Plant-Based Diet Mitigated the Risk of Chronic Kidney Disease in Overweight Individuals.

INTRODUCTION: Being overweight, with a body mass index (BMI) ≥25, is associated with multiple comorbidities, including chronic kidney disease (CKD). A plant-based diet, such as a vegetarian or lacto-ovo vegetarian diet, serves as a preventive strategy for CKD in patients. We hypothesized that a vegetarian diet lowers the occurrence of CKD in overweight subjects. METHODS: The retrospective study was performed in Taipei Tzu Chi Hospital by a self-paid health exam. Age younger than 40 years old and the subjects without sufficient biochemical or hematologic results were excluded. The incidence of

A Clinical Practice-Based Comparison of Conventional and Individualized Dosing Strategies for Therapeutic Enoxaparin.

To understand differences in anti-factor-Xa levels produced by two different dosing strategies (conventional and individualized) for therapeutic enoxaparin in a cohort of hospital inpatients. A multicenter, retrospective cohort study over a two- and a half-year period for inpatients with stable renal function and on therapeutic enoxaparin. Anti-factor-Xa levels were taken 3-5 h after enoxaparin administration and a minimum of 48 h of dosing. The final analysis included 278 patients from five hospitals: conventional dosing was used for 141, while 137 were given an unconventional dose, that is,

Precision medicine in prostate cancer: individualized treatment through radiomics, genomics, and biomarkers.

Prostate cancer (PCa) is one of the most common malignancies threatening men's health globally. A comprehensive and integrated approach is essential for its early screening, diagnosis, risk stratification, treatment guidance, and efficacy assessment. Radiomics, leveraging multi-parametric magnetic resonance imaging (mpMRI) and positron emission tomography/computed tomography (PET/CT), has demonstrated significant clinical value in the non-invasive diagnosis, aggressiveness assessment, and prognosis prediction of PCa, with substantial potential when combined with artificial intelligence. In gen

Optimising test intervals for individuals with type 2 diabetes: A machine learning approach.

BACKGROUND: Chronic disease monitoring programs often adopt a one-size-fits-all approach that does not consider variation in need, potentially leading to excessive or insufficient support for patients at different risk levels. Machine learning (ML) developments offer new opportunities for personalised medicine in clinical practice. OBJECTIVE: To demonstrate the potential of ML to guide resource allocation and tailored disease management, this study aims to predict the optimal testing interval for monitoring blood glucose (HbA1c) for patients with Type 2 Diabetes (T2D). We examine fairness acr

Risk Factors and Consequences of Cutaneous Allodynia among Individuals with Migraine: A Scoping Review.

PURPOSE OF REVIEW: Cutaneous allodynia is a frequent complaint among individuals with migraine and serves as a critical marker of central sensitization, which plays a pivotal role in migraine pathophysiology. This can exacerbate migraine severity and diminish quality of life. Understanding the risk factors associated with cutaneous allodynia is essential, as it can help identify individuals at higher risk of developing this condition and guide early intervention strategies. Identifying its consequences provides critical insights into disease progression, treatment response, and disability. Hen

Natriuretic Peptide Cut Points for Heart Failure Classification in Individuals With and Without Obesity.

BACKGROUND: The 2022 American Heart Association/American College of Cardiology/Heart Failure Society of America guidelines introduced elevated natriuretic peptide (NP) levels as a criterion for defining stage B heart failure (HF), or pre-HF, to identify individuals at greatest risk for future HF. Given the known NP deficiency in obesity, we aimed to assess whether a single NP cut point would disproportionately up-classify individuals with versus without obesity to stage B HF. METHODS: Participants free of HF from 5 community-based cohorts were included. We examined the reclassification of ind

Genetic Variation in the Alternative Complement Pathway Contributes to Individual Susceptibility to Bacteremia and Sepsis.

IMPORTANCE: The alternative complement pathway is a key component of host defense against bacteremia and other infections. However, dysregulated activation can contribute to excessive inflammation and worse clinical outcomes during bacteremia and infectious syndromes such as sepsis. OBJECTIVES: We aim to identify variants in alternative pathway (AP) genes that influence the risk for bacteremia and sepsis. DESIGN, SETTING, AND PARTICIPANTS: We used summary statistics from a Veterans Affairs Million Veteran Program (MVP) genome-wide by phenome-wide association study of more than 600,000 indivi

Intersectionality of individual and neighborhood-level adverse social determinants of health in early pregnancy.

INTRODUCTION: Individual- and neighborhood-level social determinants of health (SDOH) have been assessed separately in pregnancy, but their relationship to one another remains uncertain. We investigated the intersectionality of three neighborhood-level SDOH measures with three individual-level SDOH measures. This was done to examine the concomitant experiences of multiple SDOH in pregnancy. METHODS: A secondary analysis of data from the Nulliparous Pregnancy Outcomes Study: Monitoring Mothers-To-Be. We assessed three neighborhood-level SDOH measures using geocoded participant home addresses i

Hippocampal network connectivity and episodic memory in individuals aging with traumatic brain injury.

