Assessment of autonomic nervous system dysfunction in multiple sclerosis and association with clinical disability.

Recent studies have reported autonomic dysfunction (AD) in multiple sclerosis (MS), and bladder and/or bowel dysfunction, orthostatic hypotension, and cardiac adaptation disorders have been observed in a wide range of patients (15-80%). The primary aim of this study is to investigate the frequency and association of AD in MS patients, assessed by sympathetic skin response (SSR) and a symptoms questionnaire. The secondary aims are to study the association of AD and disease disability assessed by expanded disability status scale (EDSS), as well as disease duration. One hundred clinically definit…

[Pathology of the thyroid gland and multiple sclerosis: a possible influence on efficacy and tolerability of treatment].

This paper reviews the literature and own data on prevalence of thyroid diseases in patients with multiple sclerosis (MS). It has been shown that 20-25% of untreated MS patients have autoimmune thyroiditis (AIT) and/or subclinical hypothyroidism. No correlation was found between age, gender, disease course, MS duration, disability status and prevalence of thyroid disorders. Disease-modifying therapy (DMT) of MS can induce or worsen the thyroid pathology. Female gender, pre-existing AIT, family history of thyroid disorders are considered as risk factors of thyroid dysfunction and autoimmunity d…

PARP-1 deficiency increases the severity of disease in a mouse model of multiple sclerosis.

Poly(ADP-ribose) polymerase-1 (PARP-1) has been implicated in the pathogenesis of several central nervous system (CNS) disorders. However, the role of PARP-1 in autoimmune CNS injury remains poorly understood. Therefore, we studied experimental autoimmune encephalomyelitis (EAE), a model for multiple sclerosis in mice with a targeted deletion of PARP-1. We identified inherent physiological abnormalities in the circulating and splenic immune composition between PARP-1(-/-) and wild type (WT) mice. Upon EAE induction, PARP-1(-/-) mice had an earlier onset and developed a more severe EAE compared…

Celiac disease, Behçet, and idiopathic thrombocytopenic purpura in siblings of a patient with multiple sclerosis.

Multiple sclerosis (MS) is a demyelinating disease of uncertain etiology. Many genetic and environmental risk factors have been associated with this disease including certain human leukocyte antigen haplotypes, Epstein-Barr virus infection, and vitamin D deficiency. We report a 30-year-old woman with MS, the product of consanguineous marriage, and three siblings with three different autoimmune diseases: idiopathic thrombocytopenic purpura, celiac disease, and Behçet's disease.

Multiple sclerosis presenting with erotomanic delusions in the context of "Don't ask, don't tell".

A female patient presented with erotomanic delusions of other female soldiers sexually harassing her and plotting to sexually assault her. On MRI, the patient was found to have numerous periventricular foci of signal changes extending into the deep white matter of both hemispheres. The MRI findings, along with lumbar puncture findings of oligoclonal bands in the cerebral spinal fluid, were consistent with a diagnosis of multiple sclerosis. After treatment with aripiprazole acutely and interferon beta-1a for several months, the patient experienced a decrease in her psychosis. The discussion of …

Effectiveness of vocational rehabilitation intervention on the return to work and employment of persons with multiple sclerosis.

BackgroundMultiple sclerosis is a neurological disease that frequently affects adults of working age, resulting in a range of physical, cognitive and psychosocial deficits that impact on workforce participation. Although, the literature supports vocational rehabilitation (VR) approaches in persons with multiple sclerosis (pwMS), the evidence for its effectiveness is yet to be established.ObjectivesTo evaluate the effectiveness of VR programs compared to alternative programs or care as usual on return to work, workability and employment in pwMS; to evaluate the cost effectiveness of these progr…

Lack of effect of cannabis-based treatment on clinical and laboratory measures in multiple sclerosis.

The endocannabinoid system (ECS) is involved in the pathophysiology of multiple sclerosis (MS), and relief from pain and spasticity has been reported in MS patients self-medicating with marijuana. A cannabis-based medication containing Delta(9)-tetrahydrocannabinol and cannabidiol (Sativex) has been approved in some countries for the treatment of MS-associated pain. The effects of this pharmaceutical preparation on other clinically relevant aspects of MS pathophysiology, however, are still unclear. In 20 MS patients, we measured the effects of Sativex on clinically measured spasticity and on n…

B cells and autoantibodies in the pathogenesis of multiple sclerosis and related inflammatory demyelinating diseases.

Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system (CNS). The mainstream view is that MS is caused by an autoimmune attack of the CNS myelin by myelin-specific CD4 T cells, and this perspective is supported by extensive work in the experimental autoimmune encephalomyelitis (EAE) model of MS as well as immunological and genetic studies in humans. However, it is important to keep in mind that other cell populations of the immune system are also essential in the complex series of events leading to MS, as exemplified by the profound clinical effic…

Loss of Na+ channel beta2 subunits is neuroprotective in a mouse model of multiple sclerosis.

Multiple sclerosis (MS) is a CNS disease that includes demyelination and axonal degeneration. Voltage-gated Na+ channels are abnormally expressed and distributed in MS and its animal model, Experimental Allergic Encephalomyelitis (EAE). Up-regulation of Na+ channels along demyelinated axons is proposed to lead to axonal loss in MS/EAE. We hypothesized that Na+ channel beta2 subunits (encoded by Scn2b) are involved in MS/EAE pathogenesis, as beta2 is responsible for regulating levels of channel cell surface expression in neurons. We induced non-relapsing EAE in Scn2b(+/+) and Scn2b(-/-) mice on…

Multiple bilateral renal oncocytoms in a known case of tuberous sclerosis: a case report.

One of the rare patterns of renal involvement by tuberous sclerosis is oncocytoma. This is a report of a known male patient diagnosed with tuberous sclerosis. He was asymptomatic, but his periodic abdominal ultrasound examination revealed bilateral renal masses. CT and MRI confirmed the presence of these masses which were proven histopathologically to be multiple oncocytomas.

A case of limited systemic sclerosis with p-ANCA, complicated by multiple cerebral hemorrhages.

ANCA associated vasculitides are a group of disorders characterized by the presence of necrotizing inflammation of blood vessels that involve venules, capillaries, arterioles, arteries, and veins. They are characterized by the presence of antineutrophil cytoplasmic antibodies (ANCA). ANCA associated vasculitis commonly affects the kidney producing proteinuria, hematuria and variable degrees of renal failure. Other organs involved include the lung, skin and peripheral nervous system. We present a patient with limited systemic sclerosis, who later developed pauci-immune glomerulonephritis with p

Heterogeneity of aquaporin-4 autoimmunity and spinal cord lesions in multiple sclerosis in Japanese.

Opticospinal multiple sclerosis (OSMS) in Asians has similar features to the relapsing-remitting form of neuromyelitis optica (NMO) seen in Westerners. OSMS is suggested to be NMO based on the frequent detection of specific IgG targeting aquaporin-4 (AQP4), designated NMO-IgG. The present study sought to clarify the significance of anti-AQP4 autoimmunity in the whole spectrum of MS. Sera from 113 consecutive Japanese patients with clinically definite MS, based on the Poser criteria, were assayed for anti-AQP4 antibodies by immunofluorescence using GFP-AQP4 fusion protein-transfected HEK-293T c…
