Internal derangement and the accessory ligaments around the temporomandibular joint.

A model that describes the mechanics of jaw opening and the associated movements of the disc in a healthy human temporomandibular joint is used to explain the abnormal movements associated with internal derangement of the joint. A change in the constraints on jaw opening imposed by the accessory ligaments around the jaw can account for, among other things, clicks on opening and closing, disc displacement and replacement, and the conditions of open and closed lock.

[Changes in the wrist joint in spontaneous course of scaphoid pseudarthrosis].

Scaphoid non-union always leads to carpal arthrosis. What is the natural history and course and sequence of development of arthrosis in scaphoid bone pseudarthrosis and what factors play a role in this? Forty-nine patients with scaphoid non-union were followed up; the period of observation was between three and 39 years. For comparison, 55 patients in whom a Matti-Russe reconstruction had been carried out successfully in the same period were followed up. Both the clinical and the radiological appearance seemed to be better in the operated patients than in the unoperated patients. The time fact

Influence of heel height on ankle joint moments in running.

Clinically, heel lifting or heel wedging in running shoes has been proposed as a prevention and treatment of Achilles tendinitis. It has been speculated that heel lifting decreases the Achilles tendon forces. The purpose of this study was to determine the effect of heel height on resultant ankle flexion moments during running. It was assumed that plantarflexion moments at the ankle joint would indicate Achilles tendon loading. Each of the five subjects performed five running trials (4.6 m.s-1) for each of the five shoes, differing only in heel height (2.1-3.3 cm). Resultant plantar-/dorsiflexi

Irreducible dislocation of the fifth metatarsophalangeal joint: a case report.

Metatarsophalangeal joint dislocations are unusual in children and usually occur in the hallux. We are reporting a case of irreducible traumatic dislocation of the fifth metatarsophalangeal joint. At open reduction, the metatarsal head was incarcerated under the flexor digitorum longus. After returning the flexor digitorum longus tendon to its anatomical position, the metatarsophalangeal joint reduced and was stable.

Wear debris. An environmental issue in total joint replacement.

There is a growing concern that osteolytic lesions, often adjacent to otherwise stable implants, are a recent phenomenon caused by some recent change in polyethylene, metal, or other aspect of the total hip construction. This study investigates the possibility that bearings and modular connections used in modern hip replacements are an unappreciated source of particulate debris. Measurements taken from contemporary femoral bearings show a significant mismatch in both surface finish and sphericity of mating metal and polyethylene components, with sphericity of inserts being much worse then sphe

Anatomic consideration in the anterior approach to the sacro-iliac joint.

Anatomical study of L4 nerve branch, L5 nerve root, and bony stock in the sacroiliac region was performed in cadaveric specimens to provide anatomical references for anterior fixation of the sacroiliac joint. At 1 cm intervals along the sacro-iliac joint, the distance to the lateral border of the L4 nerve branch (to the lumbosacral trunk) and the L5 nerve root was measured. Computed tomography scans and cadaveric sectioning of the sacro-iliac region were performed to evaluate the bony stock available for fixation and the optimal screw direction to prevent neurologic damage. At 4 cm above the p

Functional joint analysis of the foot in juvenile chronic arthritis.

In juvenile chronic arthritis, foot joints become affected relatively frequently. Depending on the pattern of joint involvement, different deviations or deformities can develop. The most important malpositions are pes valgoplanus, pes cavus, heelfoot (pseudocavus), hallux flexus resp. rigidus, hallux valgus, forefoot adduction and claw or hammer toes. Combinations of several deviations occur frequently. Foot involvement influences the gait pattern. Usually the heel strike phase is shortened. The loading and push-off phases are disturbed according to the pattern of joint involvement and the res…

[Application possibilities of MR arthrography in diseases of the shoulder joint].

In a prospective study possible indications for MR arthrography of the shoulder were evaluated. 37 patients were examined before and after intraarticular administration of a 2-mmolar solution of Gd-DTPA. MR arthrography was performed if there was no joint effusion and/or an uncertain finding concerning the rotator cuff or the capsulolabral complex on plain MR images. MR arthrography leads to a better demonstration of labrum pathology in 11/22 patients and to a superior delineation of the capsuloligamentous apparatus in 20/22 cases. In 9/15 patients with impingement lesions MR arthrography allo

The shape of the condyle and position of the meniscus in temporomandibular joint dysfunction.

