Halothane-cooling contractures of mammalian muscles.

Mouse soleus and extensor digitorum longus muscles exposed to halothane concentrations (0.3-2.0%) that, at room temperature, have no effect on either resting or twitch tension, develop transient contractures when cooled to 2-10 degrees C. The halothane-cooling contractures (HCC) are reproducible and their amplitudes can be graded by both the halothane concentration and the temperature of cooling. Under identical conditions the HCC is significantly larger in soleus as compared to extensor digitorum longus muscles. The maximal HCC in soleus muscles (1-2% halothane, 2 degrees C) reaches 42% of th

A pH-dependent disordering efficacy of new local anaesthetics with carbanilate structure. A spin label study in model membranes.

Electrophysiological investigations of new drugs with carbanilate structure (heptacaine, carbisocaine) on axons and sciatic nerves of the rat have shown their high local anaesthetic effect at physiological pH. This effect is not only maintained at acid pH (6.0); it even increases on shifting pH from basic to acid values. To study this unique pH dependence of the effect of new local anaesthetics, drug-lipid interactions were investigated in liposomes prepared from egg yolk phosphatidylcholine or dimyristoyl phosphatidylcholine. The effect of the local anaesthetics on the molecular ordering of l

Perturbation effect of local anaesthetics on synaptosomes: variation with depth of the spin label probe.

Stearic acids labeled with a doxyl group at the 5th, 12th and 16th carbon positions were used as the spin probes for an investigation of the perturbation effects of two local anaesthetics (the 2-piperidinoethylester of phenyl carbamic acid and the piperidinoethylester of 2-heptyloxyphenylcarbamic acid) on rat brain synaptosomes. For a quantitation of the perturbation effects we have studied the order parameter S of the spin probe, incorporated in the membrane, calculated from the ESR spectra as a function of temperature and concentration of local anaesthetics. The local anaesthetics had a diff

The charge carried by single-channel currents of rat cultured muscle cells in the presence of local anaesthetics.

Acetylcholine-induced single-channel currents were measured in the presence of the lignocaine derivative QX222. Unit responses appeared as bursts of short current pulses as a result of the fast blocking action of the drug (QX222). The amplitude of the individual current pulses was not changed by the presence of the drug up to a concentration of 250 microM. The time integral of current during a burst, which for a sequential blocking model should be independent of drug concentration, decreased at concentrations of QX222 higher than 40 microM. The distribution of gap times within a burst could no

[Use of a ternary mixture of local anaesthetics for peridural anaesthesia during urgent caesarean operation. Report on 125 cases].

125 caesarean operations are conducted under peridural anaesthesia with a mixture of local anaesthetics containing per milliliter: 2.5 mg of bupivacaine, 2.5 mg of etidocaine, 12.5 mg of lidocaine, and 5.10(-6) epinephrine. The dose injected is 23.37 +/- 1.98 ml, the volume necessary for blocking a metamer being 1.18 +/- 0.10 ml/segment. Satisfactory analgesia is obtained in 119 cases (95.2 p. cent), with adequate muscle relaxation in all cases. Block anaesthesia occurs after 5.81 +/- 0.67 min, last 197 +/- 62 min, and enable incision after 8.80 +/- 2.55 min with extraction of the infant after

The interaction of amine local anaesthetics with muscarinic receptors.

1 Amine local anaesthetics inhibited the binding of (-)-[3H]-quinuclidinyl benzilate ((-)-[3H]-QNB) to muscarinic receptors in crude synaptosomal preparations from guinea-pig brain. The order of potency was SKF 525A greater than tetracaine greater than procaine congruent to quinidine greater than procainamide greater than bupivacaine greater than lignocaine greater than prilocaine. 2 The concentration of tetracaine or prilocaine causing 50% inhibition of the receptor-specific binding of [3H]-QNB varied linearly with the concentration of [3H]-QNB present for the range of concentrations of prilo

Lumbar facet joint injection in low back pain and sciatica: description of technique.

