Although group B Streptococcus (GBS) is a leading cause of severe invasive disease in young infants worldwide, epidemiologic data and knowledge about risk factors for the disease are lacking from low- to middle-income countries. To determine the epidemiology of invasive GBS disease among young infants in a setting with high maternal HIV infection, we conducted hospital-based surveillance during 2004-2008 in Soweto, South Africa. Overall GBS incidence was 2.72 cases/1,000 live births (1.50 and 1.22, respectively, among infants with early-onset disease [EOD] and late-onset [LOD] disease). Risk f

BACKGROUND: After a child's death, parents may experience depression, posttraumatic stress disorder (PTSD), and increased risk for cancers, diabetes, psychiatric hospitalization, and suicide. Racial/ethnic differences are unknown. This longitudinal study investigated health and functioning of Hispanic, black, and white parents through 13 months after NICU/PICU death.
METHODS: Parents (176 mothers, 73 fathers) of 188 deceased infants/children were recruited from 4 NICUs, 4 PICUs, and state death certificates 2 to 3 weeks after death. Deaths occurred after limiting treatment/withdrawing life su

OBJECTIVE: To study the changes in standardised in-hospital mortality in the period 2005 to 2010.
DESIGN: Retrospective, descriptive study.
METHOD: Data from the National Medical Registration (LMR) for the period 2005 to 2010 were used to calculate a time-series of the standardised in-hospital mortality rate. Predicted mortality was calculated for 50 principle diagnosis groups using logistic regression models with 8 explanatory variables. This was used, in combination with actual in-hospital mortality, to calculate the Time-Series Standardised Hospital Mortality Ratio (TSHMR).
RESULTS: The

BACKGROUND: Despite ongoing efforts to improve the quality of pediatric resuscitation, it remains unknown whether survival in children with in-hospital cardiac arrest has improved.
METHODS AND RESULTS: Between 2000 and 2009, we identified children (<18 years of age) with an in-hospital cardiac arrest at hospitals with >3 years of participation and >5 cases annually within the national Get With The Guidelines-Resuscitation registry. Multivariable logistic regression was used to examine temporal trends in survival to discharge. We also explored whether trends in survival were attributable to im

BACKGROUND: The reported incidence rates of vestibular schwannomas (VS) vary substantially, but it is unclear as to what extent the variation reflects differences in risk or recording practices. Our aim was to describe the incidence rates of VS in Denmark, Finland, Norway and Sweden between 1987 and 2007.
METHODS: Comprehensive data were available from all registries only for the period from 1987 to 2007. An analysis of a longer time period (1965-2007) was conducted with the Norwegian and Swedish data.
RESULTS: The average age-standardised incidence rates during 1987-2007 varied from 6.1 per

OBJECTIVE: To estimate costs of hospitalization and years of potential life lost associated with leptospirosis.
METHODS: Databases of the Brazilian Ministry of Health's information system were used to carry out probabilistic linkage of cases and hospitalizations leading to death by leptospirosis in 2007. Within the Information System for Notifiable Diseases, confirmed cases were subdivided into hospitalization and death. These were then linked to the Hospital Information System (records with primary diagnosis) and the Mortality Information System (underlying cause of death A27.0, A27.8, and A

In contrast to bacteraemic pneumococcal community-acquired pneumonia (CAP), there is a paucity of data on the clinical characteristics and outcomes of non-bacteraemic pneumococcal CAP. This retrospective study compared the outcome of hospitalized patients with bacteraemic and non-bacteraemic pneumococcal CAP treated at a medical centre from 2004 to 2008. Data on clinical outcomes including all-cause mortality, length of hospital stay, need for intensive-care unit admission and extrapulmonary involvement were analysed. In all, 221 patients with pneumococcal pneumonia (87 bacteraemic, 134 non-ba

