Attachment as a mechanism between childhood maltreatment and subclinical psychotic phenomena: Results from an eight-year follow-up study.

We previously reported that fearful attachment mediated associations of childhood maltreatment with subclinical psychotic phenomena. At an eight-year follow-up, we aimed to replicate and extend this finding by examining the mediating role of disorganized attachment. Participants were 169 young adults who completed baseline and eight-year follow-up questionnaires. Physical/emotional maltreatment was prospectively associated with paranoid beliefs and positive schizotypy. For paranoid beliefs, fearful and disorganized attachment were significant mediators in separate models, whereas only disorgan

[Neurosyphilis: a historical disease with current relevance].

In the last few years, an increasing worldwide incidence in syphilis has been reported, mostly in the United States and Western Europe. Neurosyphilis is characterized by a wide differential diagnosis, which too often causes the disease to remain undetected for a long time. We report a case of a male patient with manic psychotic symptoms, in whom neurosyphilis was identified after elaborate diagnostic investigations. We give an overview of the disease process and correlations with psychiatric symptoms, diagnostics, screening and treatment.

Defining Mental and Behavioural Disorders in Genetically Determined Neurodevelopmental Syndromes with Particular Reference to Prader-Willi Syndrome.

Genetically determined neurodevelopmental syndromes are frequently associated with a particular developmental trajectory, and with a cognitive profile and increased propensity to specific mental and behavioural disorders that are particular to, but not necessarily unique to the syndrome. How should these mental and behavioural disorders best be conceptualised given that similar symptoms are included in the definition of different mental disorders as listed in DSM-5 and ICD-10? In addition, a different conceptual framework, that of applied behavioural analysis, has been used to inform intervent

Free thyroxine and thyroid-stimulating hormone in severe mental disorders: A naturalistic study with focus on antipsychotic medication.

BACKGROUND: Disturbances in thyroid function have been associated with use of psychotropic drugs, including antipsychotics. Still, the thyroid function in relation to commonly prescribed antipsychotic drugs and polypharmacy is not fully known. We investigated thyroid function associated with use of antipsychotics in patients with psychotic disorders compared with healthy controls. METHODS: We included 1345 patients and 989 healthy controls from the Thematically Organized Psychosis (TOP) study, recruiting participants between 18 and 65 years of age in the Oslo-area. All patients underwent a th…

Mothers with acute and chronic postpartum psychoses and impact on the mother-infant interaction.

BACKGROUND: Maternal postpartum psychoses pose a serious risk to the mother-infant interaction. It is unclear how different subtypes of post … mory processing, mind-mindedness, and affect recognition, that may partially explain the effects of psychotic disorders. An acute onset of psychosis during the postpartum period (de novo or relapse) was typically related to better mother-infant interactions. Mothers with schizophrenia have the highest risk of child displacement, and interventions by social services w …

Pharmacological treatment for psychotic depression.

BACKGROUND: Evidence is limited regarding the most effective pharmacological treatment for psychotic depression: combination of an antidepressant plus an antipsychotic, monotherapy with an antidepressant or monotherapy with an antipsychotic. This is an update of a review first published in 2005 and last updated in 2009. OBJECTIVES: 1. To compare the clinical efficacy of pharmacological treatments for patients with an acute psychotic depression: antidepressant monotherapy, antipsychotic monotherapy and the combination of an antidepressant plus an antipsychotic, compared with each other and/or

Hommage to reactive psychosis.

When looking back in the history of psychiatry, the concept of reactivity had very variable impact on ethiological considerations. If classification of mental disorders is taken as a proxy, it seems that this impact is presently at the lowest that it has been in the last hundred years. The author gives a short historical overview of concept of reactive psychosis, followed by a description of the current state of the art. He pleads for a rethinking of the current position as stated in ICD-10. Reintroduction of a certain degree of the reactivity concept into the classification seems to be requir

[Central noradrenergic system in psychiatry].

The function and dysfunction of the central noradrenergic system were reviewed together with the implications for the pathophysiological understanding of functional psychosis. Like the peripheral noradrenergic system, the central noradrenergic system plays a pivotal role in fight/flight reactions and stress. Overactivity of the system induces not only a sympathotonic state but also hyperarousal accompanied by insomnia, anxiety, irritability, emotional instability, paranoia, and excitation. On the other hand, its underactivity lowers the sympathetic tone and arousal level, resulting in hypersom

Dopaminergic dysbalance in distinct basal ganglia neurocircuits: implications for the pathophysiology of Parkinson's disease, schizophrenia and attention deficit hyperactivity disorder.