The position of the meniscus seems to be significant in the natural history of temporomandibular joint dysfunction. It can be identified on MRI and arthrography but generally its displacement is referred to only in the sagittal plane. Little consideration is given to the shape and size of the condyle which could have great influence upon the position which the meniscus takes up. This paper reports the findings in 105 TM joints which have had arthrotomy for meniscoplasty (meniscopexy) and menisectomy over an 8-year period. Only 30% of the condyles had a normal size and shape; 24% were excavated…

Evaluation of assistive devices after a course in joint protection.

This study assessed a joint protection education program and investigated the costs and effects of assistive devices. Fifty-three women with seropositive rheumatoid arthritis, whose ages ranged from 29 to 65 years, attended a standardized joint protection course (13 hours). A self-report questionnaire was constructed listing the most common devices, and the subjects were asked to check which devices they had received, which ones they used and did not use, and why. Pain during activities of daily living and when using assistive devices was measured using the visual analogue scale. Results show

[Degenerative chondroarthropathy of the sternocostal joint following heart surgery].

Involvement of the sternocostal joints was investigated in a series of 46 males and 18 females following median sternotomy annually in a 5-year period and compared to 62 age- and sex matched control subjects after one year solely. Both groups had a mean age of 49.2 years. The degenerative chondroarthropathy of sternocostal joints was 1.69-times more frequent in heart operated upon patients as compared to control persons. Based on radiographic findings the degenerative chondroarthropathies were classed in 0 to 3 severity groups. Were seen articular space narrowing in 95.4%, osteophytes of the m…

[Congenital and acquired deformities of knee joint menisci in children].

Children with congenital and acquired pathology of the menisci account for 7.2% of all patients with internal pathological conditions of the knee joint. The clinical manifestations are due to instability of the joint. In rupture of normally developed menisci the instability is acute or subacute and is displayed by general unspecific reactive symptomatology. The clinical signs of instability are revealed in children with anomalous development of the menisci due to decompensation of the defence-adaptational mechanisms and are evidence of a chronic pathological condition. A precise topical diagno

Sauvè-Kapandji operation for disorders of the distal radioulnar joint.

A series of 13 patients is reported in which a Sauvè-Kapandji procedure consisting of arthrodesis of the articulation between the radius and ulna combined with resection of the collum ulnae was used to treat posttraumatic caput ulnae syndrome. Among the nine female and four male patients whose median age was 42 years (range: 23 to 77 years), nine sustained a distal fracture that had healed with shortening of the radius or with subluxation of the caput ulnae. Median postoperative observation time was 16 months (range: six to 27 months). Preoperatively, all patients had persistent medial wrist p

Rupture of the ulnar collateral ligament of the metacarpophalangeal joint of the thumb.

The results after operations for rupture of the ulnar collateral ligament of the metacarpophalangeal joint of the thumb are reported to be good. Few authors describe the measurements used to assess the postoperative function of the thumb. We review the results of 15 patients, who underwent surgery between 1982 and 1987. We found that the number of patients satisfied with the operation was consistent with that reported in the literature. On the other hand, we found poor agreement between the subjective complaints and the objective findings. We therefore propose and describe a scoring system to

Computed tomography in differential diagnosis of temporomandibular joint disorders.

Computed tomography (CT) has great potential for imaging intra- and extracapsular hard-tissue abnormality of the temporomandibular joint (TMJ). CT is not the best method of imaging disk position and form. For differential diagnosis of TMJ disorders, CT is especially successful in bony lesions. The study includes 16 examples of TMJ hard-tissue abnormality. In the differential diagnosis of all cases, CT played a decisive role.

An additional case of pachygyria, joint contractures and facial abnormalities.

Two previous case reports have described an apparently new lethal syndrome consisting of pachygyria, joint contractures and facial abnormalities (Winter et al., 1989; Tsukahara et al., 1990). Another report describes a non-lethal case in which dysmorphic features were not noted (Massa et al., 1988). We now report on what appears to be an additional lethal case. This male infant had a lethal condition with features of large fontanelle, pachygyria with incomplete opercularization, varus contractures of the hands and feet, small palpebral fissures, hypertelorism, a small penis, cryptorchidism, ca

Diagnosis of pathology of the temporomandibular joint. Clinical and imaging perspectives.

The pathologic conditions that involve the TMJ are similar to those conditions that involve other joints in the body. Therefore, many of the radiologic characteristics of these diseases are also similar. Nevertheless, because the articular surface of the condyle is an important site of mandibular growth, leading to secondary facial asymmetries when damaged in young patients, this possibility must be considered when evaluating the radiographic changes. It also is important to understand that functionally the TMJs act as a single unit and that any alteration in function caused by a pathologic pr…
