness with A constant rate infusion.

OBJECTIVE: This study aims to characterize the electroencephalographic dynamics during the transition to and from loss of responsiveness (LOR) induced by three anesthetics. METHODS: High-density EEG was recorded in 60 patients randomly receiving propofol, dexmedetomidine, or ketamine with a constant rate infusion. Spectral power, phase-amplitude coupling (PAC), coherence and directed phase transfer entropy during awake, LOR and recovery (ROR) states were analyzed to evaluate neural dynamics associated with altered conscious states. RESULTS: Propofol increased global alpha power and low-frequ

Deep learning for Alzheimer's disease: advances in classification, segmentation, subtyping, and explainability.

Alzheimer's disease (AD) poses urgent significant challenges for early detection and personalized prognostication. Deep learning (DL) is now regarded as a pivotal technology for extracting subtle imaging and non-imaging biomarkers; yet translating these advances into clinical practice demands a coherent framework. In this review, we first survey input modalities from structural and functional MRI to PET, genetic profiles, and cognitive tests and the key public cohorts that supply multimodal data. We then categorize DL architectures into three complementary pillars: (1) end-to-end classificatio

Neuromodulation and neural networks in psychiatric disorders: current status and emerging prospects.

Psychiatric disorders lead to disability, premature mortality and economic burden, highlighting the urgent need for more effective treatments. The understanding of psychiatric disorders as conditions of large-scale brain networks has created new opportunities for developing targeted, personalised, and mechanism-based therapeutic interventions. Non-invasive brain stimulation (NIBS) techniques, such as transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), can directly modulate dysfunctional neural networks, enabling treatments tailored to the individual's un

Historical Perspective of Allogeneic Hematopoietic Stem Cell Transplantation for Multiple Myeloma.

BackgroundAdvances in novel therapies have improved outcomes for multiple myeloma (MM) patients and the use of allo-SCT has decreased. Current guidelines no longer support allo-SCT as consolidation therapy for newly diagnosed MM, even in high-risk cases.SummaryAllo-SCT is now typically considered only within clinical trials for young, high-risk patients with relapsed or refractory MM (RRMM). It has not proven favorable despite its historical use. CAR T-cell therapy and bispecific antibodies have shown promise in treating triple- and penta-exposed/refractory MM, yet relapse remains common with

A Living Semiartificial Photoelectrocatalytic Biohybrid for Solar CO2 Fixation and Fermentation to Fatty Acids.

To address the global climate and energy crisis, innovative strategies are urgently needed to transform CO2 into sustainable fuels and chemicals. We present a semiartificial biophotoelectrochemical (BPEC) platform, combining solar energy conversion with naturally evolved microbes to develop solutions for transforming CO2 and water into multicarbon products─without sacrificial additives or precious materials. This remains extremely challenging for fully artificial photocatalytic systems. Our system features a scalable and low-cost CuBi2O4 photocathode, stabilized by a thin MgO interlayer, in di

Improved biosynthesis of C4 derivatives by engineered thiolase.

Ethylene glycol (EG), a major product of the enzymatic degradation of polyethylene terephthalate (PET), provides a promising feedstock for sustainable biomanufacturing. Herein, we developed a novel metabolic pathway using Escherichia coli(E. coli) as a host for the biosynthesis of four-carbon compounds such as 1,4-butanediol (1,4-BDO), 1,2,4-butanetriol (1,2,4-BTO), and succinate, from two-carbon substrates such as glycolate and EG. This represents an efficient strategy of using C2 precursors for these high-value chemicals. Through directed evolution of the β-ketoacyl thiolase B of Cupriavidus

Clinical and Financial Impacts of Caring for Mild TBI Patients in the Rural Upper Midwest.

BackgroundHealthcare disparities often cause rural BIG 1 TBI patients to be transferred to a higher level of care due to a fear of clinical decline.MethodsWe conducted a retrospective cohort study from 2020 to 2022 that compared patients with the principal diagnosis of BIG1 TBI who were transferred from rural critical access facilities in the upper Midwest to a tertiary care center vs those who were admitted directly to the same tertiary care center. The primary outcomes were cost and mortality. Statistical significance in mortality rates and length of stay was determined using Pearson's Chi S

Microbial extracellular polymeric substances as biogeobatteries: Mechanisms and geochemical controls of redox-sensitive elements.

