Elderly homeless veterans in Los Angeles: chronicity and precipitants of homelessness.

OBJECTIVES: We compared the characteristics of chronically homeless and acutely homeless elderly veterans to better understand precipitants of homelessness. METHODS: We conducted interviews with 33 chronically and 26 acutely homeless veterans aged 65 years and older receiving transitional housing services in Los Angeles, California, between 2003 and 2005. We asked questions regarding their sociodemographic characteristics and other social status measures. Other precipitants of homelessness were acquired via observation and open-ended and structured questions. RESULTS: Both veterans groups we

Adaptation of a general primary care package for HIV-infected adults to an HIV centre setting in Gaborone, Botswana.

BACKGROUND: As life expectancy of HIV-infected patients improves due to antiretroviral treatment (ART) and the importance of associated co-morbidities and chronic diseases increases, preventive care will become increasingly important. Adaptation of existing preventive guidelines to local environments will become a priority for HIV treatment programmes. METHODS: Guidance from the World Health Organization, a focused evidenced-based literature review, Botswana national guidelines, Botswana-specific morbidity and mortality data and centre-specific data were used to adapt a published general prim

Advancing critical care medicine with stem cell therapy and hypothermia for cerebral palsy.

With limited clinical trials on stem cell therapy for adult stroke underway, the assessment of efficacy also needs to be considered for neonatal hypoxic-ischemic brain injury, considering its distinct symptoms. The critical nature of this condition leads to establishment of deficits that last a lifetime. Here, we will highlight the progress of current translational research, commenting on the critical nature of the disease, stem cell sources, the use of hypothermia, safety and efficacy of each treatment, modes of action, and the possibility of combination therapy. With this in mind, we referen

Scleroderma in children: an update.

PURPOSE OF REVIEW: Scleroderma, in its localized and systemic presentation, represents the third most frequent rheumatic condition in childhood after juvenile idiopathic arthritis and systemic lupus erythematosus. Early diagnosis, appropriate assessment and effective treatment are crucial to improve the long-term outcome. RECENT FINDINGS: Recent studies, concerning histopathology and clinical associations with other conditions, open new horizons on the etiopathogenesis of scleroderma. New developments have been also reached in the field of outcome measures. In juvenile localized scleroderma (

[Hepatitis C! Experience in diagnosis and medical treatment. A literature review].

Chronic hepatitis C can have physical, psychological or emotional effects on a person. The chronic infection can result in an advanced liver disease and/or influence daily life and self-perception. Today, the possibility to eliminate the virus exists but the therapy is difficult, a decrease in quality of life is known. Anyway, the sustained viral response isn't achievable for everyone. Care in Switzerland usually takes place in the outpatient clinics. Nurse participation to improve self-management is in discussion. For this specific work knowledge of patient's perception is crucial. A literatu

Decision trees for evaluating skin and respiratory sensitizing potential of chemicals in accordance with European regulations.

Guidance for determining the sensitizing potential of chemicals is available in EC Regulation No. 1272/2008 Classification, Labeling, and Packaging of Substances; REACH guidance from the European Chemicals Agency; and the United Nations Globally Harmonized System (GHS). We created decision trees for evaluating potential skin and respiratory sensitizers. Our approach (1) brings all the regulatory information into one brief document, providing a step-by-step method to evaluate evidence that individual chemicals or mixtures have sensitizing potential; (2) provides an efficient, uniform approach t

Oropharyngeal dysphagia pathophysiology, complications and science-based interventions.

The etiology of oropharyngeal dysphagia can be broad, and includes aging with atrophy, debilitation, stroke, neurodegenerative and muscular diseases, tumor and postsurgical deformity, as well as effects due to medications and drying of the mucosal membranes. Pathophysiology depends on the multiple causative factors, including the cortex and neural connections to generate the swallow, as well as the oropharyngeal musculature. While chronic debilitation and age may result in nutritional deficiency and poor hydration, the other causes generally present with aspiration risk more acutely. Bacteriol

Bilateral Anterior Fracture-Dislocation of Shoulder Joint- A rare case with Delayed Presentation.

INTRODUCTION: The shoulder is the most frequently dislocated joint. Bilateral glenohumeral dislocations are rare and almost always posterior. Bilateral anterior fracture dislocations of humeral neck in a patient with seizure are extremely rare. We report one such case of delayed presentation of bilateral anterior fracture dislocation of shoulder after an epileptic attack. CASE REPORT: We describe a rare case of 30 year old gentleman who presented with first episode of seizure following alcohol withdrawal. Physical examination and radiographic assessment revealed fracture dislocation of bilate

Advances in sonographic scoring of rheumatoid arthritis.

