OBJECTIVE: We define criteria for insertion of cervical cerclage done electively (historical), urgently (in patients without symptoms), or emergently (in patients with symptoms). We compare outcomes as determined by prolongation of pregnancy and survival in each of these groups, and we define whether urgent or emergent cerclage imparted a higher risk of spontaneous rupture of membranes or chorioamnionitis than that associated with elective cerclage.
STUDY DESIGN: This is a retrospective analysis of all cervical cerclages placed at Memorial Medical Center between January 1, 1993, and December

Antibiotic resistance among bacteria that are commonly encountered in the pediatric emergency department is a fact of nature. New antibiotics will provide some help, but probably only temporarily. Vaccine strategies seem to provide the best answer to resistance, and many physicians eagerly await the conjugated pneumococcal vaccines, which we can only hope to be as successful as the H. influenzae type b vaccines. Vaccines against other resistant organisms are likely further off. At this point, a major goal must be to limit the prevalence of antibiotic resistance. In considering this goal, two c

Third-party payers typically use patients' discharge diagnoses to determine "appropriate" Emergency Department (ED) usage. This analysis compared the resource intensity involved in ED evaluation for "inappropriate" and all other ED visits. In this retrospective database review, 11 discharge diagnoses (DX11) (chronic nasopharyngitis; chronic sinusitis; chronic pharyngitis; rhinitis; constipation; head cold; hemorrhoids; toothache; flu; headache; and tension headache) were identified by a third party payor as being "inappropriate" for ED evaluation. The chief complaints of all patients seen in 1

Statistical data are detailed concerning compositional, therapeutic and diagnostic aspects of urgent diseases of the ear (inflammation and traumas of the external ear, acute and aggravated chronic otitis media, acute neurosensory hypoacusis, etc.). 580 patients were urgently hospitalized to ENT clinic (Moscow General Hospital N 1) at the Russian State Medical University in 1995 for urgent diseases of the external, middle and internal ear. This makes up 28% of a total of hospitalized ENT patients. The most effective policy of treatment for each ear disease form is analyzed.

Therapeutic measures, with intravenous chlorpromazine, taken during acute headache are evaluated in fourteen patients at the emergency room in Santa Casa de São Paulo. Four patients had the diagnosis of migraine with aura and five patients migraine without aura. Four patients had diagnosis of chronic daily headache with intermittent and superimposed migrainous events. Finally one patient had the diagnosis of chronic paroxysmal hemicrania. All patients were diagnosed according to International Headache Society criteria. The intravenous chlorpromazine dose used was 0.7 mg/Kg diluted in 5% glucos

Le Fort fractures are a part of the facial fracture spectrum, sustained from significant forces of impact to the midface. The mechanism of airway obstruction in Le Fort fractures is attributed to midface instability with posterior inferior displacement into the oropharyngeal airway. Otolaryngologists often participate in the evaluation and management of such patients, securing the airway, if necessary. It is important, therefore, to understand the mechanisms responsible for acute airway obstruction in these types of fractures. A retrospective review of 64 cases of Le Fort fractures, representi

OBJECTIVES: To study the incidence and nature of injuries sustained by emergency medicine (EM) residents during EMS rotations, and steps taken at EM residency programs to increase resident safety during field activities.
METHODS: An eight-question survey form was mailed to all 114 U.S. EM residency directors, with a second mailing to nonresponders eight weeks after the initial mailing.
RESULTS: A total of 105 surveys were returned (92%). Six surveys were from new programs whose residents have not yet rotated on EMS. These were excluded from further analysis, leaving 99 programs. Of these, 91

This report describes three histologically documented cases of acute eosinophilic myocarditis. These three cases illustrate the different clinical and therapeutic outcomes of this disease which can range from full recovery under prolonged corticosteroid treatment to requirement for emergency heart transplantation or death due to intractable cardiac insufficiency. In absence of specific clinical or laboratory data, diagnosis must be established in vivo by endomyocardial biopsy demonstrating eosinophil-rich inflammatory infiltration and necrotic lesions. Rapid decision-making is necessary to all

Rapid evaluation of the hypotensive patient in the emergency department is essential. The availability of ultrasonography in the ED, performed by emergency physicians and surgeons, has made it easier to evaluate the hypotensive trauma patient. We describe a 44-year-old man transferred to our institution from a community hospital for evaluation of syncope and hypotension with no obvious cause. On arrival the patient began to complain of slight lower abdominal pain. The patient's physical examination revealed minimal abdominal tenderness. A rapid ultrasound examination performed at bedside revea

An outbreak of yellow fever (YF) occurred in the central part of Senegal during October 1995. Thirty-one probable cases were detected and 79 cases were confirmed either by IgM ELISA or by virus isolation (30 strains isolated). The case fatality rate was 18.9%. Incidence of the infection was evaluated by a serosurvey in the area. Males 10-29 years old belonging to the Peul ethnic group were more affected. Moreover, 28 YF virus strains were isolated from mosquitoes and larvae pools and vertical transmission of YF virus by Aedes aegypti was also demonstrated for the first time in the field. This

During a 6-month period in 1994-1995, 0.7% of patient visits (51 visits of 47 patients) at the emergency unit of the Department of Medicine, Helsinki University Central Hospital, were due to joint symptoms. Infection seemed to play an important role as the etiologic factor for the joint complaints. Acute joint exacerbation in pre-existing arthritis was the reason for 11.7%, and suspected Baker's cyst for 3.9% of the visits. Confirmed (12.8%) or suspected reactive arthritis (10.6%), septic arthritis/spondylitis (6.4%), arthritis with inflammatory bowel disease (6.4%), and gout (14.9%) were the

We studied the knowledge and use of hormonal emergency contraception (EC) in Finland by mailing a questionnaire to a national sample of 3000 women aged 18-44 years (response rate 74%). Ten percent of the women aged under 25 and 4% of all respondents had sometimes used EC. Unmarried women were more likely to report having used hormonal EC than were married women, and nulliparous women reported more use than did parous women. However, no statistically significant difference in EC use among women with or without previous abortion history was observed. Older women were less aware of EC than of oth

OBJECTIVE: To determine which characteristics of older patients who use a hospital ED are associated with repeat visits during the 90 days following the index visit.
METHODS: The study was conducted in the ED of a 400-bed university-affiliated acute care community hospital in Montreal. Patients aged > or = 75 years who visited the ED between 08:00 and and 16:00 on a convenience sample of days over an 8-week period (July and August 1994) were assessed using a questionnaire, physical and cognitive status instruments, and a functional problem checklist. The hospital's administrative database was

The authors report 6 cases of life threatening hemorrhage which occurred in 1985-1995. The cases were characterized by massive recurrent bleeding resistant to standard conservative methods of stopping. In a posttonsillectomy patient bleeding from the tonsillar niche and in two patients nasal bleeding because of trauma or tumor were stopped only after ligation of the external carotid artery. Arrosive bleeding from the internal jugular vein in a patient after opening of a deep tonsillogenic neck flegmon was stopped by ligation of the jugular vein along the length of the wound. Bleeding after ope
