The influence of an anteroposterior accessory glide of the glenohumeral joint on measures of peripheral sympathetic nervous system function in the upper limb.

SUMMARY. The present study was undertaken to determine whether a sympathoexcitatory effect would result from a manipulative therapy technique applied to a peripheral joint. The technique selected was a Grade III posteroanterior accessory glide of the right glenohumeral joint with the arm positioned in glenohumeral abduction and internal rotation. Previously, spinal manipulative therapy techniques have been shown to produce such an effect. Nineteen normal subjects participated in the experiment. Treatment, placebo and control conditions were compared. Skin conductance and skin temperature of th

Simian immunodeficiency virus: a model for neuroAIDS.

In addition to its profound effects on the immune system, HIV also infects the CNS and can cause abnormalities in infected individuals ranging from mild cognitive and motor disorders to frank dementia. We have been actively investigating the molecular and cellular mechanisms underlying the CNS manifestations of lentivirus infection through the comparative evaluation of brain pathophysiology under a number of parallel interrelated strategies. Here we describe our ongoing studies with the SIV/rhesus macaque system. We have applied an interdisciplinary multistep approach, utilizing viral, immunol

[Neurological involvement following triple virus vaccine].

INTRODUCTION: Prophylactic immunization against measles, rubella and mumps is carried out using a trivalent vaccine. The second dose is recommended at 11-12 years-old. Neurological complications have been described after vaccination for measles and rubella. All cases occurred several days after vaccination. CLINICAL CASES: We describe four girls aged between 10 and 11 years old who were vaccinated on the same day. Almost immediately they complained of symptoms in the vaccinated arm. These were pain, paresthesia, coldness, pallor and sweating of the hand. On the following days there was loss o

Knowing where and knowing what: a double dissociation.

We report a double dissociation between visuo-spatial abilities and semantic knowledge (knowledge of the names and attributes of objects and people), in two brain-injured people with longstanding stable impairments, using a wide range of tests to explore the extent of the dissociation, MU, who has bilateral lesions of occipito-parietal cortex, shows severe spatial disorientation with relatively well-preserved semantic knowledge. He is contrasted with JBR, who has bilateral temporal lobe damage and shows severe semantic problems and no impairment on visuo-spatial tasks. Our findings thus demons

Maturation of the auditory brainstem in low risk-preterm infants: a comparison with age-matched full term infants up to 6 years.

Whether preterm birth has a significant effect on the maturation of the human brain remains an equivocal issue. Since experience plays a crucial role in the development of the brain, it is conceivable that extra-uterine preterm exposure could exert some effects on the maturation of the developing human brain. The present study compared the post-term maturation of the central components of brainstem auditory evoked response (BAER) in low risk preterm infants with that of age-matched full term infants up to 6 years. The preterm infants demonstrated similar maturational profiles to those of the t

[Verbal auditory agnosia: SPECT study of the brain].

Verbal auditory agnosia are rare in clinical practice. Clinically, it characterized by impairment of comprehension and repetition of speech but reading, writing, and spontaneous speech are preserved. So it is distinguished from generalized auditory agnosia by the preserved ability to recognize non verbal sounds. We present the clinical picture of a forty-years-old, right handed woman who developed verbal auditory agnosic after an bilateral temporal ischemic infarcts due to atrial fibrillation by dilated cardiomyopathie. An experimental study in chickens for the pathogenesis of idiopathic scoliosis.

Experimentally induced scoliosis was investigated in pinealectomized chickens using pathologic and neurophysiologic means. A total of 90 chickens were tested; 30 served as a normal control, 30 received an autografted pineal body in the intramuscular tissue of the trunk, and 30 underwent pinealectomy without autograft. Scoliosis developed in all pinealectomized chickens within 2 weeks, showing gradual progression during the next 5 or 6 weeks. At 3 months, the three-dimensional spinal deformity consisted of lateral curvature and vertebral body rotation, resulting in a prominent lordoscoliosis at

Relative frequency of nerve conduction abnormalities at carpal tunnel and cubital tunnel in France and the United States: importance of silent neuropathies and role of ulnar neuropathy after unsuccessful carpal tunnel syndrome release.

The two most frequently diagnosed nerve compression syndromes of the upper extremities are carpal tunnel syndrome and cubital tunnel syndrome. In order to determine the relative frequencies of nerve conduction abnormalities at the carpal tunnel and cubital tunnel in France and the United States, we evaluated all patients (nFR = 882 and nUS = 818) who had nerve conduction studies of the upper extremities over a six-yearFR or three-yearUS interval. In both France and the United States, the risk of electrophysiological abnormalities was 2 to 1 for the wrist relative to the elbow. Abnormal median

[Acute meningoencephalitis caused by Coxiella burnetii with periodic EEG complexes].

Q fever is a zoonosis found worldwide and is produced by Coxiella burnetii. It may be acute or chronic with neurological manifestations being infrequent. Several cases of acute encephalitis or meningoencephalitis have been described, generally with an evolution towards cure regardless of the use of selective antibiotic treatment. Recently the authors had the opportunity to study a 33 year old male presenting acute meningoencephalitis in which the clinical manifestations, CSF findings (increase in cellularity with lymphocytic predominance and excess proteins ) and neurophysiological findings (a

Bulbocavernosus reflex: its validity as a diagnostic test of neurogenic impotence.

