Implanting straight into cochlea risks the facial nerve: a Cartesian coordinate study.

PURPOSE: To describe the straight-into-cochlea line that affords the best access for an electrode array to enter via the round window, and how this line relates to the facial nerve, the incus, and mastoid size. The straight-into-cochlea line is important to minimize the cochlear trauma and maximize the likelihood of placement into the scala tympani. METHODS: High-resolution CT scans were obtained for ten craniums with the extremes of large (N = 5) and small (N = 5) mastoid pneumatization; the specimens were from a series of 41 ear normal craniums. Using FIJI, a publicly available software pro

Connexin 43 contributes to ectopic orofacial pain following inferior alveolar nerve injury.

BACKGROUND: Clinically, it is well known that injury of mandibular nerve fiber induces persistent ectopic pain which can spread to a wide area of the orofacial region innervated by the uninjured trigeminal nerve branches. However, the exact mechanism of such persistent ectopic orofacial pain is not still known. The present study was undertaken to determine the role of connexin 43 in the trigeminal ganglion on mechanical hypersensitivity in rat whisker pad skin induced by inferior alveolar nerve injury. Here, we examined changes in orofacial mechanical sensitivity following inferior alveolar ne

Pre-expanded, Prefabricated Monoblock Perforator Flap for Total Facial Resurfacing.

In this article, we present the pre-expanded, prefabricated supercharged cervicothoracic monoblock perforator flap for total or subtotal facial resurfacing. This technique can be a reliable reconstruction option for extensive facial skin defect with undamaged muscles and deep structures, which could provide excellent aesthetic and functional outcomes with acceptable complications. Our approach may replace a conventional "skin-only" face allotransplantation in selected patients.

Changes in transient receptor potential channels in the rat geniculate ganglion after chorda tympani nerve injury.

We reported differential expression of the transient receptor potential vanilloid 1 (TRPV1), the transient receptor potential ankyrin 1 (TRPA1), and the (TRPM8) in the geniculate ganglions (GGs) of naive rats. In medical practice, the chorda tympani nerve (CTN) is injured in some patients during middle-ear surgery, and results in tongue numbness and taste disorder. We investigated changes in the expression of these receptors in GGs after CTN injury. In naive-rat GGs, 11.4, 11.8, and 0.5% of neurons were found to express the TRPV1, the TRPA1, the TRPM8, respectively. At 3 days after CTN injury,

Use of intraoperative computed tomography for maxillofacial reconstructive surgery.

IMPORTANCE: Intraoperative computed tomography (CT) provides surgeons with real-time feedback during maxillofacial trauma and reconstructive surgery, which can affect intraoperative decision making. OBJECTIVES: To evaluate the time needed to perform intraoperative CT scans during maxillofacial surgery, determine any trend toward shorter total scan times as experience is gained with the technique, and identify the characteristics of cases that required intraoperative revision based on the results of intraoperative CT scanning. DESIGN, SETTING, AND PARTICIPANTS: A retrospective review was comp

Optimal care for eyelid contraction after radiotherapy: case report and literature review.

Radiotherapy represents a major problem in facial surgery. Orbital and periorbital radiation therapy causes a contraction of the soft tissues. Scarring with ectropion is the most severe complication, with shrinking of the anterior lamella, skin dystrophy, muscle atrophy, and alteration of the remaining soft tissues. Goals for reconstruction include correction of distorted orbitofacial tissues and the restoration of orbital structures. The management of these patients is not standardized. We suggest systematically using a combined approach of surgery and lipofilling to restore the orbital defor

Dacryo salivary fistula. Unusual case of salivary fistula draining into the inferior fornix of the eye.

Salivary fistula after a traumatic injury in the buccal region is not common. It is most commonly encountered in injuries involving the pre auricular region or soft tissues along the course of the parotid duct. It is more frequent in the parotid because its anatomical location is superficial; traumatic salivary fistulae of the other two major salivary glands are rare. This report presents an unusual case of a salivary fistula that occurred following trauma to the infraorbital region and drained into the inferior fornix of the left eye. A review of cases reported in the literature is presented.

An algorithmic approach for reconstruction of burn alopecia.

Although various methods for reconstruction of burn alopecia have been described in case reports and small case series, a comprehensive comparison of these methods has not been performed. The purpose of this study was to assess the appropriateness of the clinical indications for the various reconstructive methods for burn alopecia and suggest an algorithm for individualized reconstruction. A review of 83 patients who underwent reconstruction for burn alopecia between 1995 and 2007 was conducted. Demographics, associated injuries, preoperative findings, surgical techniques, and postoperative co

A review of intraoperative airway management for midface facial bone fracture patients.

In craniofacial trauma patients, oral route endotracheal intubation may thwart the accuracy of dental occlusion and nasotracheal intubation carries the risk of intracranial invasion in skull base fracture cases. Between November 2005 and June 2006, patients receiving facial bone fracture operations at Kaohsiung Medical University Hospital were enrolled in this study. Intraoperatively, the endotracheal tube was pushed to either the retromolar space or the missing tooth space and secured by two 4.0 silk stitches. Then, surgeons could perform the usual procedure to explore the fracture sites, che

Intraoral approach for the treatment of submandibular salivary gland mixed tumors.

Pleomorphic adenoma represents a most common benign neoplasm of major salivary glands. Most occurrences of this benign tumor in the submandibular gland have been treated surgically without difficulty via transcervical approach. However, a few clinical problems in the transcervical approach have been mentioned, such as nerve injury or aesthetic scaring. In this study, we introduced the intraoral approach for the removal of the submandibular mixed tumor. In total, 12 cases of pleomorphic adenoma of the submandibular gland were treated via intraoral approach. The surgical technique and morbidity
