Supportive bandage, removable splint, or walking casts for low-risk ankle fractures in children: a feasibility randomized controlled trial.

AIMS: It is unclear if a supportive bandage, removable splint, or walking cast offers the best outcome following low-risk ankle fractures in children. The aim of this study was to evaluate the feasibility of a randomized controlled trial to compare these treatments. METHODS: Children aged five to 15 years with low-risk ankle fractures were recruited to this feasibility trial from 1 February 2020 to 30 March 2023. Children were randomized to supportive bandage, removable splint, or walking cast for two weeks. Follow-up at two, six, and 12 weeks was undertaken to determine feasibility for a def

Barriers in motion: a multi-methods approach to exploring physical activity, physical function, and program challenges in adult day services.

INTRODUCTION: Physical activity (PA) helps older adults age in place and retain independence. Adult Day Centers (ADCs) are critical community-based spaces that provide PA programming, yet the dosage and impact of PA in these settings remains empirically unassessed. METHODS: This study used a multi-methods cross-sectional design to assess PA and physical function among ADC participants, as well as directors' perspectives on PA programming. PA was assessed via an ActivPAL inclinometer, and physical function was assessed via the Short Physical Performance Battery (SPPB) and the Timed Up and Go (

Palliative care models in primary health care system of India: a scoping review.

INTRODUCTION: Palliative care is essential in alleviating the suffering of patients with chronic or terminal illnesses and improving their quality of life. Despite progress, in India it reaches only 1% of the population, highlighting significant gaps in access and awareness that can be mitigated by integration into primary health care. States like Kerala have demonstrated community-based palliative care through Local Self-Government Institutions (LSGIs). This scoping review aimed to examine models and interventions for integrating palliative care into India’s Primary Health Care (PHC) system,

Characterization of the family-level Borreliaceae pan-genome and development of an episomal typing protocol.

UNLABELLED: The Borreliaceae family includes many obligate parasitic bacterial species etiologically associated with a myriad of zoonotic borrelioses, including Lyme disease and vector-borne relapsing fevers. Borreliaceae infections are difficult to detect by both direct and indirect methods, often leading to delayed and missed diagnoses. Efforts to improve diagnostics center around the development of molecular diagnostics (MDx), but due to deep tissue sequestration and the lack of persistent bacteremias, even MDx assays suffer from a lack of sensitivity. Additionally, the extensive genomic he

Estimating the epidemiological and economic impact of providing nutritional care for tuberculosis-affected households across India: a modelling study.

BACKGROUND: Approximately 20% of global tuberculosis incidence is attributable to undernutrition, increasing to more than a third in India. Targeting nutritional interventions to tuberculosis-affected households is a policy priority, but understanding of epidemiological and economic impacts is limited. We aimed to estimate the population-level epidemiological and economic effect of such an intervention. METHODS: We used a previously published, age-stratified, compartmental transmission model of tuberculosis in India, and incorporated explicit BMI strata linked to disease progression and treat

The Bangladesh clubfoot project: the first 5000 feet.

BACKGROUND: Bangladesh is one of the most populous countries in the world at 160 million with 1/3 existing below the poverty line. With an annual birth rate of approximately 3.2 million, an estimated incidence of 1:900 live births, the country has approximately 5000 new cases of idiopathic congenital talipes equinovarus per annum. The Bangladesh sustainable clubfoot program, Walk for Life (WFL), was conceived to respond to this unmet need. METHODS: WFL started in 2009 and has rapidly grown to 35 clinics. Overseas experts initially increased local capacity by training a team of national parame

Management of onchocerciasis among adolescents in nkwanta north district of Ghana: Qualitative study of adolescents' perception, community and health system support.

BACKGROUND: Onchocerciasis affects the quality of life to a greater extent among affected individuals. The World Health Organization (WHO)'s Onchocerciasis Control Program (OCP) has effectively reduced the prevalence of onchocerciasis by interrupting the transmission of the parasite and by mass population treatment in the regions at risk of the disease. Despite the successful reduction of the prevalence of onchocerciasis by WHO, the socioeconomic burden resulting from the disabilities caused by onchocerciasis are still immense. This study sought to explore the adolescents' perception regarding

The body and the brain keep the score: a data-driven conceptual model linking trauma and postural tachycardia syndrome.