Lumbar facet arthropathy is an important cause of low back pain and sciatica. Intra-articular facet block can confirm symptomatic facet joint disease. Good understanding of the anatomy of the joint and fluoroscopic control are essential for intra-articular placement of anesthetics and steroids.

[The influence of the continuous epidural anaesthesia on uteroplacental blood flow (author's transl)].

Continuous lumbar epidural anaesthesia causes a drop in maternal blood pressure by decreasing flow resistance in the peripheral vascular bed. By infusion of 1000 ml NaCl blood pressure can be kept constant. Our work shows, that dilatation of the peripheral vessels has a positive effect in patients with reduced uteroplacental flow, as it results in an augmentation of placental flow. In women with normal placental function such an effect could not be seen. The practical use of these results is illustraded by a case report.

Ropivacaine 0.25% compared with bupivacaine 0.25% by the caudal route.

We compared in a randomized double-blind study, the postoperative analgesia and degree of motor block produced by the new local anaesthetic ropivacaine, with bupivacaine, for caudal anaesthesia in children. Eighty children, 2-5-years-old, ASA I, received one of two local anaesthetics; either ropivacaine 0.25% (1.0 ml x kg(-1)) or bupivacaine 0.25% (1.0 ml x kg(-1)). They were sedated with a continuous infusion of propofol (200 microg x kg(-1) min(-1). The lungs were ventilated with a mixture of 50% nitrous oxide with oxygen. 60 min after local anaesthetic injection, and every 60 min, the exten

Pain Scores Associated with Lidocaine-Tetracaine Versus Lidocaine-Prilocaine Topical Anesthesia During Facial and Cervical Microneedling: A Retrospective Single-Center Observational Analysis.

Background: Effective topical anesthesia is essential to patient comfort and adherence during minimally invasive esthetic procedures. We retrospectively reviewed pain scores recorded after microneedling in a single private clinic where two topical anesthetic formulations-lidocaine 7%/tetracaine 7% (Pliaglis) and lidocaine 2.5%/prilocaine 2.5% (Anesderm)-were used as part of standard clinical practice on different anatomical sites and under different application protocols. Methods: Records were reviewed from 26 healthy female patients (mean age 42 ± 4 years; range 34-48) who underwent microneed

Carpal tunnel release under local anesthesia with or without distal median nerve block: Double-blind randomized clinical trial.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper extremity, and its surgical treatment is carpal tunnel release (CTR). It is mostly performed in local anesthesia. There are no clinical randomized controlled trials (RCTs) comparing local infiltration anesthesia with or without a distal median nerve block in CTR. The aim of the PERSONNEL trial (carPal tunnEl ReleaSe lOcal iNfiltratioN mEdian bLockade) is to assess whether a distal median nerve block reduces pain during and after the procedure in addition to local anesthesia. METHODS: This is a singl

Use of Topical Phenol in Awake Young Children for Tympanostomy Tube Placement.

Importance: Phenol kits cleared by the Food and Drug Administration (FDA) are indicated as a topical anesthetic for the tympanic membrane (TM) in adults. However, there is no existing literature that reports outcomes to support the safety and use of phenol on the TM of awake children. Objective: Determine if topical phenol is safe and at low risk for complications and therefore be used effectively in awake children to facilitate office otologic procedures as in adults. Design, Setting, and Participants: Children under 21 years of age whose parents agreed to participate in an awake office setti

Patient satisfaction with tourniquet application and local anaesthesia infiltration in carpal tunnel decompression and the relationship with overall satisfaction.

This was a prospective study designed to determine what effect poor tolerance to tourniquet application and local anaesthesia infiltration in open carpal tunnel decompression has on overall patient satisfaction with the surgical procedure. Fifty patients were recruited into the study. Visual analogue scores (VAS) were recorded postoperatively for pain related to tourniquet application, local anaesthesia infiltration and the surgical procedure overall. In terms of the procedure, poor tolerance to the tourniquet and local anaesthetic showed no significant relationship to the overall patient sati
