Hip Labral Reconstruction: Lessons Learned from 1000 Cases.

Hip labral reconstruction has evolved into a reproducible and effective technique for managing irreparable labral tears. The knotless pull-through method eliminates graft sizing, simplifies passage, and ensures secure fixation. Based on experience with over 1000 cases, key lessons include meticulous rim preparation, consistent suture management, and the importance of graft availability even when repair is planned. Patient-reported outcomes and return-to-sport rates are consistently improved and comparable to labral repair, while reconstruction remains clearly superior to excision or debridemen

Case Report: Radical urethrectomy with partial cystectomy and bladder outlet reconstruction in giant female urethral adenocarcinoma infiltrating the bladder: A rare case report.

INTRODUCTION: Female urethral adenocarcinoma (FUA) is an exceptionally rare and aggressive malignancy, accounting for less than 0.02% of all cancers in women. Its nonspecific symptoms often lead to delayed diagnosis, with many cases detected at advanced stages. The rarity of FUA, particularly when presenting with a large mass, underscores the challenges in developing standardized treatment protocols. CASE PRESENTATION: A 65-year-old woman presented with urinary retention. Clinical examination revealed a large mass obstructing the urethral orifice. A computed tomography (CT) scan showed a mali

Resection of tracheal tumours and tracheal reconstruction under spontaneous breathing anaesthesia with a simple and practical intraoperative ventilation technique: a case report.

BACKGROUND: Tracheal tumours can be resected under spontaneous breathing anaesthesia (SBA) with cross-field endotracheal intubation. However, given the intermittent ventilation interruption during the anastomotic process, more convenient alternatives are being explored. In this context, our centre presents a case of tracheal tumour resection and reconstruction (TRR) involving a simple and practical intraoperative ventilation technique. CASE PRESENTATION: A 49-year-old man was admitted to our hospital and presented with pharyngeal discomfort and difficulty breathing for half a year. After discu

Mechanical Characterization of Slide Tracheoplasty Versus End-to-End Anastomosis in Ex Vivo Porcine Tracheas.

BACKGROUND: Slide tracheoplasty and end-to-end anastomosis are two common reconstructive techniques for airway stenosis. While slide tracheoplasty is often preferred for its larger luminal diameter and reduced anastomotic tension, its mechanical integrity relative to end-to-end repair remains incompletely characterized. This study aimed to compare tensile strength and elastic properties between the two repair techniques in an ex vivo porcine model. METHODS: Porcine tracheas were harvested, bisected, and reattached using either end-to-end anastomosis or slide tracheoplasty. Tensile testing was

Strategic four-dimensional reconstruction in cervical exenteration: A preventive reconstructive framework for anticipating and managing defects.

BACKGROUND: Anterior mediastinal tracheostomy (AMTr) following cervical exenteration creates massive, three-dimensional defects with high morbidity, particularly catastrophic great vessel rupture. To mitigate these risks, we developed a hierarchical conceptual framework categorizing reconstructive objectives into primary (immediate form and functional restoration), secondary (intraoperative complication prevention), and tertiary (postoperative functional and structural maintenance) goals. METHODS: We retrospectively reviewed 9 patients (mean age, 66.7 years) who underwent cervical exenteratio

Vascularized versus nonvascularized free fibular grafts in reconstruction of post-traumatic critical long bone defects, a comparative study.

BACKGROUND: Reconstruction of critical-sized long bone defects is a complex orthopedic challenge. Free fibular grafts, vascularized (FVFG) and nonvascularized (NVFG), are established reconstructive options, but comparative clinical outcomes remain uncertain. PURPOSE: To compare clinical and radiological outcomes of FVFG and NVFG in patients with post-traumatic critical bone defects more than 10 cm. METHODS: A randomized controlled trial was conducted with 50 patients assigned equally to FVFG or NVFG groups. The primary outcome was time to union, while secondary outcomes included graft hypert

Contemporary Review of Pelvic Fracture Urethral Injury Management and Outcomes.

As a life-threatening injury, the initial management of pelvic fractures focuses on trauma resuscitation. A secondary goal is the treatment of associated injuries, such as urethral damage, to prevent late complications because urethral stenosis has been observed in >90% of the patients. Additionally, urethral injury is often linked with erectile dysfunction and urinary incontinence. When analyzing the long-term outcomes of the initial management of complete urethral injuries, no significant impact on stenosis rates or the complexity of future urethral reconstruction is observed. The best time

Nasal Septum Reconstruction With Medial Femoral Condyle Free Flap: A Case Report and Literature Review.

Nasal septum defects cause significant functional and aesthetic morbidity. In large septal defects requiring reconstruction, the goal is to achieve appropriate tissue matching, obtain well-vascularized tissue, and minimize donor site morbidity. Repair alternatives are quite limited in large defects. The medial femoral condyle (MFC) free flap is a preferred option in craniofacial reconstructions due to its vascularized, cortical bone with periostal architecture. In this article, we present a patient who underwent successful reconstruction of a severely advanced nasal septum defect using a media

Optimal approaches to flap reconstruction following abdominoperineal resection: A systematic review.

PURPOSE: Abdominoperineal resection (APR) frequently results in a large volume perineal defect. Flap reconstruction is commonly undertaken to reduce the rate of perineal complications associated with primary closure. Several techniques can be employed including vertical rectus abdominis (VRAM), gluteal myocutaneous and gluteal fasciocutaneous flaps. We aimed to compare perineal complication rates between flap reconstruction techniques. METHODS: A systematic review was conducted following PRISMA guidelines. Databases were searched for studies reporting perineal complications following flap rec

[Perioperative Assessment and Management of Hypercoagulability and Thrombophilia in Microsurgery: Consensus Report of the German-Speaking Society for Reconstructive Microsurgery (GSRM)].

Uninterrupted blood flow through microsurgically anastomosed vessels is crucial for the postoperative success of reconstructive microsurgery. Contrary to the highly standardized anastomosis techniques, the pivotal partner discipline of evidence-based microsurgical haemostaseology and haemorrheology is still in its infancy. Prospective clinical studies yielding evidence-based recommendations are notably lacking. Currently, perioperative management in microsurgery is based solely on site-specific empirical experience. The collective aim of these diverse efforts is the preoperative identification