Postural tachycardia syndrome (POTS) is a common, often disabling disorder of autonomic nervous system regulation without a unifying etiological account. Converging evidence suggests infectious, physical, and emotional threats frequently precede onset. We propose a hypothesis-generating model in which POTS may, in some individuals, involve threat-induced, centrally maintained disruption of brain-body communication that may be responsive to neuroplasticity-based behavioral medicine. In this paper, we synthesized multidisciplinary literature and our team's data spanning (a) autonomic and central

Epigenetic Programming of Stress-Response Systems: A Systematic Review of How Early Life Stress Shapes Developmental Trajectories of Mental Health.

Early life stress (ELS) is a critical environmental factor influencing developmental trajectories of behavioral and emotional regulation. Increasing evidence suggests that such experiences can alter biological systems during sensitive developmental periods through epigenetic processes. This systematic review aims to synthesize empirical human studies examining how ELS contributes to the developmental programming of mental health via epigenetic regulation of stress-response systems. This systematic review was conducted in accordance with PRISMA 2020 guidelines. A comprehensive search of PubMed/

Diagnostic Accuracy of Tests for Hymenoptera Venom Hypersensitivity: A Systematic Review and Bayesian Meta-Analysis Protocol.

BACKGROUND AND AIMS: Hymenoptera venom hypersensitivity is a potentially life-threatening condition that may affect up to 7.5% of adults worldwide and can result in systemic allergic reactions ranging from generalized cutaneous symptoms to severe anaphylaxis. Accurate diagnosis following a systemic sting reaction is essential to guide initiation of venom immunotherapy, a highly effective but resource-intensive intervention. However, the diagnostic performance of commonly used tests, including venom skin testing, serum venom-specific IgE, and component-resolved diagnostics, varies across studie

A survey of SARS-CoV-2 tropism.

The coronavirus disease 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has significantly burdened global public health. However, the tropism of SARS-CoV-2 within the human body remains not fully understood. In this review, we overview the literature on SARS-CoV-2 infection across various human organs and tissues. We summarize the relevant specimen types, techniques for examining SARS-CoV-2 tropism, and findings at both organ/tissue and cellular levels. To systematically evaluate the evidence supporting SARS-CoV-2 tissue tropism, we establish a

Effective Management of Severe Epidermal Growth Factor Receptor Inhibitors-Induced Papulopustular Eruption With Low-Dose Isotretinoin and Corticosteroids.

Epidermal growth factor receptor inhibitors (EGFRi) are approved for treating various cancers. Given that EGFR signaling is crucial for normal skin growth and repair, inhibiting this pathway can disrupt skin homeostasis and integrity. Although generally well tolerated, molecularly targeted therapies can lead to skin-related adverse effects that significantly impact patients' quality of life, often resulting in treatment interruptions. The most common cutaneous reaction associated with EGFRi is a diffuse, papulopustular acneiform eruption. Management strategies can be tailored based on the seve

Healthcare waste: from incineration to pyrolysis: a comparative life cycle assessment of two different healthcare plastic waste management processes.

BACKGROUND: Plastic waste from healthcare is an increasing environmental challenge, particularly in resource-intensive clinical pathways such as dialysis. Peritoneal dialysis (PD) generates large quantities of single-use plastic materials, making its waste stream a priority for sustainable intervention. Conventional disposal through incineration contributes substantially to greenhouse gas emissions (GHG) and produces hazardous ash. Pyrolysis, a chemical recycling process that converts mixed plastic waste into usable outputs, has been proposed as a lower-carbon alternative. This study aimed to

Telling our history: a narrative study with Indigenous elders, wisdom for the coming generation.

INTRODUCTION: The history of AIAN people in the US has been told many times, but Western academic researchers have failed to shift that history into wisdom and uncover teachings for living in the present day. The aim of this paper is to document Indigenous elders' childhood experiences and stories, and to record the wisdom they wish to pass on to the next generation in an accessible, written form. METHODS: This study used elder-centered research methods situated within a postcolonial paradigm. Methods centered on the lived experience and values of Indigenous people and a three-step process of
