Estrogens and the pathophysiology of the biliary tree.

The scientific framework concerning estrogen effects on different tissues has expanded enormously during the last decades, when estrogen receptor (ER) subtypes were identified. Estrogens are not only essential for the female reproductive system, but they also control fundamental functions in other tissues including the cardiovascular system, bone, brain and liver. Recently, estrogens have been shown to target the biliary tree, where they modulate the proliferative and secretory activities of cholangiocytes, the epithelial cells lining bile ducts. By acting on both estrogen receptors (ER-alpha)

Clinical utility of treatment method conversion during single-session endoscopic ultrasound-guided biliary drainage.

BACKGROUND: Although several techniques for endoscopic ultrasound-guided biliary drainage (EUS-BD) are available at present, an optimal treatment algorithm of EUS-BD has not yet been established. AIM: To evaluate the clinical utility of treatment method conversion during single endoscopic sessions for difficult cases in initially planned EUS-BD. METHODS: This was a single-center retrospective analysis using a prospectively accumulated database. Patients with biliary obstruction undergoing EUS-BD between May 2008 and April 2016 were included. The primary outcome was to evaluate the improvemen

Technical feasibility should not outweigh long-term outcomes in biliary reconstruction for children.

The choice of biliary reconstruction after excision of choledochal malformations in children should prioritize long-term physiological outcomes over operative convenience. In response to the article by Kanojia et al. on the management of aberrant bile ducts at the porta hepatis during minimally invasive surgery, we wish to highlight that while hepaticoduodenostomy is technically attractive (particularly with laparoscopic and robotic platforms) evidence suggests it is associated with increased biliary reflux and reflux-related morbidity, especially in cases with complex hilar anatomy requiring

Integrating systemic therapy in the earlier stages of liver cancer.

The treatment of advanced primary liver cancer has seen major improvement in recent years. In hepatocellular carcinoma (HCC), immunotherapy has improved both survival and quality of life. In biliary tract cancers (BTC), immunotherapy has shown modest benefits, while molecular targeted therapies have had a major impact in selected populations defined by molecular alterations. These advances are now being tested at earlier disease stages. In this review, we first discuss the challenges in designing trials that combine local and systemic treatment at earlier stages, and the different endpoints th…

Nutritional Management of Hepatobiliary Diseases in Dogs and Cats.

Dogs and cats with liver disease often benefit from nutritional modifications. The most important consideration for patients with severe disease is often providing sufficient energy intake to prevent a catabolic state. Additionally, targeted nutritional modifications address specific clinical signs and liver disease pathogenesis. For instance, patients with hepatic encephalopathy often benefit from a diet with a reduced amount of protein, or a vegetarian diet as these modifications can help reduce the severity and frequency of clinical signs. This article aims to guide the clinician in underst

Risk factors for surgical site infections following hepatobiliary surgery: An umbrella review and meta-analyses.

BACKGROUND: In the hepatobiliary (HPB) surgical cohort, surgical site infections (SSI) can extend hospital stays, result in higher morbidity, and poor patient outcomes. This umbrella review and meta-analysis aimed to synthesise the evidence for the association between clinical and patient risk factors and SSI in patients following HPB surgery. METHODS: We searched MEDLINE, CINAHL, EMBASE and Scopus from January 2000 to April 2023 to identify systematic reviews and meta-analyses where patient and/or clinical factors of SSIs following HPB surgery were reported. The summary effect size, its 95 %

[A fatal case intra-abdominal hemorrhage following self-removal of a percutaneous transhepatic gallbladder drainage tube].

A 92-year-old woman with gallstone pancreatitis and acute cholangitis was admitted to our hospital where endoscopic retrograde cholangiopancreatography was performed for emergency biliary drainage. Biliary cannulation was unsuccessful. Consequently, percutaneous transhepatic gallbladder drainage (PTGBD) was performed, and her symptoms improved. The PTGBD tube was removed by the patient on the third day of admission resulting in cardiopulmonary arrest two hours later. Cardiopulmonary resuscitation restored spontaneous circulation. Contrast computed tomography revealed intra-abdominal hemorrhage

Short Term Outcome of Therapeutic ERCP in the Management of Biliary Ascariasis in a Tertiary Care Hospital of Bangladesh.

The history of ascariasis is very old. It is endemic in various parts of Bangladesh. Hepatobiliary ascariasis is a well-known complication of the intestinal ascariasis. Although cosmopolitan, it predominantly affects people in Asia, Africa, and South America. This was a prospective observational study conducted from July to December 2014 to explore the short term outcome of therapeutic ERCP in the treatment of biliary ascariasis in a tertiary care hospital of Bangladesh. A total of 60 cases of biliary ascariasis were selected conveniently in this study. Data were collected by face-to-face inte

Levels of Circulating PD-L1 Are Decreased in Patients with Resectable Cholangiocarcinoma.

UNLABELLED: Tumor resection represents the only curative treatment option for patients with biliary tract cancers (BTCs), including intrahepatic cholangiocarcinoma (CCA), perihilar and extrahepatic CCA and gallbladder cancer. However, many patients develop early tumor recurrence and are unlikely to benefit from surgery. Therefore, markers to identify ideal surgical candidates are urgently needed. Circulating programmed cell death 1 ligand 1 (PD-L1) has recently been associated with different malignancies, including pancreatic cancer which closely resembles BTC in terms of patients' prognosis a…

Comparison of 2K and 4K imaging systems for laparoscopic repair of choledochal cyst in children.

AIM: The safety and success of laparoscopic choledochal cyst surgery (LapCC) depends upon two critical elements during the hepaticojejunostomy anastomosis (HJA). These may be termed the Difficulty of Differentiation (DOD) and the Difficulty of Suturing (DOS). The type of imaging system (2 or 4 K) used may influence either of these. We compared outcomes of LapCC using 2 or 4 K imaging systems. METHODS: LapCC were performed at a single institution by the same team using a 2 K system (2009-2018; n = 26) and a 4 K system (2018-2019; n = 11) were compared. 4 K cases were chosen to match 2 K cases