Accurate assessment of disease activity and joint damage in rheumatoid arthritis (RA) is important for monitoring treatment efficiency and for prediction of the outcome of the disease. Therefore, a reliable imaging method needs to be used. Musculoskeletal ultrasound (US) is a sensitive method for the detection of both early inflammatory soft tissue lesions (eg, synovitis, tenosynovitis, and bursitis) and early bone lesions (eg, erosions) in arthritic joint diseases and correlates well with MRI. Several musculoskeletal US scores are used for monitoring RA disease activity. Different qualitative

Traumatic anterior shoulder instability in the athlete.

Anterior glenohumeral dislocation is common among athletes and may progress to recurrent instability. The pathoanatomy of instability and specific needs of each individual should be considered to prevent unnecessary absence from sport. Traditionally, primary dislocations have been managed with immobilization followed by rehabilitation exercises and a return to sporting activity. However, arthroscopic stabilization and external rotation bracing are increasingly used to prevent recurrent instability. In addition to the typical capsulolabral disruptions seen following a primary dislocation, patie

The role of childhood trauma, psychological problems, and coping in the development of deviant sexual fantasies in sexual offenders.

To understand how and why most people entertain deviant sexual fantasies without really desiring to act on them whereas some people put them into practice, it is essential to better understand the origin, development, and functions of deviant sexual fantasy in sexual offenders. The present paper investigates three areas of the literature on sexual offenders, i.e. early traumatic experiences, psychological problems, in terms of psychiatric disorders and negative emotional states, and coping strategies, hypothesizing a possible path through which early traumatic experiences may lead to deviant s

Imaging methods for determining uptake and toxicity of carbon nanotubes in vitro and in vivo.

Demand for carbon nanotubes (CNTs) is increasing rapidly in electrical, mechanical, and health and medical applications due to their thermal, electrical conductive and other properties. The continued commercial up-scaling of CNT production and application needs to be accompanied by an understanding of the occupational health, public safety and environmental implications of these materials. An increasing volume of literature on the toxicity of CNTs is being published; however, the results of these studies are frequently inconclusive. Due to the enormous number of permutations of nanoparticle sh

[Lung transplantation for pulmonary emphysema--who, when, how?].

End stage pulmonary emphysema is the most common indication for lung transplantation worldwide. The shortness of donor organs and the better natural prognosis compared to other diseases leading to transplantation such as pulmonary fibrosis and cystic fibrosis demands careful patient selection.Lung transplantation is considered in patients with declining lung function after receiving all conservative treatment options including smoking cessation and rehabilitation programmes. Preoperative evaluation using consensus criteria needs to be performed by a multidisciplinary team in specialized centre

Neuroimaging of narcolepsy.

Neuroimaging techniques have refined the characterization of neural structures involved in the regulation of normal sleep-wake cycle in healthy humans. Yet brain imaging studies in patients with sleep disorders still remain scarce. In narcoleptic patients, structural and functional brain imaging studies have suggested the involvement of the hypothalamus in the pathophysiology of narcolepsy, plausibly consistent with an impairment of the hypocretin-orexin system. Some recent studies have further suggested that cataplexy, a key feature of the narcoleptic syndrome, might result from a dysfunction

Rehabilitation and stoma care: addressing the psychological needs.

Stoma surgery may be required for many reasons. The circumstances in which intervention is required, with possible permanent changes to the patient's conventional body image and lifestyle, will influence psychological recovery (Borwell, 1997; Black, 2000). Within a diverse multicultural society, addressing the individual psychological problems of the effects of mutilating surgery due to acute/chronic illness can be particularly challenging for health professionals. This can be further compounded if the patient has a physical or mental disability. Stoma care nurses play a vital role in supporti

Prevention, screening, and management of osteoporosis: an overview of the current strategies.

Osteoporosis is a major public health concern, resulting in increased fracture risk. Osteoporosis-related fractures impose a considerable economic burden on health care systems, and the disease has severe, debilitating consequences if left untreated. Measures for osteoporosis prevention should begin at childhood and include balanced nutrition, physical activity, and avoidance of risk factors such as smoking. In adulthood, early recognition of osteoporosis followed by timely and effective management can reduce fracture risk. However, the rates of screening and treatment for osteoporosis are low