Aging is associated with marked declines in episodic memory corresponding with decreased volume in studies of morphology and reduced network response in studies of functional connectomics. Furthermore, recent research has demonstrated that reductions in resting state network connectivity are related to declines in episodic memory, specifically in the default mode and frontoparietal cortical networks. Additionally, the interactive effects of aging and traumatic brain injury (TBI) are associated with increased risk for neurodegeneration and episodic memory impairments. However, there is a gap in

Glycemic control and prostate antigen levels in individuals with diabetes based on NHANES data.

The relationship between diabetes and prostate-specific antigen (PSA) levels is complex, with potential implications for prostate cancer screening. This study examined the association between glycemic control and total PSA (tPSA) levels in patients with diabetes. We analyzed data from the 2001-2010 NHANES to assess the relationship between glycated hemoglobin (HbA1c) and tPSA in adults with diabetes, categorizing HbA1c as < 7% (good glycemic control) or ≥ 7% (poor glycemic control). Multivariable regression models were used, adjusting for key demographic, clinical, and lifestyle factors, inclu

Probing photoreactions of individual suspended carbonaceous aerosols by multi-wavelength OT-SERS.

This work reports the first application of multi-wavelength optical trapping surface-enhanced Raman spectroscopy (OT-SERS) for in situ probing of photochemical reactions on individual suspended aerosol particles. A composite aerosol model (4-MBA/silver nanoparticles/activated carbon) with a size ranging from 2 to 10 μm, comparable to the particle sizes of PM2.5 and PM10, was designed to validate the ability of OT-SERS to probe interfacial photoreactions on aerosols. By integrating non-contact optical trapping with SERS detection, we directly monitored the interfacial photoreactions occurring o

Functional analysis of SRY variants in individuals with 46,XY differences of sex development.

In mammals, male sexual development is initiated by the expression of the Sex-determining-Region-Y (SRY) gene. SRY contains a highly conserved high mobility group (HMG) box essential for DNA binding and activity. Variants in SRY cause Differences of Sex Development (DSD), accounting for 10-15% of 46, XY gonadal dysgenesis cases. Here, we present the functional analysis of five SRY coding variants identified in patients with 46, XY DSD. Four variants (p.Asp58Glu, p.Arg75Lys, p.Met85Thr, and p.Arg86Ter) are located within the HMG box and one variant (p.Tyr198Cysfs∗18) located in the C-terminal d

Pharmacokinetic-Driven Individualized Anticoagulation Plan for Pediatric Athletes on Direct Oral Anticoagulants.

Children on anticoagulation have traditionally been discouraged from participating in contact and collision sports due to a perceived bleeding risk. The predictable pharmacokinetics of direct oral anticoagulants permit more individualized management. At Boston Children's Hospital, pharmacokinetic (PK) data were used to develop individualized anticoagulation plans for five athletes on anticoagulation who wished to participate in contact or collision sports. Median (range) duration of follow-up for the cohort was 27 (1-42) months. During this period, two patients sustained four sports-related in

Validation of the Food Safe Zone questionnaire for families of individuals with Prader-Willi syndrome.

BACKGROUND: Prader-Willi syndrome (PWS), a genetic neurodevelopmental disorder, is characterized by hyperphagia and significant behavioral problems. Hyperphagic individuals with PWS are chronically hungry yet rarely feel sated, and often engage in food-seeking behaviors. To avoid life-threatening obesity in their children, families implement food security strategies (e.g., locking food sources, constant supervision around food, alerting others). Although widely used, these strategies have yet to be systematically examined. We thus developed and analyzed the psychometric properties of a new mea

Tau-Clinical Mismatch Identifies Individuals with Co-Pathology and Predicts Clinical Trajectory.

IMPORTANCE: The heterogeneous course of Alzheimer's disease makes it difficult to predict individuals' cognitive trajectories, which is particularly important in the era of disease modifying therapy. Identifying individuals more likely to have co-pathology and differing disease courses using clinically practical tools remains a critical gap. OBJECTIVE: To evaluate tau-clinical mismatch for identifying resilient and vulnerable individuals and compare levels of co-pathology and clinical trajectories between groups. Apolipoprotein E abundance is elevated in the brains of individuals with Down syndrome-Alzheimer's disease.

Trisomy of chromosome 21, the cause of Down syndrome (DS), is the most commonly occurring genetic cause of Alzheimer's disease (AD). Here, we compare the frontal cortex proteome of people with Down syndrome-Alzheimer's disease (DSAD) to demographically matched cases of early-onset AD and healthy ageing controls. We find wide dysregulation of the proteome, beyond proteins encoded by chromosome 21, including an increase in the abundance of the key AD-associated protein, APOE, in people with DSAD compared to matched cases of AD. To understand the cell types that may contribute to changes in prote