Accumulating evidence demonstrates that microbial extracellular polymeric substances (EPS) are a complex mixture of macromolecules at the soil-water-microbe interface, playing an essential role in determining the environmental fate of redox-sensitive elements (RSEs) such as arsenic, chromium, uranium, antimony, and selenium. Microbial EPS include redox-active components like reducing saccharides, redox-active proteins, quinone-like moieties, flavins, and tryptophan that function as "biogeobatteries" (i.e., biological systems capable of storing and transferring electrons), enhancing extracellul

A protocolised approach provides a favourable outcome in patients with severe pelvic trauma.

Background Pelvic fractures are often a result of injuries caused by a major force and lead to considerable morbidity and mortality. Despite advances in acute management protocols involving angioembolization as a prime intervention, mortality in complex pelvic fractures remains high. We analysed our experience with the outcomes of a pelvic trauma management protocol using surgical haemostatic techniques over 5 years. Methods We did a retrospective analysis of all patients admitted with pelvic factures from January 2014 to December 2018. Demographic profile, clinical details, imaging findings,

[Biomarkers and Their Role in Understanding Osteoarthritis].

Osteoarthritis (OA) is a degenerative joint disease characterized by progressive damage and loss of articular cartilage with concomitant structural and functional changes in the joint. It is the most common cause of joint pain globally and the resulting productivity loss to the economy. The clinical symptoms of osteoarthritis are mostly determined by the difficulties of patients related to the development of articular degenerative changes, which secondarily lead to joint stiffness and functional limitation. The diagnosis of this disease is currently based on typical clinical symptoms and radio

Antibody-Drug Conjugates (ADCs): current and future biopharmaceuticals.

Antibody-drug conjugates (ADCs) represent a novel class of biopharmaceuticals comprising monoclonal antibodies covalently conjugated to cytotoxic agents via engineered chemical linkers. This combination enables targeted delivery of cytotoxic agents to tumor site through recognizing target antigens by antibody while minimizing off-target effects on healthy tissues. Clinically, ADCs overcome the limitations of traditional chemotherapy, which lacks target specificity, and enhance the therapeutic efficacy of monoclonal antibodies, providing higher efficacy and fewer toxicity anti-tumor biopharmace

Unveiling a Rare Cause of Dysphagia.

INTRODUCTION: Malignant melanoma of the esophagus is an uncommon cause of dysphagia and accounts for only 0.1-0.2% of esophageal neoplasms. Diagnosis is based on esophagogastroduodenoscopy (EGD) with biopsies and immunohistochemical analysis, the latter being crucial in the amelanocytic variant. Gastrointestinal melanomas are more invasive and comprise poorer prognosis than cutaneous melanomas. CASE PRESENTATION: The authors present the case of a 70-year-old woman admitted to the emergency department for progressive dysphagia with 2 months of evolution. EGD revealed the presence of an extensi

Unraveling the Brain-Heart Axis: More Than Inflammation.

Cardiovascular disease remains the leading cause of death worldwide, with a rising burden projected over the coming decades. Although traditional risk factors such as diabetes, hypertension, and dyslipidemia form the main targets of prevention strategies, many individuals carry a "residual" risk secondary to systemic inflammation which remains underrecognized. Landmark trials have established inflammation as a causal and modifiable driver of atherosclerotic cardiovascular disease. This review focuses on how inflammation can modulate interconnected diseases of the brain and heart. Growing evide

A Cas12a Toolbox for Rapid and Flexible Group B Streptococcus Genomic Editing and CRISPRi.

Streptococcus agalactiae (group B Streptococcus; GBS) is a leading cause of neonatal sepsis and meningitis. Despite advances in molecular microbiology, GBS genome engineering remains laborious due to inefficient mutagenesis protocols. Here, we report a versatile and rapid Cas12a-based toolkit for GBS genetic manipulation. We developed two shuttle plasmids-pGBSedit for genome editing and pGBScrispri for inducible CRISPR interference-derived from an Enterococcus faecium system and optimized for GBS. Using these tools, we achieved targeted gene insertions, markerless deletions, and efficient, tem