Measurement of the bulbocavernosus reflex is used widely to diagnose underlying neurogenic disorders in erectile dysfunction. A prolonged bulbocavernosus reflex latency (that is more than 45 msec.) or the absence of a reflex response of the bulbocavernosus muscles during electrical stimulation of the glans penis is considered a sign of neurological disease. Since only a few experimental studies have been performed in man related to the neurophysiological mechanism of erection, and since the results of these studies were contradictory the diagnostic validity of bulbocavernosus reflex measuremen

Electromyography of the larynx and related technics.

Electrodiagnosis of the laryngeal muscles by electromyography, neuromyography and reflexmyography enables diagnosis, follow up and prognosis of vocal cord palsies as well as indications for surgery. The development of new endoscopes now permits direct inspection of the oropharyngeal and endolaryngeal muscles. A method of transoral electromyography of laryngeal muscles in the awake patient has been established using these zoom-endoscopes for supervision of the transoral placement of needle or hooked-wire electrodes into the muscular agonists and antagonists of the larynx. [Indications for studying evoked potentials in childhood. Methods--indications--value].

Evoked potentials (EP) represent a valuable addition to currently applied diagnostic methods in neuropediatrics. Profound knowledge of the neurophysiological conditions producing EP-alterations allows basic conclusions, that cannot be gained or replaced by other investigations. EP-investigation demonstrate the existence but not the nature of a lesion in the CNS. Further diagnostic work-up usually will be necessary. Proved EP-alterations produce reproducible diagnostic results and give clues regarding its localization. Evoked potentials can be used as a screening-method for neuropediatric disea

Comparison of Hoffmann reflex with quantitative assessment of cutaneous sensation in diabetic neuropathy.

Neurophysiological studies of diabetic patients may show abnormalities even when clinical examination has revealed no sign of neuropathy. Quantitation of sensory function provides additional information when examining a patient with neuropathy. The value of the Hoffmann (H) reflex was investigated in diabetics who had undergone this examination. H-M intervals of the H-reflex, vibration perception thresholds (VPT's) and thermal discrimination thresholds (TDT's) were determined in 34 diabetics with clinical evidence of neuropathy and 16 diabetics without symptoms or signs of neuropathy. Median nerve compression in the carpal tunnel--functional response to experimentally induced controlled pressure.

Controlled external compression was applied to the medium nerve of 16 volunteer subjects. Tissue fluid pressure in the carpal canal was monitored with a wick catheter and pressures of 30, 60 and 90 mm Hg were induced for periods varying from 30 to 90 minutes.l Sensory and motor conduction and two-point discrimination were continuously monitored. Tissue compression at 30 mm Hg caused mild neurophysiological changes and symptoms of hand paresthesias. Compression at both 60 and 90 mm Hg induced a rapid, complete sensory conduction block which consistently preceded a motor block by 10 to 30 minute

Sensorimotor polyneuropathy in light chain multiple myeloma.

The authors describe an adult onset sensorimotor polyneuropathy in a woman with light chain K-type myeloma. Neurophysiological investigation showed a severe reduction of conduction velocity of the peripheral nerve to the four limbs. A muscle and sural nerve biopsy were performed. Sural nerve displayed a severe loss of myelinated and unmyelinated fibers. No amyloid deposit was observed in vessels, perineurium and endomysium by electron microscopy examination. No onion bulb formation was present in nerve tissue, but many demyelinated fibers were surrounded by circumferentially disposed Schwann c

The carcinomatous neuromyopathy of oat cell lung cancer.

A prospective controlled clinical-neurophysiological-pathological study of 71 patients with oat cell carcinoma of the lung revealed no increased incidence of peripheral neuropathy at the initial stages of illness. All patients developed neuropathy by the time they had lost 15% of their body weight, but the neuropathy was less severe than in 20 age-matched alcoholic patients with an equal degree of weight loss. The weight loss and peripheral neuropathy progressed with atrophy of type II (adenosine triphosphatase-positive) muscle fibers out of proportion to the patient's loss of body weight. By

[Importance of EEG studies in the assessment of hearing and equilibrium disorders of central origin].

In a group of 52 patients with hearing and equilibrium disturbances of central origin EEG was done. The obtained EEG findings were correlated with: a) main clinical manifestations of the type of function loss or stimulation, and b) disturbances of behavioural perception, that is specific signs of disturbed perception and non-specific extraperceptive signs. Disturbed hearing, independently of the side of lesion, was accompanied by focal disturbances of electrogenesis in left temporal leads. Vertigo was associated as a rule with focal pathological changes in parieto-occipital leads bilaterally,

From detection to decision: a conceptual framework of uncertainty migration in disorders of consciousness.

Accurate assessment of consciousness in patients with disorders of consciousness (DoC) remains a longstanding challenge in neurocritical care and neurorehabilitation. Although standardized behavioral assessments continue to serve as the clinical foundation for diagnosis, their reliance on overt behavioral responses may underestimate residual consciousness in a subset of patients. Recent advances in multimodal neuroimaging and neurophysiological techniques have substantially expanded the capacity to detect covert consciousness, reducing some of the inherent limitations of behavioral assessment