The basal ganglia form a forebrain system that collects signals from a large part of the neocortex, redistributes these cortical inputs both with respect to one another and with respect to inputs from the limbic system, and then focuses the inputs of this redistributed, integrated signals into particular regions of the frontal lobes and brainstem involved in aspects of motor planning and motor memory. Movement disorders associated with basal ganglia dysfunction comprise a spectrum of abnormalities that range from the hypokinetic disorder (from which Parkinson's disease, PD, is the best-known-e

Bipolar depression: the role of atypical antipsychotics.

Acute manic episodes in bipolar disorder require rapid and effective relief. Bipolar depression is a major component of the bipolar disorder spectrum. Existing treatment options for bipolar depression include lithium, lamotrigine and conventional antidepressants. However, lithium is more effective in treating mania or hypomania than depression, both acutely and prophylactically, lamotrigine has only been demonstrated to be effective in one adequately powered study in acute bipolar I depression, and conventional antidepressants have been associated with emergent mania and cycle acceleration. [Clinical aspects of "folie du doute"].

"Folie du doute" related to obsessive disorders manifests with doubts about managing habitual actions, reality of what is happening and existence of surrounding subjects; ruminations, indecision emerged as incapability to choose a definite behavior line. Sixty-six patients, 27 men and 39 women (mean age 31.3 years) have been examined. Three clinical "folie du doute" variants were distinguished: "repeated control" obsessions (doubts are realized by re-control of the actions committed); obsessive doubts defined by contrastive ideas of expedience for choosing the approach contrastive to the taken

Premorbid personality and psychopathological dimensions in first-episode psychosis.

BACKGROUND: Broadening our knowledge of the relationship between personality and psychopathological dimensions in psychosis would provide insights into the nature of their underlying pathophysiology. Research, to date, has been carried out in chronic samples and the possibility that the personality assessment may have been contaminated by the psychotic episode cannot be ruled out. The aim of this study was to examine the relationships between personality and psychopathology in a 'first-episode psychosis' sample using a dimensional approach. METHOD: Premorbid personality dimensions of 94 conse

"Living turned inside out": the musical expression of psychotic and schizoid experience in talking heads' Remain in light.

The rock album form, advantageously suited for the artistic expression of intrapsychic experience, has been virtually ignored by analytic writers. Remain in Light by Talking Heads is presented as an example of an album that effectively utilizes the potentialities of this form to give powerful and disconcerning musical expression to a variety of psychotic and schizoid experiences, bringing to life the formulations for these phenomena of Bion, Winnicott, Guntrip, Ogden, Grotstein and others. The album suggests a variety of mechanisms by which sound and music may serve both defensive and compensa

[Inhalant abusers and psychiatric symptoms].

There are different opinions about the cause of chronic psychiatric symptoms observed in drug abusers between Japanese and foreign psychiatrists. The Japanese seem to recognize the chronic psychosis as the result of drug abuse. In the other hand, foreigners diagnose these cases as dual diagnosis of drug abuse and psychosis. Authors studied the problem in this research. One of the authors has examined 120 inhalant abusers of all, in- and out-patients in Kanagawa Prefectural Center of Psychiatry, Serigaya Hospital from 1991 to 1995. These patients were classified into three groups: psychosis gro

The emerging role of valproate in bipolar disorder and other psychiatric disorders.

Although valproate, a simple branched-chain fatty acid, is generally considered to be an antiepileptic agent, a large literature dating back to 1966 describes its use in primary psychiatric disorders. The significant role that gamma-aminobutyric acid plays in mood provided the rationale to examine valproate in this regard. Numerous uncontrolled as well as placebo- and lithium-controlled studies verified the drug's efficacy in the short-term management of bipolar and schizoaffective disorders. The response appears to be independent of response to traditional therapies such as lithium, neurolept

Psychosis, aggression, and self-destructive behavior in hospitalized adolescents.

The authors studied the history of aggressive and self-destructive behaviors in psychotic and nonpsychotic hospitalized adolescents (N = 137). A multidimensional measure of self- and other-directed aggression was retrospectively applied to each patient's social and developmental history. Nonsignificant gender and diagnostic differences were obtained on ratings of violence and suicide. Broader definitions of internal and external aggression yielded nonsignificant diagnostic differences, but gender differences were observed on both internal and external aggression measures. Females displayed gre