BACKGROUND: Encephalopathy is a serious neurologic complication from shigellosis often with fatal outcome but data on simple clinical and sociodemographic predicting factors are limited. We sought to identify those factors in children.
METHODS: In this unmatched case-control design, children of both sexes, aged 0 to 15 years having positive stool culture for Shigella from 1997 to 2006 were studied. Children with Shigella encephalopathy constituted the cases (n = 29), whereas randomly selected children with shigellosis having no encephalopathy constituted controls (n = 87).
RESULTS: The case-

BACKGROUND: Advances in the treatment of childhood hematologic malignancies have led to improvements in survival for several of these conditions during the past few decades, but most population-based survival data available to date refer only to patients diagnosed up to the mid-1990s.
METHODS: We used period analysis to assess trends in 5- and 10-year survival in US patients younger than 15 years of age at diagnosis with four hematologic malignancies--acute lymphoblastic leukemia, acute non-lymphoblastic leukemia, Hodgkin lymphoma, and non-Hodgkin lymphoma--over three recent 5-year intervals,

BACKGROUND: The chest cage is a common target for traumatic damage. Although relatively rare, it is considered to be a serious condition with significant reported mortalities. As most flail injuries are accompanied by severe extrathoracic injuries, it is often difficult to pinpoint a single injury responsible for the patient's death.
AIM: To investigate the factors related to mortality when flail injury is diagnosed.
METHODS: Data from the Israel National Trauma Registry between 1998 and 2003 included 11,966 chest injuries (262 flail chest injuries) out of a total of 118,211 trauma hospitali

UNLABELLED: The association between short term changes in ambient air concentrations of TSP and SO2 and day to day fluctuations in mortality was analyzed in the highly polluted Northern Bohemia region inhabited by approximately 630,000 people. A logistic regression model was adjusted for long term trends, seasonal cycles, influenza epidemics and weather parameters. The pollution and mortality data were available from 1982 to 1994. When the association was evaluated regardless of gender and age, 100 micrograms/m3 increase of TSP, but not SO2, was associated with a 3.4% increase of daily mortali

This retrospective study of 645 black patients, carried out over a five year period, showed that appendicitis is twice as common in males as in females and that it occurs predominantly in young people (median age 20 years). The classical presentation of periumbilical pain (16%) was outnumbered by right iliac fossa pain (36%) and non-specific pain (27%). The majority perforated (43%) and appendiceal inflammation was second commonest (37%). The negative appendicectomy rate was 8.8% and there was a diagnostic error of 14%. Mortality was two per cent mainly from patients complicated by peritonitis

To investigate acute, irreversible effects of exposure to ozone and other air pollutants, the authors examined daily death counts in relation to air pollution levels in Mexico City during 1990-1992. When considered singly in Poisson regression models accounting for periodic effects, the rate ratio for total mortality associated with a 100-ppb increment in 1-hour maximum ozone concentration was 1.024 (95% confidence interval (CI) 1.011-1.039). Measures of average ozone concentration were somewhat more strongly related to mortality. The rate ratio was 1.024 (95% CI 0.984-1.062) per 100 ppb for s

The population-based epidemiological indices (crude incidence, survival rate, mortality, etc.) of childhood leukemia (0-14 years of age) from 1969 to 1993 in Hokkaido Prefecture, Japan, were calculated, using data obtained from the Registry of Childhood Malignancies in Hokkaido Prefecture. A total of 1,084 cases of leukemia were diagnosed in the 1969-93 period. The annual incidence of all types of leukemia from 1984 to 1993 was about 4 per 100,000 children aged 0-14 years, with the incidence of ANLL decreasing slightly and that of ALL increasing. The ratio of ALL/ANLL could similarly be seen t

A retrospective study of the pattern of paediatric deaths at the children's emergency room (CHER) of the University College Hospital, Ibadan revealed a steady decline from 208 per 1000 admissions in 1978 to 179 per 1000 admissions in 1986. The study, however, showed that one in every five children admitted to the CHER died over the study period 1978, and 1981-1986. Measles has the highest proportionate death rate of 13.1% and it shares the highest fatality rate of 32.6% with tetanus. Gastroenteritis and bronchopneumonia, which were major causes of deaths in the CHER in the early 1970s decline
