Clinical characteristics and prognosis of adult orbital cellulitis in a tertiary general hospital.

Adult orbital cellulitis (OC) is a rare ophthalmic condition. This study aimed to identify risk factors influencing the prognosis of adult OC by evaluating the clinical characteristics and prognosis in an Asian population in southern Taiwan. In this retrospective study, OC patients were categorized based on the underlying causes of their condition: (1) preseptal cellulitis, (2) rhinosinusitis, (3) endophthalmitis, and (4) ocular implant-related infections. Prognosis was determined by changes in logarithms of the minimum resolution angle scored by the best-corrected visual acuity (LogMAR BCVA) …

Role of oncoplastic surgery in the management of primary orbital squamous cell carcinoma.

Primary squamous cell carcinoma (SCC) of the orbit is a rare and uncommon form of invasive epithelial malignancy, with few cases reported. Due to its rarity, treatment protocols remain undefined, and the therapy choices are debated. We present the case of a male in his early 50s from a low-income country who underwent treatment based on basic oncoplastic principles. He presented with a painful, blind right eye for 4 months, with clinical and imaging studies ruling out other primary malignancies. The diagnosis was locally advanced primary orbital SCC. A multidisciplinary team recommended orbita…

Orbital and medial rectus muscle involvement as initial presentations of hydatid disease.

PURPOSE: To report a rare case of orbital hydatid cyst involving the medial rectus muscle, which presented as progressive proptosis, with subsequent detection of liver involvement after further investigations. CASE PRESENTATION: We present the case of a 12-year-old boy from a rural area with exposure to wildlife dogs. The patient had a two-month history of gradually progressive proptosis in the right eye, accompanied by periorbital swelling and limited medial ocular motility. Orbital magnetic resonance imaging (MRI) revealed a large mass within the medial rectus muscle, which showed periphera…

Evaluating Zygomatic Implants in Orbital Floor Complications: A Clinical and Anatomical Study.

BACKGROUND: Sickle cell disease (SCD) is an inflammatory disease that can result in both chronic and acute inflammation. Immature granulocytes (IG) are not-yet-mature white blood cells that can be easily detected in complete blood count (CBC) tests. In recent studies it has been suggested that IG may play a role in determining the prognosis of inflammatory diseases. The aim of our study was to investigate the role of IG percentage on predicting acute chest syndrome (ACS) and the severity of vaso-occlusive crisis (VOC) in patients with SCD. METHODS: The study cohort consisted of 49 SCD patient

Optic nerve sheath enhancement on orbital MRI in giant cell arteritis.

BACKGROUND: Differentiating arteritic anterior ischaemic optic neuropathy (A-AION) due to giant cell arteritis (GCA) from non-arteritic anterior ischaemic optic neuropathy (NA-AION) may pose a diagnostic challenge. Our study aimed to assess the use of standard orbital MRI in distinguishing ocular manifestations of GCA from NA-AION. METHODS: This study included 25 consecutive patients (11 GCA, 14 NA-AION) who underwent contrast-enhanced orbital MRIs within 3 months of symptom onset. Two radiologists blinded to clinical data independently evaluated MRIs for the enhancement of the optic nerve sh…

Eyelid Arteriovenous Malformation With Orbital Fistula: Literature Review and Case Report.

Orbital arteriovenous malformations (AVMs) are rare entities, either congenital or acquired, that can significantly impair ocular function and aesthetics. Diagnostic evaluation requires multimodal imaging, and treatment is extremely complex, not only due to the limited information available in the literature, but also because of the high recurrence rate of the lesion, therefore requiring a multidisciplinary approach. Treatment involves a multimodal approach, often requiring AVM embolization followed by surgical excision. Endovascular embolization with agents such as ethylene-vinyl alcohol (Ony…

Inflammatory Bowel Disease-Associated With Orbital Myositis and an Internal Necrotizing Collection.

Inflammatory bowel disease (IBD) is an autoimmune inflammatory condition of the gastrointestinal tract and encompasses 2 major subtypes: Crohn's disease and ulcerative colitis. Orbital myositis is an uncommon ophthalmic extraintestinal manifestation of IBD and may be associated with an internal necrotic collection. The presence of a necrotic collection in orbital myositis narrows the diagnostic differential to several infectious and noninfectious etiologies. The distinction between an infectious and noninfectious etiology is of significant therapeutic consideration, as management differs great…

Retrospective Study for Intraoral Approach for Zygomaticomaxillary Complex and Orbital Floor Fracture.

OBJECTIVE: Traditional surgical approaches for zygomaticomaxillary complex (ZMC) and orbital floor (OF) fractures, such as transconjunctival or transcutaneous routes, carry risks of visible scarring and other complications, prompting the exploration of alternative techniques as the intraoral approach. This study hypothesizes that the intraoral approach can achieve comparable clinical outcomes to traditional methods while reducing complications associated with external incisions. METHODS: This retrospective study analyzed the outcomes of ZMC or OF fracture surgeries performed between January 2…

PEEK-on-PEEK orbital implantation: a secondary augmentation approach for delayed enophthalmos.

This case report discusses the long-term management of a patient's left zygomatic fracture with an associated orbital floor injury. The initial treatment involved open reduction and internal fixation (ORIF), followed by the placement of a polyetheretherketone (PEEK) implant for globe dystopia. Despite this intervention, the patient developed progressive secondary enophthalmos and globe dystopia. To correct this, a second PEEK implant was used to improve the position of the eye. The aim of this report is to present a novel technique of PEEK-on-PEEK implants to restore globe alignment. This case…

Unusual Presentation of Orbital Cellulitis as an Inflammatory Pseudotumor in a Kidney Transplant Recipient.

We reported the unusual presentation of a 62-year-old kidney transplant recipient with acute unilateral eye swelling and fever suggestive of severe orbital cellulitis with features of a space-occupying inflammatory lesion of the orbit. The clinical course deteriorated despite the therapy with broad-spectrum antibiotics alone. The computed tomography scan findings were significant for moderate thickening of the right superior rectus muscle and asymmetric enlargement of the superior ophthalmic vein. These features brought an orbital pseudotumor, or nonspecific orbital inflammation, to the differ…

Probable Ciliary Nerve Within Capsule of an Orbital Cavernous Venous Malformation.

Removal of an orbital cavernous venous malformation in a 39-year-old white female resulted in a significant intraoperative sectoral mydriasis. White fascicles were recorded as running into the mass at surgery, and one of these was confirmed histologically to be peripheral nerve within the capsule of a markedly fibrous orbital cavernous venous malformation. Most efferent pupillary anomalies after orbital surgery are probably due to ischemic or tractional neuropraxia, but the resected nerve raises the possibility of unplanned excision of a few fascicles of the postganglionic (parasympathetic) sh…
