Fertility therapies, infertility and autism spectrum disorders in the Nurses' Health Study II.

BACKGROUND: An increasing number of women are utilizing fertility treatments, but little is known about their relation to autism spectrum disorders (ASD). METHODS: To determine the association between maternal fertility therapy use and risk of having a child with ASD, we conducted a nested case-control study within the Nurses' Health Study II (n = 116,430). Maternally reported diagnoses of ASD were confirmed through a supplementary questionnaire and, in a subgroup, the Autism Diagnostic Interview-Revised. Controls were randomly selected by frequency matching to case children's year of birth.

The availability of allied health care in nursing homes.

PURPOSE: To determine the availability of allied health care in nursing homes in the Netherlands, and its dependency on characteristics of the nursing home. METHODS: Structured surveys by telephone were carried out in a sample of 100 from a country total of 286 somatic (for somatic patients only) and combined (with units for both somatic and psychogeriatric patients) nursing homes. Multiple linear regression analyses were performed to determine relationships between the availability of care and the type of nursing home, its country location (urban/non-urban) and the presence of specific wards

Predicting the risk of "permanent" nursing home residence: the role of community help as indicated by family helpers and prior living arrangements.

OBJECTIVE: This study examines the difference between permanent and transitory residence in a nursing home with special emphasis on the extent to which the risk of a long nursing home stay is reduced by the availability of informal help in the community. DATA SOURCE: Secondary data were used, taken from the National Long-Term Care Surveys of 1982 and 1984. The 1982 NLTCS samples disabled elderly living in the community. For these community dwellers, the 1984 NLTCS provides information on continued residence in the community and on their nursing home episodes between 1982 and 1984. METHOD OF A

Developing and Validating an Inclusive and Cost-Effective Prediction Algorithm for Survival and Death Among People Living With HIV in Sub-Saharan Africa: Protocol for a Meta-Analysis and Case-Control and Cost-Effectiveness Study.

BACKGROUND: Premature death in people with HIV in sub-Saharan Africa (SSA) is highly preventable. However, the lack of inclusive, cost-effective prognostic tools remains challenging. Most prognostic tools have been developed in high-income economies. The distinct cultural dynamics in HIV-related death epidemiology makes them unsuitable for the region. Additionally, the models lack systematic stratification of death determinants based on clinical relevance, and some included factors are too expensive for people with HIV in SSA. OBJECTIVE: We aimed to create a tailored predictive model that con

Effects of clinical, comorbid, and social determinants of health on brain ageing in people with and without HIV: a retrospective case-control study.

BACKGROUND: Neuroimaging reveals structural brain changes linked with HIV infection and related neurocognitive disorders; however, group-level comparisons between people with HIV and people without HIV do not account for within-group heterogeneity. The aim of this study was to quantify the effects of comorbidities such as cardiovascular disease and adverse social determinants of health on brain ageing in people with HIV and people without HIV. METHODS: In this retrospective case-control study, people with HIV from Washington University in St Louis, MO, USA, and people without HIV identified t

Post-Acute Care Pathways After Sexual Violence and Intimate Partner Violence: An International Health-Services Scoping Review with Implications for Italy.

Background/Objectives: Survivors of sexual violence and domestic violence/intimate partner violence (IPV) often require support beyond the immediate emergency encounter; however, post-acute care remains inconsistently defined, unevenly organised or conceptualised, and fragmented across service systems. This scoping review mapped international post-acute follow-up, care, assistance, and support pathways, with particular attention to organisational models, continuity mechanisms, loss to follow-up after first access, and implications for the Italian context. Methods: We conducted an international

Coping in crisis: a qualitative study of stress and resilience in Ontario's long-term care workforce.

BACKGROUND: The global long-term care (LTC) sector is increasingly strained by entrenched structural vulnerabilities, including persistent workforce shortages, constrained resources, high resident-to-staff ratios, and rising clinical complexity, all of which compromise the delivery of safe, high-quality care. In Canada, these challenges are particularly acute, placing substantial psychological, emotional, and ethical strain on LTC workers. Despite this, empirical insight into their lived experiences, particularly within Ontario remains limited. OBJECTIVE: This study explores the lived experie

Automated video-based AVPU assessment within a FHIR-enabled clinical decision support framework.

Accurate and timely assessment of consciousness is critical for triage, escalation of care, and patient safety in emergency and hospital settings. However, documentation using the AVPU scale (Alert, Verbal, Pain, Unresponsive) remains inconsistent owing to high workload, subjectivity, and fragmented workflows. This study developed and evaluated Consc.ia, a video-based clinical decision-support platform that automates AVPU inference while preserving clinician oversight and enabling seamless, interoperable documentation through HL7 FHIR. A simulated AVPU dataset comprising 136 videos from 58 hea

A prediction model for 'ICU mortality or prolonged ICU stay' in critically unwell patients with acute pancreatitis: insights from a 2003-2020 cohort analysis using the ANZICS-CORE database.

OBJECTIVE: Critically unwell patients with acute pancreatitis (AP) are at increased risk of mortality and prolonged ICU length of stay (LOS). We quantified the frequency, risk factors and complications of prolonged ICU LOS in a large cohort of critically unwell adult patients with AP and developed a model to predict a low-risk trajectory 'survived ICU with ICU LOS ≤7 days' versus a high-risk trajectory 'ICU mortality or ICU LOS > 7 days'. METHODS: A retrospective cohort analysis of adult patients admitted to Australian and New Zealand ICUs with AP between 2003 and 2020 was conducted using the

Social Acceptance of Smart Glasses in Health Care: Model Evaluation Study of Anticipated Adoption and Social Interaction.

BACKGROUND: Despite the growing interest in smart glasses, it is striking that they are not widespread among health care professionals. Previous research has identified issues related to social interactions involving the use of smart glasses in public settings, which may differ from those associated with their application in health care contexts. OBJECTIVE: Assuming that smart glasses mediate contact between the health care provider and patient, the objectives of this research are two-fold: (1) to develop an instrument that combines the adoption and mediation perspectives, and (2) to gain ins

Predicting Episodes of Hypovigilance in Intensive Care Units Using Routine Physiological Parameters and Artificial Intelligence: Derivation Study.

BACKGROUND: Delirium is prevalent in intensive care units (ICUs), often leading to adverse outcomes. Hypoactive delirium is particularly difficult to detect. Despite the development of new tools, the timely identification of hypoactive delirium remains clinically challenging due to its dynamic nature, lack of human resources, lack of reliable monitoring tools, and subtle clinical signs including hypovigilance. Machine learning models could support the identification of hypoactive delirium episodes by better detecting episodes of hypovigilance. OBJECTIVE: Develop an artificial intelligence pre
